Work reported over the past year has provided insights into the mechanisms whereby ligand activation of the nuclear receptor peroxisome proliferator-activated receptor gamma (PPARgamma) regulates systemic glucose and lipid homeostasis. PPARgamma has also been implicated recently in the biology of monocytes and in cell-cycle regulation and cancer. Polyunsaturated fatty acids and eicosanoids bind and activate PPARgamma, suggesting that these lipids may serve as hormonal regulators of a variety of biological processes.

Radiotherapy has a major role in the multidisciplinary approach to cancer therapy. It is widely used for curative and palliative treatment of cancer involving various sites. Radiotherapy is of particular benefit to older and frail cancer patients as an alternative to surgery and to systemic therapy. The available data on the sensitivity of normal tissues to radiotherapy in elderly patients strongly suggest that older patients with good functional status tolerate radiotherapy as well as younger patients and have comparable tumor response and survival rates. Aggressive radiotherapy should not be withheld from older patients because of chronological age alone.

A new prenylated stilbene, mappain (1), was isolated from leaves of Macaranga mappa by bioassay-guided fractionation. The structure was established by application of spectroscopic methods. Mappain is cytotoxic but it appears to be a poor substrate for P-glycoprotein-mediated transport because it is equally potent and effective against the drug-sensitive SK-OV-3 and drug-resistant SKVLB-1 ovarian cancer cell lines, exhibiting an IC(50) value of 1.3 microM in both cases.

Cancer surgery has existed as an elective treatment modality for more than 150 years, yet it is relatively unsophisticated in light of the advances seen in other medical disciplines. Using the nuclear medicine technique of lymphoscintigraphy (i.e., mapping lymph nodes using a radioactive isotope), the surgeon can perform a sentinel lymph node biopsy that targets only the lymph nodes that first drain a primary tumor site. This procedure can provide accurate staging information without risking the potential morbidity associated with lymph node dissection. This type of precise biopsy is made possible by using a gamma-detecting instrument coupled with an ultrasensitive probe designed for intraoperative use.

The association of multiple cutaneous cancers and long-term use of hydroxyurea is now being recognized. In this article, we describe the development of multiple skin tumors in two patients who were receiving hydroxyurea therapy. These cases illustrate the late onset of subsequent skin cancers despite discontinuation of therapy. As hydroxyurea continues to have a prominent role in the treatment of myeloproliferative diseases, clinicians must be aware of the increased risk of multiple skin cancers and use preventive and skin cancer screening practices in patients with these diseases.

Irinotecan is the only accepted second-line treatment for colorectal cancer in the USA. Doses are, however, frequently limited by associated late-onset diarrhoea. Thalidomide has antiangiogenic and immunomodulatory properties and is being investigated as an antineoplastic. We did a pilot study of combination therapy with thalidomide and irinotecan for metastatic colorectal cancer. In an interim analysis of nine patients, thalidomide had almost eliminated the dose-limiting gastrointestinal toxic effects of irinotecan, especially diarrhoea and nausea (each p<0.0001), and eight of nine patients were able to complete the chemotherapy course.

Standard curative therapy in head and neck cancer, especially in oral cavity carcinoma, consists, so far, of surgery. Until today, chemotherapy using standard drugs has failed to be effective beyond controlling cancer manifestations and palliation of symptoms. The introduction of chemotherapy and radiotherapy as adjuvant treatment, however, may improve overall survival and curative options. A multicentric randomized trial, initiated by DOSAK, has compared results after radical surgery alone with results of combined preoperative radio-chemotherapy followed by radical surgery. The life table analysis substantiated improved survival rates in the group of patients whose treatment included chemotherapy and radiation.
