TITLE: Early detection of pancreatic carcinoma.
PUBDATE: 20020201
The poor prognosis and late presentation of pancreatic cancer patients emphasize the importance of an effective early detection strategy for patients at risk of developing pancreatic cancer. In current practice, the use of CA 19-9 levels and imaging techniques is not optimal for detecting small pancreatic lesions. It is hoped that the understanding of genetic alterations in combination with the development of high-throughput sensitive techniques will lead to the rapid discovery of a panel of biomarkers that will save lives by enabling aggressive therapy at the time when tumors are curable.

TITLE: Cancer in rodents: does it tell us about cancer in humans?
PUBDATE: 20051001
Information obtained from animal models (mostly mice and rats) has contributed substantially to the development of treatments for human cancers. However, important interspecies differences have to be taken into account when considering the mechanisms of cancer development and extrapolating the results from mice to humans. Comparative studies of cancer in humans and animal models mostly focus on genetic factors. This review discusses the bio-epidemiological aspects of cancer manifestation in humans and rodents that have been underrepresented in the literature.

Breast cancer in males is rare, accounting for less than 1% of all cases. Bilateral male breast cancer is reported to occur in fewer than 2% of all the diagnosed cases of male breast cancer, and synchronous tumors are exceedingly rare. The authors report a case of male breast cancer that is bilateral and synchronous, and review the literature on male breast cancer.

TITLE: [Magnetic resonance colonography: a new diagnostic tool]
PUBDATE: 20051101
Magnetic resonance colonography (MR colonography) is a three-dimensional imaging of the colon. Data postprocessing also allows for virtual colonoscopy, depicting the intraluminal morphology. Acquisition and postprocessing of high-resolution images require a state-of the-art modern MR scanner and a workstation. Regarding patient preparation, a bowel cleansing is necessary. The results of cancer screening have shown MR colonography to be superior to the traditional double-contrast barium enema. In inflammatory bowel disease, MR colonography can be used to assess disease activity, including spreading.

PUBDATE: 20050801
Mammography is the best tool available for screening for breast cancer. Although the data supporting clinical breast examination are not as strong, this procedure continues to be widely used in the United States. To maximize accuracy of results, women who undergo screening during their premenopausal years should attempt to schedule mammography during the follicular phase of the menstrual cycle. All women should be educated about the benefits and the harms of screening, including the risk of being called back for further testing.

PUBDATE: 20040401
One of the effects of oncogenic signaling appears to be differentiated recruitment of mRNA's to ribosomes. The mRNA's so affected frequently encode proteins involved in growth regulation cell-cell interaction. These functions are critical for both cancer and development, potentially suggesting that the normal role of differential translation may be during development. It is not known whether this effect is sufficient to induce cancer from cells with an initial non-neoplastic gene expression profile.

INTRODUCTION: A 14-year-old boy treated with Hodgkin's disease developed muscular weakness and pain, hypotonia, abolished deep tendon reflexes. Examination of the cerebrospinal fluid showed albuminocytologic dissociation, the characteristic finding of Guillain-Barré syndrome. After high dose steroid treatment complete recovery occurred. DISCUSSION: Paraneoplastic neuropathies may develop in patients with cancer, Guillain-Barré syndrome occurs in patients with active Hodgkin's disease. CONCLUSIONS: Progressive neurological signs may indicate the presence of a malignancy, or in case of chemotherapy shows the activity of the tumour.

A new isoflavone 5,7,4'-trihydroxy-3'-(3-hydroxy-3-methylbutyl)isoflavone (isowigtheone hydrate) (1), together with six known isoflavones 2-7 and (-)epicatechin, were isolated from the root barks of Brosimum utile. Their structures were established on the basis of spectroscopic evidence. The in vitro cytotoxic activity of the new compound 1 was evaluated against cell lines MCF7 (human breast carcinoma), PC3 (human prostate carcinoma), HT29 (human colon cancer) and human dermis fibroblasts.

The drawback was the impaired function of the bladder (9-year prevalence SOMA G3-4 symptoms: 66%), indicating the need for treatment aimed at reducing chronic morbidity.

Reconstruction of total or near-total glossectomy defects has been challenging and the functional outcomes are often disappointing. In this article, the 10-year experience of total or near-total tongue reconstruction at the University of Texas M.D. Anderson Cancer Center is reviewed. The trends of surgical and functional outcomes, length of hospital stay, and choices of flaps and recipient vessels are identified. In addition, the investigators' experience, as well as a literature review, of sensory and motor reinnervation for tongue reconstruction is presented.

This article describes the author's technique of pancreaticoduodenectomy (PD), together with guidelines for disease staging, pre-operative work-up and patient selection. The role of neo-adjuvant and adjuvant chemotherapy is currently under evaluation and all patients who have a curative resection should be considered for entry into the ESPAC 3 trial that aims to establish the definitive role of adjuvant chemotherapy in pancreatic cancer.

PUBDATE: 20050801
Pain is the major source of anxiety and distress at the end of life, particularly in cases of end-stage cancer. However, pain management is not always effective or effectively implemented. This article identifies several barriers to effective pain relief in terminal cancer--the complexity of pain; difficulties in physical, emotional and spiritual assessment; difficulties in the delivery of medication--that challenge the skills of all professionals involved in palliative care. There are no simple answers, but awareness of the breadth of the issues may help focus nurses' minds on the patient in every encounter.

TITLE: Epigenetic alterations in gastric carcinogenesis.
PUBDATE: 20050401
Gastric cancer is believed to result in part from the accumulation of multiple genetic alterations leading to oncogene overexpression and tumor suppressor loss. Epigenetic alterations as a distinct and crucial mechanism to silence a variety of methylated tissue-specific and imprinted genes, have been extensively studied in gastric carcinoma and play important roles in gastric carcinogenesis. This review will briefly discuss the basic aspects of DNA methylation and CpG island methylation, in particular the epigenetic alterations of certain critical genes implicated in gastric carcinogenesis and its relevance of clinical implications.
