TITLE: Malignant pleural mesothelioma among Swiss furniture workers. A new high-risk group.

Within the framework of a research project concerning occupational mortality, attention was drawn to a highly significant standardized mortality ratio for mesothelioma among furniture workers. From information drawn from the mortality records for the years 1979-1985 and from the 1980 national census, the number of pleural mesotheliomas among this occupational group was examined. The expected number of deaths from pleural mesothelioma among Swiss furniture workers for this period was 4.4; the observed number was 12. This finding indicates a 2.7-fold relative risk (P = 0.004) for death due to these cancers among Swiss furniture workers.

TITLE: Correlation of ascitic fluid cytology with histologic findings before and after treatment of ovarian cancer.

One hundred thirteen specimens of ascitic fluid from 97 patients with primary, persistent, or recurrent ovarian cancer were examined cytologically. Advanced stage of disease, involvement of the ovarian surface, a moderate or large volume of fluid, and nonbloody serous ascites were factors that correlated with a high rate of positive findings in ascites in these patients. Other factors influencing the positive rate appeared to be the invasive as opposed to borderline malignant histology of the tumor. Involved external surface and volume of ascites were influencing factors only in advanced cases. Therefore, the most important factor influencing the positive rate of ascites cytology was the proportion of cases in advanced stages to total cases.

TITLE: Adjuvant chemotherapy in advanced head and neck cancer. An update.

Two courses of triple-agent chemotherapy were given to 47 patients with advanced head and neck cancer. All but one patient with a nasopharyngeal carcinoma had resectable stage III and IV squamous cell carcinoma. Eighty-eight percent of the patients responded to the chemotherapy with a 50 percent or greater decrease in tumor size. Forty-three patients underwent surgical resection and 19 were histologically upgraded to stage II or better (T less than 2 cm, N0). None of these 19 patients have relapsed, with follow-up ranging from 12 to 42 months. In 12 of the 47 patients, recurrent tumor developed within the first 18 months of follow-up.

TITLE: Results of combined radiotherapy and multidrug chemotherapy in advanced squamous cell cancer of the head and neck.

Treatment results of combined radiotherapy and polychemotherapy in a group of 38 patients with advanced squamous cell of the head and neck cancer are presented. The radio-chemotherapy schedule included 60Co irradiation with 20 Gy in 10 fractions over two weeks followed by multidrug chemotherapy with vincristine, bleomycin, methotrexate and leucovorin. Three series of radio-chemotherapy were given. In 14 patients the total dose of irradiation was increased to 70-105 Gy. The probabilities of overall and symptom-free survival at 2 years were 32% and 20%, respectively. Treatment results correlated with stage of the disease. Patients who received higher doses of radiation seemed to have better chances of symptom-free survival.

TITLE: Vincristine and etoposide: an effective chemotherapy regimen with reduced toxicity in extensive small-cell lung cancer.

Chemotherapy prolongs survival of patients with small-cell lung cancer, but very few are cured, and the treatment is unpleasant. Thirty patients, 28 with advanced disease, were treated with etoposide 250 mg/m2 orally, daily for 5 days, plus vincristine 2 mg intravenously on the first day, the cycle being repeated 3-weekly, to a maximum of 6. There was a response rate of 70%, a median survival of 249 days, and an 11% 2-year survival. Symptomatic side-effects were less pronounced than with most other regimens of comparable efficacy.

TITLE: Lobular cancerization: incidence and differential diagnosis with lobular carcinoma in situ of breast.

A series of 120 breast biopsies and mastectomy specimens originally diagnosed as carcinoma was reviewed in order to emphasize the differences between lobular carcinoma in situ and cancerization of lobules. Criteria for differential diagnosis between the two types of lobular lesion are proposed. In the reviewed material 15 (12.5%) cases of lobular carcinoma in situ and 64 (53.3%) of lobular cancerization were found. The carcinoma in situ usually co-existed with other types of breast carcinoma, such as intraductal carcinoma, cancerization and invasive carcinoma of different types.

TITLE: The natural history of lumbosacral plexopathy in cancer.

We studied 85 cancer patients with lumbosacral plexopathy and documented pelvic tumor by CT or biopsy. Three clinical syndromes were delineated: lower (L4-S1), 51%; upper (L1-L4), 31%; and pan-plexopathy (L1-S3), 18%. Seventy percent of patients had the insidious onset of pelvic or radicular leg pain, followed weeks to months later by sensory symptoms and weakness. The quintet of leg pain, weakness, edema, rectal mass, and hydronephrosis suggests plexopathy due to cancer. CT showed pelvic tumor in 96%. On myelography, epidural extension, usually below the conus medullaris, was seen in 45%. With treatment, only 28% of patients had objective responses on CT and 17% on examination.

Solid state nuclear track detectors were used in a regional survey of radon in indoor air. The study area comprises seven rural municipalities and two towns in an area of 80 X 50 km2 with a population of about 65,000. Measurements were made in 754 houses in 31 subareas. The highest and lowest subarea means were 1,200 Bq/m3 and 95 Bq/m3, respectively. The estimated mean for the whole area was 370 Bq/m3. The concentrations 2,000 Bq/m3 and 800 Bq/m3 were exceeded in 32 and 90 houses, respectively. The present lung cancer incidence does not differ significantly from the national mean.

One hundred-thirty-nine patients with advanced breast cancer were treated during five years since 1977 in our Department. Treatments consisted of chemotherapy (ACF and ACFM), hormone therapy (tamoxifen) and their combination (ACFT). Seventy patients had lung metastases at the initiation of these therapies. An overall response rate was 41%, whereas a response rate for metastatic lung lesions was 26% (18/69). Breast cancer is known as a systemic disease, thus other therapies such as pulmonary resection or irradiation had a limited indication to control the lung metastasis. Our result indicates that favorable responses to advanced breast cancer with systemic therapies will improve the prognosis of breast cancer patients with lung metastasis.

The use of the NPCP grading system does not contradict the principle that an accurate prediction of tumor volume, presence, and extent of metastases, therapeutic response, and clinical behavior of prostate cancer cannot be entirely based on the grade of the disease at the time of initial diagnosis. The morphology of prostate cancer, like any other cancer, is probably no more than one of a group of parameters that are decisive for the prediction of the biology and clinical course of the disease. Nevertheless, the present data support the premise that the NPCP system of grading prostate cancer, among others, is a very helpful indicator of the extent and behavior of prostate malignancy. Table 6 illustrates the correlation between grades and stages of prostatic carcinoma that can be drawn from the preceding data.

We postulated previously that systematic differences in menstrual cycle length and/or variability might be used as indicators of underlying hormonal abnormalities that could help explain the endocrine biology of some breast cancer risk factors. In the present study, we prospectively and retrospectively analyzed menstrual cycle patterns in breast cancer cases and controls in two populations. No significant differences were found. This and previous studies emphasize that contemporary women have a long reproductive experience characterized by uninterrupted, regular menses, which is a condition of maximum ovulation potential and which contributes to estrogen stimulation over time.

40 women with breast cancer were checked by the ultrasound Octoson technique and the results compared with the histological tumour types. We grouped the carcinomas into 3 types: scirrhous, solid or medullary, and the invasive lobular tumour. Our diagnostic ultrasound criteria: internal echoes, as well as contour and boundary echo, showed a close relationship to the histological types. Irregular internal echoes, spiculated contours and weak boundary echoes were seen in most cases with scirrhous or lobular carcinomas, whilst the other histological types also showed characteristics similar to benign tumours such as absent internal echoes, regular contours and strong boundary reflexions. The criterion "shadowing" was not closely related to the degree of internal echoes either. Hence, a needle biopsy under ultrasound control should be performed to verify the diagnosis in all doubtful cases.

Ovarian carcinoma is the most severe gynecological cancer with an overall incidence of 12 per 1000 Americans or Europeans developing it over 40 years of age. Only 3 of the 12 cases will receive efficient care because the diagnosis will be made too late. This study reveals the principal risk factors i.e. upper socioeconomic echelon, ovarian function uninterrupted by a pregnancy or usage of oral contraceptives, anamnestic evidence of ovarian carcinoma in the family, some hereditary disorders, external insults (talcum powder). The synthesis of these various risk factors permits a comprehensive review of the hypotheses of pathogenesis concerning recurrence of tumors. But corollary epidemiologic studies are still needed to try to define better the high risk groups whose follow-up systematic detection and testing is a priority.

The serum level of six acute phase proteins (APP) has been evaluated preoperatively and for a few days in the postoperative period. Thirty patients undergoing total gastrectomy for gastric cancer have been studied in two subgroups according to their nutritional status. Those with gastric cancer had significantly higher baseline serum levels of alpha 1 acid glycoprotein (alpha1AGP) and C-reactive protein (CRP) than a control group. Malnourished patients also had reduced acute phase response for alpha1AGP and alpha 1 antitrypsin (alpha1AT) a higher increase of CRP and fibrinogen, and a lower decrease for transferrin and retinol binding protein (RBP).
