TITLE: A view of partial laryngectomy in the treatment of laryngeal cancer.

Attention is drawn to the problems of surgical resection by partial laryngectomy after full therapeutic dosage of telecobalt irradiation. Material is presented from two treatment centres to indicate trends in surgical treatment and the complications experienced. Results will be given which confirm a reasonable expectation of cure and functional conservation by vertical partial laryngectomy for recurrence of glottic cancer after irradiation. The results of treatment of recurrent supraglottic cancer by horizontal partial laryngectomy gave more cause for concern in terms of complications and survival. In conclusion an attempt is made, based on the quoted experience, to define the position of conservation surgery in two centres for laryngeal cancer in the United Kingdom.

TITLE: Digital dermal patterns in breast cancer.

Fingerprints of 570 breast cancer cases and the same number of matched controls were obtained from the population-based finger print file in Hawaii for studying the association between breast cancer and digital dermal patterns and ridge counts. The results showed that breast cancer patients had a significant excess of radial loops on the left hand. It was also found that the frequency of ulnar loops on the left hand was significantly elevated for premenopausal women with breast cancer, whereas an excess of radial loops on the left hand was observed for the postmenopausal women with breast cancer. No significant difference for the total and absolute ridge counts was found between breast cancer patients and controls.

A radioimmunoassay was used in vitro in the blood serum of 156 lung cancer patients, 21 patients with benign pulmonary tumors and 100 patients with chronic nonspecific pulmonary diseases to study the levels of 9 hormones reflecting function of the major links of endocrine regulation. The most important feature of pulmonary tumor growth was the activation of the hypophyseal-adrenal system. Endocrine shifts in lung cancer and in chronic nonspecific pulmonary diseases were similar in nature. Of the entire spectrum of hormones the importance of ACTH, cortisol, calcitonin and parathyroid hormone was emphasized in differential diagnosis of lung cancer.

AIDS is an enigmatic disease which presents special clinical diagnostic and management problems. Although any system may be affected the lungs and the brain are the most important target organs. Though the underlying disease is currently untreatable, many of the complications of AIDS are amenable to prompt therapy. Seropositive patients should be monitored to detect early signs of significant HIV disease. Skilled counselling about the avoidance of co-factors which may potentiate HIV infection, is important.

The relationship between bladder cancer and alcohol consumption was investigated in a case-control study conducted in 506 patients (453 males and 53 females) with diagnoses of papillary urothelial carcinoma during 1978-1982 at "La Paz" hospital (Madrid), and an equal number of controls matched for age, sex and date of admission to the hospital. The results showed that the risk of bladder cancer does not increase with the intake of beer, wine and spirit beverages. However, a high risk of bladder cancer was associated with consumption of wine mixed with "gaseosa" (a refreshment beverage consisting of carbonated water plus artificial sweeteners).

The differential diagnosis of multiple cranial nerve palsies in patients with cancer includes meningeal infections, meningeal carcinomatosis, and skull base metastases. In distinguishing these, spinal fluid analysis and skull base tomography should be helpful in most cases. In circumstances when results of skull base tomography are negative, radionuclide bone scans can demonstrate metastatic disease in the base of the skull, and it should be obtained in all patients who are highly suspicious for having skull base metastasis with negative skull base tomography, including computed tomography (CT).

A 40-year-old black male had unresectable giant condylomata acuminata of the anorectum with invasive, squamous-cell carcinoma. He was treated with intravenous 5-fluorouracil and mitomycin-C and with extended field radiation to the primary lesion. An abdominoperineal resection was performed, and the surgical specimen contained no residual cancer. Three years after diagnosis, the patient has no recurrent cancer. This case illustrates that otherwise unresectable giant condylomata acuminata with invasive carcinoma may be rendered operable with chemotherapy and radiation.

The advances made in mammography, including its accuracy, safety and publicity, will lead to an increasing number of biopsies of the breast after needle localization. The surgeon must carefully evaluate the mammograms and orient the location of the tip of the wire with respect to the mammographic abnormality and the exact location within the breast. The technique described has been used in more than 600 instances. It has led to the detection of cancer in 20 per cent of nonpalpable suspicious mammographic findings. It is well tolerated, readily accomplished, cost effective and accurate.

In Britain and other high-risk countries, about a third of patients with breast cancer are premenopausal at diagnosis. In the remainder, tumour initiation might have occurred in the premenopause, even though the clinical presentation was late in life. This possibility has important implications for breast cancer prevention and screening. The relations between the patient's age and tumour kinetics, prognosis, oestrogen receptors, and environmental X-ray carcinogenesis were studied, together with the age-related protection afforded by pregnancy. The findings support the hypothesis that breast cancer is initiated in the premenopause.

Sperm chromosome studies have been performed in 70 normal males. The incidence of aneuploidy in this group is approximately 3-4%, and that of structural anomalies close to 5%. In carriers of reciprocal or Robertsonian translocations, the results are extremely variable, with percentages of unbalanced sperm from 8 to 87%. No unbalanced spermatozoa have been observed in patients with pericentric or paracentric inversions. In cancer patients treated with radio and/or chemotherapy, the incidence of chromosome abnormalities is much higher, and significantly different from that found in controls.

In a study of 118 patients with advanced breast cancer treated with aminoglutethimide plus hydrocortisone replacement the overall objective response rate was 42 (36%). Response was not determined either by the nature or the extent of previous treatment, including endocrine therapy and/or chemotherapy, nor by oestrogen-receptor status. The vast majority of patients who responded were postmenopausal but a complete response was seen in a premenopausal patient with skin metastases. Aminoglutethimide is useful in the second-line treatment of postmenopausal subjects with advanced breast cancer.

After standardization for age and menopausal status, the prevalence of estrogen receptor positivity among 88 White breast cancer patients was about .72 compared with a prevalence of about .54 among Black patients. The prevalence of progesterone receptor positivity was also higher among White than among Black patients, although the magnitude of the difference was smaller. These differences were unchanged after adjustment for tumor size and nodal and distant metastases in addition to age and menopausal status.
