MINDY.HMP is the conversion template I developed for HTML Transit to convert
stuff into HTML.

The *.fm files are nominally the source.  The .mifs are needed by HTML Transit,
and as near as I can tell the only way to generate a MIF is to manually zot
thru 3 menus for each file, so I kept them around.  Also, the foo-extensions
mifs are slightly different, in that the title has no - in it, so that the text
button bar in the HTML looks better.

The .doc files are generated table of contents and index files.  Though the
book hangs important information off their names.

everything.book is supposed to format everything with an index and stuff, but I
just broke it by flattening the directory hierarchy, so there are currently
unresolved references.  Ah, for a primitive formatter where references are via
names!

I forgot to convert format-out into HTML. oops.

itime.fm is not in the book because it's broke.
