TITLE: [Symptoms of depression in cancer patients undergoing chemotherapy]

Symptoms of depression and anxiety were measured in a prospective follow-up study of 36 cancer patients before and during chemotherapy by means of the Hamilton rating scales and the Melancholia Scale. These scales proved useful in the examined population. Moderate or severe depression was seen in almost half of the patients before chemotherapy, but this figure decreased during follow-up. The symptomatology resembled that of primary depression, and the brief rating scales for depression seemed suitable for screening purpose. Problems in identifying morbid states of anxiety and depression are discussed.

TITLE: Mucinous adenocarcinoma in defunctionalized bladders.

Primary adenocarcinoma of the bladder accounts for less than 2% of all bladder cancers. There is no report of such a case in a defunctionalized bladder. All reported cases of carcinoma in defunctionalized bladders were either squamous cell, signet ring cell, or transitional cell carcinoma, detected within an average of 5 years after urinary diversion, and all have been associated with chronic inflammation of the bladder. We report on 2 cases of adenocarcinoma that developed in defunctionalized bladders 30 and 8 years after ileal loop diversion for prune belly syndrome and neurogenic bladder, respectively.

TITLE: First two years of a follow-up breast clinic led by a nurse practitioner.

After special training a nurse practitioner ran an independent clinic for follow-up patients with breast disease. All patients referred to a single surgical firm with breast cancer and most patients with benign disease who required follow-up were included. In the first 2 years of the service 382 clinic visits were recorded (median 5/clinic, range 1-12). The nurse practitioner reviewed 236 (62%) patients alone but involved the consultant surgeon in the remainder. No significant lesion was missed in these patients. The nurse-led clinic is popular with patients and, subject to careful supervision, offers an attractive option for follow-up of patients with breast disease.

Most of breast cancer patients are treated with CMF, which is a combination of three anticancer agents, cyclophosphamide, methotrexate and 5-fluorouracil. Metabolites of CMF induce lipid peroxidation by inactivating the antioxidant enzymes, thereby rendering the system inefficient in management of the free radical attack. Acrolein and phosphoramide mustard are the metabolites of cyclophosphamide which are among the causative agents which reduce the activity of superoxide dismultase, catalase, glutathione peroxidase, glutathione reductase, glutathione-S-transferase and glucose-6-phosphate dehydrogenase in erythrocytes of CMF treated breast cancer patients.

Childhood Hodgkin's disease has a high cure rate but second cancers are common, related to treatment and perhaps genetic predisposition. The glycophorin A (GPA) mutation assay measures the frequency of NO and NN mutant erythrocytes of MN blood-group heterozygous individuals. Mutant frequencies in Hodgkin's disease patients were compared with controls. No significant difference from controls was found pretreatment or in patients treated with radiotherapy only. Patients who received chemotherapy had a significantly increased frequency (total mutation frequency per 10(6) cells: 31 vs 11, p < 0.001), which may be relevant to their known increased risk of secondary leukaemia.

Authors reported on transplantation glomerulopathy (TGP), occurred in a patient's cadaver kidney allograft, as well as on orchitis, lymphogranulomatosis (Hodgkin's lymphoma) and lip cancer detected thereafter in the same patient. While TGP was definitely be verified morphologically, all other observed diseases are commonly and typically met with membranous glomerulopathy (MGP), which is a separate entity. Similar case has not yet been reported in the literature. Since in majority of the cases the MGP is a paraneoplastic phenomenon, and the TGP is a part of chronic rejection in kidney allografts the differential diagnosis may have therapeutic consequences.

In 75-80% of all clinically significant prostate cancer, the peripheral zone appears to be hypoechoic on transrectal ultrasound (TRUS). However, examination by TRUS alone in a screening program is not recommended due to its low sensitivity. The combination of prostate-specific antigen, digital rectal examination, and TRUS increases the detection rate of prostate cancer. In prostate cancer arising from the transition zone, TRUS findings are often nonspecific. Even prostate biopsies might be negative due to the location of this tumor. Anterior biopsies of the transition zone will identify these prostate cancers. Color Doppler ultrasound of the prostate has little additional value over TRUS alone in the diagnosis of prostate cancer.

From February 1965 to April 1993, 88 patients with middle lobe carcinoma of the lung were treated surgically. It constituted 7.7% of the total patients of operated lung cancer in our hospital. Clinical symptoms were mainly fever, cough, bloody-sputum and chest pain. 84 of 88 patients were treated by wedge resection, lobectomy and pneumonectomy. Exploration was preformed in 4 patients. The 1, 3, 5 and 10-year survival rates of the 78 patients followed up were 87.18%, 43.08%, 38.18% and 24.13% respectively. It related to four clinical features of middle lobe cancer of the lung were treated surgically is discussed.
