Do you get the
correct behaviour with 11" under 1.96? How about 11" under 1.94? We
need to know what triggered the breakage, and when. Shouldn't matter 'NY
Ballot' or otherwise. Just dig up any old 11"
database.

When a ballot has triggered the AVOS as being
unacceptable, for whatever reason, the AF stops and the ballot cleared, but
when AF lid is lowered, the AF will still try to feed the next ballot, even
though the AVOS has not been cleared.
It appears as though there is no communication
from the AVOS to the AF to stop until the operator touches the YES
key.
