QUESTION: A 35-year-old woman attending my clinic in the 20th week of pregnancy had a lump in her breast. Open biopsy revealed ductal carcinoma. Radical mastectomy with axillary lymph node biopsy showed three out of 10 nodes positive. What are the short- and long-term implications of chemotherapy during pregnancy? ANSWER: Patients with breast cancer during pregnancy should be treated using the same criteria as their nonpregnant counterparts, with the modifications required by pregnancy. The benefits of the diagnostic workup and use of chemotherapy, radiotherapy, and surgery have to be weighed carefully against their risk to the unborn baby.

A sensitive technique using reverse transcriptase-polymerase chain reaction (RT-PCR) for PSA mRNA has been used to detect circulating tumor cells in the peripheral blood of prostate cancer patients. We evaluated the clinical utility of this method for staging and monitoring for prostate cancer. The number of patients who were RT-PCR positive was increased in higher clinical stages. This technique may provide useful information for treating patients with prostate cancer, especially candidates for radical prostatectomy. Confirming the value of this modality as a prognostic factor will require further study.

The heat shock proteins (hsp) are ubiquitous molecules induced in cells exposed to sublethal heat shock, present in all living cells, and highly conserved during evolution. Their function is to protect cells from environmental stress damage by binding to partially denatured proteins, dissociating protein aggregates, to regulate the correct folding, and to cooperate in transporting newly synthesized polypeptides to the target organelles. The molecular chaperones are involved in numerous diseases, including cancer, revealing changes of expression. In this review, we mainly describe the relationship of hsp expression with human cancer, and discuss what is known about their post-translational modifications according to malignancies.

In eukaryotes, 3-hydroxy-3-methylglutaryl coenzyme A (HMG-CoA) reductase is a key enzyme that catalyses the synthesis of a precusor of cholesterol as well as non-sterol isoprenoids, mevalonate. The regulation of the enzyme activity occurs at the transcription, post-transcription, translation and protein degradation levels. Cholesterol contributes to the generation and development of atherosclerosis while non-sterol isoprenoids play a role in regulation of cell proliferation, signal transduction and generation of cancers. At present, the enzyme is the target of several drugs effective against atherosclerosis.

We report a case of successful liver transplantation in a young man with metastatic hemangiopericytoma confined to the liver. He had a primary retroperitoneal hemangiopericytoma removed in 1982 and was diagnosed as having liver metastases 8 years later. Subsequently, he developed hypoglycemia, which did not respond to chemotherapy. Hepatectomy and transplantation resolved his hypoglycemic attacks. He remained well and symptom free for 4 years. Liver transplantation can provide effective palliation and should be considered in carefully selected patients with localized metastatic cancer.

Intestinal obstruction is a major complication of colorectal cancer. Acute surgical decompression frequently requires subsequent operative interventions and is associated with mortality in more than 3% of the cases. Transanal metallic stenting is now possible to perform on an outpatient basis, thus providing quick symptom relief and the opportunity to cleanse the bowel for work-up or surgery. This stenting provides temporary relief for patients and is gaining acceptance rapidly. Randomized controlled trials are needed to help pinpoint refinements that can minimize the difficulty of stent insertion.

Fluconazole pretreatment had no effect on doxorubicin pharmacokinetics.
