Kluwer style files ("get" the following files the same way you did README):

Monograph
---------------
Documentation:  kluwerbk.doc
Sample file: booksamp.tex

Main Style file:  kluwerbk.sty
Optional font file: psfonts.sty

Edited Book
-----------------
Documentation:  editedbk.doc
Sample file:  edbksamp.tex

Main Style file:  kluwerbk.sty, editedbk.sty
Optional font file: psfonts.sty

Note, PostScript fonts have different names on different systems, so
psfonts.sty file may need to be edited to match the fonts on your system.

IMPORTANT:  Plese make sure you are using the most recent .sty files.

IMPORTANT:  please make sure that your chapter openers begin on
an odd numbered page.  The .sty file should force all chapter 
openers onto an odd numbered page; however, if it does not please
contact prod@world.std.com
