Helicobacter pylori causes peptic ulcer disease and some forms of gastric cancer; it is one of the most common chronic bacterial infections of humans. Although several prototype protein-based vaccines have shown promising results, they have not cleared infection and/or prevented reinfection. Nucleic acid vaccination offers a useful alternative to protein immunization, especially now that two complete H. pylori genome sequences are available, and facilitates the selection of antigenic targets.

Nonhormonal antineoplastic therapy can affect the endocrine system with consequent effects on the growth of hormone-sensitive tumors. Amongst anthracycline antibiotics, we have found daunorubicin and epirubicin able to acutely stimulate prolactin (PRL) secretion both in vivo, in the rat, and in vitro, from rat anterior pituitary cell cultures. Despite a similar structure, doxorubicin showed no such activity. Considering the possible role of PRL in breast cancer cell proliferation, the effects of certain anthracyclines might be viewed as an adverse drug reaction involving the anterior pituitary gland.

TITLE: A case of rapidly progressing small cell lung cancer incidentally found during the course of renal failure.

A 65-year-old man with rapidly progressing small cell lung cancer found in the course of renal failure is reported. The patient had a medical history of hypertension, diabetes mellitus, and cardiovascular disease. Hemodialysis was introduced following renal failure, but pneumonia resulted in a transient exacerbation and his complaint of general fatigue did not improve. Examination for the fatigue revealed no apparent abnormalities. Three months later, he died of small cell lung cancer.

Langerhans cell histiocytosis (LCH) is a clonal proliferation of Langerhans cells of unknown etiology that results in a range of clinical manifestations. LCH has been known to be associated with a variety of malignant diseases. A 7-year-old boy was treated for standard-risk acute lymphoblastic leukemia (ALL) at age 2 years, on a Children's Cancer Group chemotherapy protocol for 3 years and developed LCH 2 years after completion of chemotherapy. The case and a review of literature on the association of LCH and ALL are presented.

This review explores interactions between classes of compounds, including anti-estrogens, retinoids, monoterpenes and tyrosine kinase inhibitors, that are effective independent, and how their synergistic interaction could be exploited in cancer treatment. The effects of these compounds on insulin-like growth factors (IGF) and transforming-growth factor-beta (TGF-beta) will also be examined.

Cancers metastatic to the eye are the most common intraocular malignancy, the choroid being the most common site of affection. Breast and lung cancers are the two most common malignancies metastatic to the eye. Patients usually present with diminished visual acuity of field loss. Management of these cases is very gratifying with a large majority of patients showing resolution and maintenance of useful vision in the involved eye. This communication presents our experience in management of a case of choroidal metastasis from a breast primary. A combination of radiotherapy and chemotherapy lead to complete resolution of the lesion with no recurrences in a follow-up of over 6 months.

A general review of the results and technique of intraarterial chemotherapy in cases of breast cancer is given. The remission rate for untreated primary tumors is nearly 100%, for completely treated local relapses approximately 70%. The complication rate is quite low using an intraarterial well-tolerated cytostatic agent such as mitoxantrone. However, thromboembolism of the vertebral artery facing the internal mammary artery may occur. The indication for intraarterial chemotherapy should always be set by an interdisciplinary board.

Gynecologic malignancies account for 15% of all cancer diagnosis in women. Primary lymphatic spread is well recognized in vulvar, cervical, uterine, and ovarian carcinomas. Vulvar carcinoma spreads locally to the inguinofemoral lymph nodes in a relatively predictable fashion similar to the local spread of breast carcinoma. Lymphatic mapping using radioactive colloid should provide adequate means to sample these nodal basins while attempting to reduce postoperative morbidity. Methods of vulvar lymphoscintigraphy are described.
