A 53-year-old man admitted for thirst, polyposia, and polyuria. Large cell lung cancer T3N2M1 (Stage IV), and central diabetes insipidus caused by pituitary metastasis of lung cancer, were diagnosed. We gave him desmopressin acetate, and chemotherapy with paclitaxel and carboplatin. But pituitary metastasis increased and invaded the hypothalamus. After irradiation of the pituitary and hypothalamus, metastatic focus diminished and desmopressin acetate was tapered.

There is increasing evidence that oral clodronate has important clinical benefits in early breast cancer. Longer-term follow-up data from two out of three randomized studies have shown that adjuvant clodronate improves survival. This article outlines the results of these studies and discusses the potential role for other bisphosphonates, such as ibandronate and zoledronic acid, in the adjuvant treatment of breast cancer.

RET is the receptor for glial-derived neurotrophic factor growth factors. It is a paradigm of a single gene that causes different types of human cancer when targeted by different genetic alterations. Like other receptor tyrosine kinases, once activated, RET recruits a variety of signaling molecules that mediate biological responses. Here we review data on the signaling pathways that lead to RET-mediated cell transformation and recent evidence that manipulation of RET holds promise for thyroid cancer treatment.

We treated a 67-year-old female with hypopharyngeal cancer (T2N0) in the posterior wall, who had a complete response after two cycles of chemotherapy with docetaxel followed by radiation. Five years after this therapy she has had no recurrence and no metastasis, with preserved laryngeal framework and function. Docetaxel is useful for treating early head and neck cancer.

Fibromatosis of the breast is a rare benign tumor that should be included in the differential diagnosis for breast cancer. It is usually indistinguishable from malignancy on ultrasound, mammography, physical examination, and on gross evaluation. Distinction is easily made by histologic findings. This benign tumor does not metastasize, but is locally aggressive and tends to recur postoperatively, which accounts for considerable morbidity. We present two cases and a discussion from the perspective of the radiologist, the surgeon, and the pathologist.

Chemoradiotherapy has become the standard treatment for patients with locally advanced non-small-cell lung cancer on the basis of several large randomized trials. Despite an increase in median survival from 10 months with radiotherapy alone to 16 to 17 months with concurrent chemoradiotherapy, long-term survival in this disease remains modest at best. With the advent of new biologic agents targeting specific cellular pathways associated with malignant progression, combined-modality therapy has the potential to target tumors selectively with less toxicity.

Stubborn and extensive dermatoses, such as psoriasis vulgaris or atopic dermatitis (eczema), are indications for phototherapeutic methods. These include UVB phototherapy, UVA photochemotherapy (PUVA), UVA irradiation following prior photosensitization, UVA, treatment and combination treatments. Phototherapy is contraindicated in the case of light-induced dermatoses. Caution must also be exercised in patients receiving photosensitizing medications. When establishing the indication, consideration must be given to the treatment-related increased risk of developing skin cancer. The patient must also be informed of such undesired potential side effects as burns and premature aging of the skin.

Pancreatic resection offers the potential for long-term cure in 15% of patients with pancreatic cancer. This article describes the author's technique of pancreaticoduodenectomy (PD), together with guidelines for disease staging, pre-operative work-up and patient selection. The role of neo-adjuvant and adjuvant chemotherapy is currently under evaluation and all patients who have a curative resection should be considered for entry into the ESPAC 3 trial that aims to establish the definitive role of adjuvant chemotherapy in pancreatic cancer.
