TITLE: Urinary beta-glucuronidase in screening and followup of primary urinary tract malignancy.

PURPOSE: The usefulness of urinary beta-glucuronidase in screening and followup of urinary tract malignancy was tested. MATERIALS AND METHODS: The activity and concentration of urinary beta-glucuronidase were determined in 80 healthy subjects, 113 urology patients with no cancer, and 60 with transitional cell and 23 with renal cell carcinoma. Patients with cancer were followed for 3 to 33 weeks. RESULTS: The enzyme activity and concentration were significantly greater in the cancer group, including residual tumor or recurrence after therapy, than in the controls with excellent discrimination. CONCLUSIONS: Urinary beta-glucuronidase was indeed useful in screening and followup of urinary tract malignancy.

From June-78 to December-89, 300 patients with histologic diagnosis of non-small cell lung cancer were treated with radiotherapy by different indications: radical intention, post-chemotherapy, palliative intention, symptomatic and postsurgery. It was employed an electrons lineal accelerator principally, and for symptomatic radiotherapy treatment was used telegammatherapy by cobalt-60. It was treated an retrospective non comparative study which it has analyzed the survival of the different radiotherapy indications.

A 89-year-old woman was referred to our hospital because of a relatively defined shadow of the right peripheral lung in chest roentgenograms. Although this case couldn't be diagnosed with transbronchial lung biopsy, adenocarcinoma of the lung (c-T1N0M0) was suspected because of radiological findings. We planned video-assisted pulmonary partial resection of the S6 in consideration of age and general status. Her postoperative courses was uneventful with no complications. Video-assisted thoracoscopic surgery is one of the treatments for elderly patients with peripheral early stage lung cancer.

The difficulties associated with the analysis of the longitudinal QOL data, and the contributions that the QOL endpoint made to the understanding of treatment differences, will be the focus of this paper.

Quality mammography remains the primary modality used to image the breast. Sonography currently is the most useful adjunctive technique to mammography, having roles in differentiating cystic from solid masses and in guiding interventional procedures. This article reviews these and other indications for breast ultrasound studies, including evaluation of palpable masses not visible in radiographically dense breasts, masses not completely evaluable with mammography, young patients especially susceptible to radiation damage, and abscesses. Ultrasound interpretation of cysts and solid masses are discussed, as are the relative disadvantages of ultrasound as a breast cancer screening tool.

TITLE: [Are silicone breast prostheses dangerous?]

Since 1964 silicone breast prostheses have been implanted in 5 million women for breast reconstruction after cancer operations, to correct congenital deformities and for cosmetic reasons. There is no evidence in the literature of a correlation between silicone implants and breast cancer. Some authors claim that silicone implants reduce the reliability of mammography. Autoimmune diseases in patients with silicone mammary implants are reported in less than 40 cases. Most of these are localized or systemic scleroderma. An association with silicone implants has been suggested, but this suggestion has not been confirmed by scientific data.

Differential hybridization analysis revealed two cDNA clones, pBUS19 and pBUS30, to be overexpressed in progressionally advanced rat prostatic tumors. Northern blot analysis suggested the clones to be related although no homology in nucleotide sequence could be shown. Isolation and characterization of a pBUS19-related clone, pJG116, and computer-assisted database comparison showed that all three clones could be mapped within a rat-specific endogenous retrovirus. The importance of overexpression of retroviral sequences in advanced prostatic cancer remains unclear.

The combined cause-of-death category pneumonia and influenza (P&I) (International Classification of Diseases, Ninth Revision, codes 480-487) ranks as the sixth leading cause of death in the United States following heart disease, cancer, stroke, unintentional injuries, and chronic obstructive pulmonary disease. Changes in the epidemiology of Streptococcus pneumoniae and other recognized respiratory pathogens, the increasing occurrence of drug-resistant microorganisms, and the detection of new respiratory pathogens have heightened awareness of the public health importance of severe respiratory infections. To characterize the epidemiology of P&I deaths in the United States, CDC further analyzed underlying and multiple cause-of-death mortality files for 1979-1994. This report summarizes the results of this analysis.

An analysis of the clinical symptoms has been carried out in the group of 110 patients who underwent nephrectomy for a clear-cell carcinoma. Total hematuria (47.3% of patients) has been a predominant symptom of the tumour. Pain felt in the back has been noted in 39.1% of patients. Palpable tumour has been noted in 34.5% of patients. Such symptoms have been present simultaneously (Israel's triad) in 12.3% of patients. Tumour has been diagnosed accidentally, mainly with USG, in 37.3% of patients. It has been found that classic clinical symptoms of this cancer are nowadays less frequent than before introducing ultrasound examination.

This paper reviews the noncontraceptive benefits and therapeutic uses of depot medroxyprogesterone acetate (DMPA). Relevant articles were reviewed using a computerized Medline search of the literature from 1966 to 1995. Good evidence shows that DMPA use is associated with reduced iron-deficiency anemia, protection against pelvic inflammatory disease, protection from endometrial cancer and improved hematologic parameters among users with sickle cell disease. More studies are needed to fully assess DMPA's impact on other disorders.

A 50-y-old male patient with prostate cancer showed two suspicious lesions in bone scintigrams. They were assessed to be bone metastases by biopsy of the os ilium. After i.v. injection of 99mTc-HMDP a probe-guided localization permitted optimal surgical treatment. A bone metastasis in the os ilium was confirmed. The intraoperative detection with a hand-held gamma probe permitted accurate and complete excision of the tumour; the bone defect could thus be reduced to a minimum.

Free fasciocutaneous forearm flaps, based on the radial artery, were used to replace the tissue defect caused by pull-through operations for cancer of the floor of the mouth in 11 patients. There was no total or partial flap failure. The greatest advantage of the method is that restriction of the tongue movement is less when compared with the results of other methods. The thin forearm flaps are adaptable and pliable. The newly developed lower alveolar crest and the sublingual sulcus make prosthetic rehabilitation possible.

Efforts to target ovarian cancer with antibodies directed against tumor-associated antigens have begun to take advantage of molecular engineering techniques and have also explored an expanded panel of radionuclides with improved conjugation technology. It is hoped that these efforts will translate into successful strategies to improve the long-term survival of patients in conjunction with conventional chemotherapy.

Tenascin immunohistochemical staining was investigated in 82 patients with primary invasive breast carcinoma followed for at least 5 years after surgery; the staining was analyzed in relation to clinicopathological factors. An intense tenascin staining pattern was noted in 59 patients (72%), but no definite correlation was found between tenascin positivity and clinical stage, lymph node metastasis, or estrogen receptor status. However, tenascin-positive patients had a significantly better prognosis than tenascin-negative patients. Tenascin staining may therefore be a useful marker for predicting the survival of breast cancer patients.

Maxillary sinus cancers are rare as compared with cancers of other sites. Because the onset is insidious, they are at an advanced stage at diagnosis and therefore present a therapeutic challenge for local control. Traditionally, treatment has been surgery or irradiation, or a combination of the two. Local control and survival, however, remains poor. Numerous chemotherapeutic agents have shown activity against squamous cell cancers of the head and neck. In several studies the use of either neoadjuvant or concurrent chemotherapy has improved local control and survival when compared with historical controls. Randomized studies are needed to confirm the benefit of chemotherapy over the standard forms of surgical and radiation therapy.
