TITLE: A syndrome resembling idiopathic thrombocytopenic purpura in 10 patients with diverse forms of cancer.

Ten patients with diverse forms of carcinoma (six patients) and of cancer in the lymphoid system (four patients) presented with or subsequently had a syndrome resembling idiopathic thrombocytopenic purpura (ITP). These patients were older than patients who had ITP alone. ITP was coincident with a diagnosis of cancer (three patients), preceded a diagnosis of cancer (three patients) or followed a diagnosis of cancer (four patients). No patients responded to steroid therapy permanently. Therefore, the course of our patients was fairly typical of ITP in the adult. Since we found nine patients with cancer in a survey of 52 consecutive patients with ITP in the adult population admitted to one hospital, this diagnosis may be missed with some frequency; the development of thrombocytopenia being attributed to other causes.

Without recourse to any formal psychotherapy, the family physician aware of this reaction may be able to enhance the immune defences and increase the quality of life of such patients.

TITLE: Carcinoma of the breast in a community hospital: who makes the diagnosis?

A review of 300 cases of breast cancer in a community hospital was undertaken to document who made the initial discovery of the breast lesion. In this series, the woman discovered her own cancer in 75% of the cases. Obstetrician-gynecologists, who made up only 8% of the medical staff, discovered almost one third of the primary lesions that were first noticed by physicians. Obstetrician-gynecologists were the first physicians consulted by 20% of the women who discovered their own breast lesions. Preoperative xeromammography was performed on 58% of the patients with breast cancer, and there was a false negative report in 21% of these patients.

TITLE: Initial clinical trials of maytansine, an antitumor plant alkaloid.

We have conducted a phase I clinical trial of maytansine, a plant alkaloid with potent tubulin-binding activity. For evaluation of toxicity, the schedule of drug administration consisted of a single iv infusion given every 3 weeks. Dose-limiting toxicity was observed at 2 mg/m2, and was manifested as profound weakness, diarrhea, nausea, and vomiting. Symptoms persisted for 3--14 days after drug administration. No consistent myelosuppression occurred at any dose level. Responses were observed in two patients (one each with non-Hodgkin's lymphoma and ovarian cancer) who were treated on the every-3-week schedule, as well as in two patients with acute lymphocytic leukemia treated with single weekly doses. Three of the four responding patients had received extensive prior treatment with vincristine, and two were clearly resistant to vincristine.

TITLE: Selective localization and growth of Bifidobacterium bifidum in mouse tumors following intravenous administration.

A strain of domestic bacteria, Bifidobacterium bifidum (Lac B), which is nonpathogenic and anerobic, selectively localized and proliferated in several types of mouse tumors following i.v. administration. None of the same bacilli could be detected in the tissues of healthy organs such as the liver, spleen, kidney, lung, blood, bone marrow, and muscle 48 or 96 hr after i.v. administration into tumor-bearing mice. Proliferation of Lac B in the tumor was artifically stimulated by i.p. administration of DDD-H-2s mice of a synthesized disaccharide, lactulose (4-O-beta-D-galactopyranosyl-D-fructofuranose), a sugar which is not metabolized by mammalian tissue cells. Lac B, which survices and proliferates selectively in the tumor following i.v. administration into the tumor-bearing host, should aid in diagnosis and selective therapy for cancer.

TITLE: Causes of death among lumberjacks--a pilot study.

Causes of death among 375 union-affiliated male lumberjacks throughout Sweden were studied. The proportional mortality rates for ischemic heart diseases, cerebrovascular diseases, all other circulatory diseases, violent death, all cancers and "other causes" were compared to the general Swedish [male] mortality pattern. There was a significantly elevated mortality from violent death and a slight numerical excess of deaths due to ischemic heart diseases, cerebrovascular diseases and all other circulatory diseases. There were fewer deaths than expected for cancer and "other causes." A case-referent type of analysis showed an increased standard mortality ratio for kidney cancer and cancer of the lymphatic and hematopoietic system.

TITLE: Mutagenic and recombinogenic effects of the antitumor antibiotic anthramycin.

Anthramycin, one of the pyrrolo(1,4)benzodiazepine antibiotics with potent antitumor activity, was tested for its effects on a number of genetic parameters. The results show that this antibiotic is nonmutagenic in the Ames strains of Salmonella typhimurium while mutagenic in only one and antimutagenic in the rest of the genes tested in the eukaryotic organism Saccharomyces cerevisiae. The antibiotic is, however, a potent recombinogen inasmuch as it induced mitotic crossing over, mitotic gene conversion, and possibly other chromosomal alterations in a diploid strain of S. cerevisiae. These studies emphasize the need for a battery of test systems including eukaryotic organisms to detect the genetic activity of certain antitumor drugs. The importance of considering data distinguishing between highly mutagenic and poorly mutagenic cancer chemotherapeutic agents is also discussed.

TITLE: Premature deaths in schizophrenia and affective disorders. An analysis of survival curves and variables affecting the shortened survival.

We have found shortened survival in 200 schizophrenics, 100 manics, and 225 depressives by comparing them with the Iowa general population. The numbers of years of shortened survival were estimated from the survival curves of each diagnostic group by sex. Variables affecting shortened survival were diagnostic group, sex, age at admission, and pay status at admission. Excess causes of death were suicides, accidents, and infective and circulatory system diseases. The absolute mortality for deaths due to neoplasm in our sample was not significantly different than that in the Iowa general population. However, the proportional mortality from the same data showed a deficiency in deaths due to neoplasm in schizophrenia and affective disorders. Well-designed epidemiologic studies are needed to see if there is a real deficiency in deaths due to cancer in the major psychoses.

N-nitroso-dimethylanine dissolved in aquarium water at a concentration of 400 ppm (1/4 of the LD50/30d) induced malignant tumors in 39 (54.2%) out of 72 young adult animals of the amphibian species Xenopus borealis. The first tumor was detected at 11 weeks, and the average latency was 35.1 weeks. Tumors were located in the liver (32 cholangiocarcinomas and 13 hepatocellular cancers) and the kidney (9 adenocarcinomas and 2 anaplastic cancers). Thirteen animals bore two or more types of tumors. These results demonstrate that a high percentage of tumors can be induced in amphibians by chemical carcinogens if appropriate methods are used.

The relation between replacement estrogens and breast cancer in menopausal women (age 45-64 years) was evaluated in the Group Health Cooperative of Puget Sound, Washington State, a prepaid health care organization with fully computerized information on outpatient drug use and hospital diagnoses. Little association between current estrogen use and breast cancer was noted in women with a previous hysterectomy (relative risk (RR) = 1.1; 90% confidence interval (Cl), 0.7-1.8). In women with a natural menopause there was a positive association between current estrogen use and breast cancer (RR = 3.4; 90% Cl, 2.1-5.6). This association was strong in naturally menopausal women aged 45-54 years (RR = 10.2) and weaker in the older age group (RR = 1.9).

TITLE: Oestrogen-receptor status and sites of metastasis in breast cancer.

The oestrogen receptor (RE) status of the primary tumour has been assessed in 466 of a consecutive series of 550 patients with primary operable breast cancer. All patients were followed up (without treatment) until the development of recurrence or metastases. Distant metastases have so far occurred in 124 patients and 82 have had symptomatic local or regional recurrence. A significant correlation exists between the RE status of the primary tumour and subsequent patterns of metastasis. Symptomatic metastases to regional lymph nodes are more common with RE- cancers. There is no significant difference in either time of onset or total incidence of distant metastases between patients with RE+ and RE- tumours. Distribution of distant metastases is influenced by RE status: RE+ tumours tend to recur in bone, RE- tumours show affinity for viscera.

TITLE: Pretreatment nausea in cancer chemotherapy: a conditioned response?

Many patients receiving cancer chemotherapy become nauseated as they anticipate their treatments. We studied this phenomenon in eighteen cancer chemotherapy patients. The eight patients who reported pretreatment nausea had more extensive disease than the other patients and had received twice as much chemotherapy. In most cases pretreatment nausea developed only a after a number of months of treatment. Nausea was usually precipitated by the odor of the clinic and similar odors elsewhere also caused nausea. Patients continued to experience nausea during follow-up visits after treatment was completed. This syndrome of pretreatment nausea can be understood as a classically conditioned response. Clinical recommendations can be made on this basis.

Tobaccos from countries with high and low incidences of lung cancer were analyzed. Tobacco concentrations of polonium-210 were similar in cigarettes from high- and low-incidence countries, as were levels of cigarette smoke tar and nicotine. Tobaccos from low-incidence countries had significantly lower Alternaria spore counts. Mean selenium concentrations of tobaccos from the high-incidence countries (0.16 +/- 0.05 micrograms/g) were significantly lower than those of tobaccos from the low-incidence countries (0.49 +/- 0.22 micrograms/g).

Sixty-one patients with granulosa cell tumors of the ovary were treated in the A.M. Evans Clinic of the British Columbia Cancer Control Agency between 1944 and 1974. Their treatment and follow-up are reviewed, and while the 5-year survival rate was 78%, the 15-year rate fell to 50%. The material suggests that the trend to total hysterectomy and bilateral salpingo-oophorectomy has improved the prognosis, but other explanations are, of course, possible as this is a historical comparison.

Prostaglandin F2 alpha can be measured as a stable degradation product, 13,14-dihydro-15-keto-prostaglandin F2 alpha (DHK-PGF2 alpha). Pathological serum levels of this lipid compound have been observed in prostate hyperplasia and treated testicular cancer patients. In renal cancer patients elevated DHK-PGF2 alpha shows a trend to normalize 12 months following nephrectomy in stage T0N0M0. Bladder cancer patients have slightly increased DHK-PGF2 alpha that differ in relation to preinvasive bladder cancer. In contrast serum haptoglobin levels were significantly pathological in patients with kidney and bladder cancer, suggesting involved cancer metabolism. Patients with prostate and testis cancer are without distinct relation to the tumour stage. After specification of haptoglobin types no coincidence to urogenital tumours could be detected. Correlated aberration of PGF2 alpha and haptoglobin displayed different patterns depending on the type of urogenital tumour.
