In recent years, the association between breast and endometrial cancer has been the subject of many studies. The present report describes four cases of this association in which tamoxifen had been administered to all of the patients. Data have been published regarding the possibility that tamoxifen may be responsible for the subsequent development of carcinoma of the corpus uteri in these patients. The authors intend to carry out a case-control study on patients treated with tamoxifen for breast carcinoma to reveal the possible presence of endometrial carcinoma.

Gallium nitrate, a novel drug for the treatment of cancer-related hypercalcaemia, inhibits osteoclast activity but does not affect osteoclast morphology or viability. Limited clinical experience in patients with cancer-related hypercalcaemia indicates that gallium nitrate is effective in restoring normocalcaemia in 75 to 85% of patients and is well tolerated in those with preserved renal function, producing few clinically relevant adverse effects. In comparative clinical trials it proved a more effective antihypercalcaemic agent than calcitonin or etidronate and produced a longer lasting normocalcaemic response. Gallium nitrate would appear to be indicated in symptomatic patients with cancer-related hypercalcaemia who have failed to respond to adequate rehydration.

Many forms of cancer have a higher incidence in relatives of patients than in the general population, and some show Mendelian inheritance. Although individuals genetically predisposed to cancer represent a minority of all cancer patients, the genetic basis for their disease has profound significance. These familial cases provide strong evidence that germline alterations can contribute to cancer. They also provide an ideal opportunity to identify and isolate the genes mutated in common cancers. Products of the tumour suppressor genes have been implicated in several hereditary forms of cancer. The distinct functions of these proteins and their roles in familial cancer will be discussed.

Prevention of lung cancer remains the best method of decreasing lung cancer mortality. Patients who smoke should be urged to quit, and children, teenagers, and young adults must not begin smoking. At high risk are smokers, especially those under 40 years of age who may have smoked two to four packs of cigarettes per day for 20 years; persons who have had a previous lung cancer; patients with bullous emphysema; patients with asbestosis; and patients with evidence of chronic airflow obstruction. Although radiographic screening may detect lung cancer earlier and lead to increased 5-year survival rates, it does not reduce lung cancer mortality rates.

This paper discusses the application of CT, MR imaging, and 131I-6 beta-iodomethyl-19-norcholesterol scintigraphy in establishing the nature of incidentally discovered adrenal masses. It is concluded that CT and MR imaging can permit confident differentiation between nonhyperfunctioning adenomas and metastases in cases of incidentalomas that meet all the criteria of a nonhyperfunctioning adenoma. In cancer patients, fine-needle aspiration biopsy of lesions is, in general, essential because a definitive diagnosis of metastatic disease is crucial to disease management. MR imaging and norcholesterol scintigraphy are useful if a relative contraindication for fine-needle biopsy exists or when biopsy results are inconclusive in patients with a presumably benign lesion.

From Nov. 1991 to Dec. 1992 63 patients were examined by preoperative intrarectal 3-dimension ultrasonography (IR3DUS) for the staging of their rectal carcinoma. IR3DUS assessed the tumor penetration depth with an accuracy of 93.65 percent (P < 0.001, Kappa statistic). IR3DUS predicted lymphatic metastasis with an accuracy of 92.1 percent vs. 63.6% by CT. Correlated with histopathologic findings, the overall accuracy of the preoperative staging utilizing IR3DUS was 93.65 percent (P < 0.001, Kappa statistic). Therefore, IR3DUS is considered a very useful tool for the assessment of the depth of cancer invasion in the rectal wall and perirectal lymph node metastasis.

Energy and protein intake in 32 consecutive patients with small cell lung cancer was examined at initiation of cyclical chemotherapy and after 1 and 3 months. With each cycle intakes decreased in the first 2 days following chemotherapy, but were back to pretreatment levels on the third day. Eleven of the patients lost weight in the study period. Their energy and protein intakes were lower following chemotherapy compared to pretreatment intakes and to post-therapy intakes in weight-stable patients. Pretreatment intakes did not decrease over time, either in weight-losing or in weight-stable patients.

A case of early gastric cancer arising from Ménétrier's disease in the stomach is described. The patient was a 53-year-old man, who suffered from epigastralgia. Radiological and endoscopic examination demonstrated giant mucosal folds along the greater curvature of the stomach. On the distal margin, signet-ring cell carcinoma was demonstrated by biopsy. The coexistence of Ménétrier's disease and early gastric cancer has rarely been reported. Cases reported in the literature were reviewed and the increased malignant potential of Ménétrier's disease was discussed.

Breast tissue obtained from 18 of 246 women (7.3%) having localization biopsy for microcalcification contained crystals of Weddellite (calcium oxalate dihydrate). This was characterized by microscopic infra-red spectroscopy in three cases. Weddellite calcification was associated with benign lesions in 16 cases, but incidental atypical lobular hyperplasia and lobular carcinoma in situ were present, each in one case. Pathologists should be aware of this common non-haematoxyphilic, birefringent form of breast micro-calcification. Its presence has implications for the examination of biopsy specimens resulting from breast cancer screening using mammography.

Inoculation of the pancreatic cancer cell line PC-1 into a defined area of the tail of the hamster pancreas led to development of multiple cancerous foci in the body of the pancreas along the main pancreatic duct. Each carcinoma showed identical histological characteristics and was completely separated from the others by regions of normal pancreatic tissue. This observation suggests spread of the cancer from the primary site in discontinuity through the ductal system of the pancreas.
