From June-78 to December-89, 300 patients with histologic diagnosis of non-small cell lung cancer were treated with radiotherapy by different indications: radical intention, post-chemotherapy, palliative intention, symptomatic and postsurgery. It was employed an electrons lineal accelerator principally, and for symptomatic radiotherapy treatment was used telegammatherapy by cobalt-60. It was treated an retrospective non comparative study which it has analyzed the survival of the different radiotherapy indications.

TITLE: [Are silicone breast prostheses dangerous?]

Since 1964 silicone breast prostheses have been implanted in 5 million women for breast reconstruction after cancer operations, to correct congenital deformities and for cosmetic reasons. There is no evidence in the literature of a correlation between silicone implants and breast cancer. Some authors claim that silicone implants reduce the reliability of mammography. Autoimmune diseases in patients with silicone mammary implants are reported in less than 40 cases. Most of these are localized or systemic scleroderma. An association with silicone implants has been suggested, but this suggestion has not been confirmed by scientific data.

Differential hybridization analysis revealed two cDNA clones, pBUS19 and pBUS30, to be overexpressed in progressionally advanced rat prostatic tumors. Northern blot analysis suggested the clones to be related although no homology in nucleotide sequence could be shown. Isolation and characterization of a pBUS19-related clone, pJG116, and computer-assisted database comparison showed that all three clones could be mapped within a rat-specific endogenous retrovirus. The importance of overexpression of retroviral sequences in advanced prostatic cancer remains unclear.

Free fasciocutaneous forearm flaps, based on the radial artery, were used to replace the tissue defect caused by pull-through operations for cancer of the floor of the mouth in 11 patients. There was no total or partial flap failure. The greatest advantage of the method is that restriction of the tongue movement is less when compared with the results of other methods. The thin forearm flaps are adaptable and pliable. The newly developed lower alveolar crest and the sublingual sulcus make prosthetic rehabilitation possible.

Tenascin immunohistochemical staining was investigated in 82 patients with primary invasive breast carcinoma followed for at least 5 years after surgery; the staining was analyzed in relation to clinicopathological factors. An intense tenascin staining pattern was noted in 59 patients (72%), but no definite correlation was found between tenascin positivity and clinical stage, lymph node metastasis, or estrogen receptor status. However, tenascin-positive patients had a significantly better prognosis than tenascin-negative patients. Tenascin staining may therefore be a useful marker for predicting the survival of breast cancer patients.

Maxillary sinus cancers are rare as compared with cancers of other sites. Because the onset is insidious, they are at an advanced stage at diagnosis and therefore present a therapeutic challenge for local control. Traditionally, treatment has been surgery or irradiation, or a combination of the two. Local control and survival, however, remains poor. Numerous chemotherapeutic agents have shown activity against squamous cell cancers of the head and neck. In several studies the use of either neoadjuvant or concurrent chemotherapy has improved local control and survival when compared with historical controls. Randomized studies are needed to confirm the benefit of chemotherapy over the standard forms of surgical and radiation therapy.

TITLE: Two sisters with coeliac disease and jejunal cancer: just a coincidence?

Two sisters with asymptomatic coeliac disease are described; they both developed a primary jejunal cancer at the same age. While screening the family, a third sister was found to have coeliac disease, but without detectable cancer in the small intestine or stomach. These findings suggest an increased susceptibility in this family for carcinoma developing secondary to asymptomatic coeliac disease. We conclude that the finding in a patient of the combination of coeliac disease and malignancy in the digestive tract is sufficient reason to investigate the first-degree relatives with regard to the presence of coeliac disease and a secondary carcinoma.

A case is presented of endometrioid carcinoma of the ovary, metastasizing to the breast in a 63-year old woman. Differentiation of metastatic cancer to the breast for primary breast carcinoma and discovering the primary tumor site are rather important for treatment and prognosis. Lumpectomy, followed by panhysterectomy, was performed and six courses of cisplatinum and cyclophosphamide were given. No signs of recurrence or metastasis are apparent 16 months after the discovery of the breast lesion.

The magnetic resonance imaging (MRI) findings in a 42-year-old woman with a giant Krukenberg tumor from gastric cancer are reported. The macroscopic features of the Krukenberg tumor resulting from gastric cancer were clearly and precisely shown on MRI. Furthermore, MRI was more useful than ultrasonography or CT for determining the origin and further tissue characterization of the tumor. These findings suggest that MRI can be especially useful in the evaluation of giant ovarian tumors as in the present case.

In some families breast cancer aggregates as the predominant site-specific neoplasia and in others in association with defined malignancies. In the case of familial adenocarcinomatous (Lynch II-syndrome) it occurs together mainly with colorectal and endometrial carcinoma whereas in the case of the Li-Fraumeni/SBLA-syndrome it belongs to a wider spectrum including sarcoma, brain tumors, lung and laryngeal cancer, leukaemia and adrenocortical carcinoma. The occurrence of these malignancies is reported as they were observed in the families of 600 women suffering from breast cancer.

The identification of new agents active against small-cell lung cancer (SCLC) remains a high priority for clinical investigators. Recently, 2 very different approaches have been used to evaluate new agents in this disease. Some groups have opted to test new agents in previously untreated patients with extensive-stage SCLC, and the second approach is to continue to test new agents only in previously treated patients. Abdominal and particularly vaginal sonography are useful diagnostic techniques for evaluating endometrium. Endometrial thickness can be measured and the echo pattern of the different parts of endometrium analysed. During normal cycles a triple line sign is typical for the late proliferative and periovulatory phase. In the secretory phase the endometrium is echogenic and posterior enhancement of echoes can be seen. It is also useful to control the effects of hormonal treatments or the endometrium using sonography. In postmenopausal women endometrial thickness of 4-5 mm or more is abnormal and further evaluations are indicated. In endometrial cancer sonography is a reliable way to estimate myometrial invasion.
