TITLE: A comparison of three blood tests for cancer.

Plasma carcinoembryonic antigen concentration, serum phosphohexose isomerase activity and the erythrocyte sedimentation rate were determined in patients with cancer, patients being treated for various benign conditions and healthy young persons. This was to make an intercomparison between the ability of these assays to distinguish patients who have cancer from those who do not and from normal persons. The upper boundaries of the assays were defined by the one-sided, distribution-free, upper tolerance limits of the reference populations. Only by means of the assay for carcinoembryonic antigen was it possible to distinguish patients who had cancer from those who did not. Twenty-two per cent of patients with cancer could be identified by this assay with a false-positive error rate of less than 5 per cent.

TITLE: Group support for patients with metastatic cancer. A randomized outcome study.

The effects of weekly supportive group meetings for women with metastatic carcinoma of the breast were systematically evaluated in a one-year, randomized, prospective outcome study. The groups focused on the problems of terminal illness, including improving relationships with family, friends, and physicians and living as fully as possible in the face of death. We hypothesized that this invention would lead to improved mood, coping strategies, and self-esteem among those in the treatment group. Eighty-six patients were tested at four-month intervals. The treatment group had significantly lower mood-disturbance scores on the Profile of Mood States scale, had fewer maladaptive coping responses, and were less phobic than the control group. This study provides objective evidence that a supportive group intervention for patients with metastatic cancer results in psychological benefit. Mechanisms underlying the effectiveness of this group intervention are explored.

Cisplatin (cis-diammine-dichloro-platinum) administered at a dose of 3 mg/kg iv induced a reproducible and characteristic emetic response in the dog. It was characterized by a latency period (90-120 min) and multiple emetic episodes occuring within 5 hours following drug administration with sporadic delayed emesis later within the first 24 hours. There was a qualitative similarity between the emetic response of Cisplatin seen in dogs and cancer patients. Metoclopramide (1, 3 mg/kg sc) was found to be the most effective antagonsit of Cisplatin emesis in the dog while haloperidol (1 mg/kg sc) and chlorpromazine (0.3, 1, 3 mg/kg sc) offered a less complete protection. Nabilone (0.1 mg/kg iv) and AL-1612 (1 mg/kg sc) failed to to demonstrate any significant activity. A relationship between antagonism patterns of emetic responses induced by Cisplatin and apomorphine was discussed.

TITLE: C1q-binding substances in peripheral lymph in bronchial carcinoma.

Peripheral leg lymph has been studied for the presence of C1q-binding substances by the I-C1q binding radioassay in six male patients with untreated bronchial carcinoma. In serum, this assay is highly specific for antigen-antibody complexes (immune complexes). Three of the patients had immune complexes in serum, and they all also had C1q-binding substances in peripheral lymph. The C1q-binding activity was quite similar in serum and peripheral lymph. In the other three patients, immune complexes were not found in serum, but in ome of them, moderate amounts of C1q-binding substances were found in peripheral lymph. The results suggest that immune complexes may be found in peripheral lymph in cancer patients in about the same amounts as in serum. However, formal proof is lacking that the C1q-binding substances of peripheral lymph are real immune complexes.

The effect of PMA in pain therapy was investigated over a period of one year on 52 patients suffering from terminal cancer. These patients, 22 being outpatients, received a total number of 85 peridural catheters. 75 of these catheters were evaluated according to morphine dosage and effect. The onset of PMA resulted in a drastic reduction in the need for high dose systemically applied analgesics. PMA was also successfully applied in cases with rapidly spreading metastases. Our catheters have remained in place up to 170 days. Side effects were rarely observed. Tachyphylaxis did not develop and almost all patients were satisfied with the therapy. Due to special care of the catheter we observed only 7 cases of infected puncture sites. Our clinical studies have proven that peridural morphine analgesia can be successfully applied as pain therapy in patients suffering from terminal cancer.

The evidence relating to pre-natal radiation exposure and the subsequent occurrence of malformations and cancer suggests that the overall risk lies in the range 0--1 cases per 1000 irradiated by one rad in utero in the first four months of pregnancy. The natural level of occurrence of serious handicaps in average pregnancies is at least 30 times higher. Is the much lower probability of radiation-induced harm sufficiently high to justify (a) concern when a woman who has been irradiated is found to have been pregnant at that time, or (b) the maintenance of restrictions on medical uses of ionizing radiation in women in the reproductive age, such as the ten day rule?

Computed Tomography (CT) was introduced to detect lesions in 48 cases with various hepatobiliary disease. In 8 out of 11 cases with liver cell cancer, diagnosis were made by CT. A case was detected in early stage, the low density area was so small that it can be resected segmentally. In 7 out of 9 cases with metastatic liver cancer, multiple low densities were detected by C.T. Gallbladder cancers, which is known as diagnostic difficulty, were detected in five cases as low density in gallbladder area. CT has superiority to liver scintiscan, 1) CT can determine the tumor more exactly except for the diffuse type. 2) CT can be performed repeatedly without any risk and use of isotopic substances. 3) CT observes the transverse configuration of the organ, which could not be done in any conventional method.

TITLE: Cause of death among jewelry workers.

A proportional mortality analysis of jewelry workers as identified on death certificates is presented. The study group consisted of 931 males who died in Attleboro, Mass., between 1956 and 1975. An excess proportion of pancreas cancer was found in the entire group (16/9; p < 0.05) and was not explained by ethnic or other non-occupational factors. Job titles were specific enough to identify a subset of polishers and findings for this job category were compared to those for all other categories. Excesses of stomach cancer (odds ratio 4.4; p < 0.01) and stomach ulcer (odds ratio 5.0, p < 0.01) were found, but for each the observed number of deaths is small. Possible important exposures in the jewelry industry are reviewed.

TITLE: National cancer mortality and incidence in Japan.

In Japan cancer mortality statistics have been prepared by site, sex, and age group since 1937. The trends in age-adjusted death rates for selected sites from 1937 to 1975 in intervals of two or three years, and the age-adjusted death rates for some sites by prefecture in 1968-1972 were summarized. Concerning the accuracy of cancer mortality statistics, special consideration should be given to senility in Japan. In 1975 deaths due to senility accounted for 4.3% of all deaths for all ages. Though the regional cancer registration was being operated in 16 prefectures in Japan as of 1976, the incidence rates were published for only six prefectures. The percentage of cases registered from death certificate only in six registries in Japan was compared with that in the registries in the United States, indicating that the completeness of registration was yet unsatisfactory in Japan. As an index of accuracy of diagnosis, the percentage of cases confirmed histologically was also compared.

Colon tumors were induced by 1,2-Dimethylhydrazine (DMH) and Acetoxymethyl-methyl-nitrosamine (AMMN) in male Sprague-Dawley rats. The usefulness of the Haemoccult-test (HT) was investigated for early detection of these chemically induced tumorous injuries. As a prescreening method HT was shown to be unsuitable for early detection of tumours under the conditions of the models used. The accuracy of HT in cases of advanced chemically induced tumors was explored in rats with known colonic tumors, diagnosed by rectoscopy and exploratory laparotomy. In animals with proven colonic tumors HT gave a positive result only in 63% of rats with AMMN-induced tumors and in 50% rats with DMH-induced cancers. Therefore, HT is thought to be an insensitive and thus unsuitable method for the detection of colonic cancer in rats.

Surgical and pharmacological management of cancer pain is described and discussed according to the physiopathological mechanisms underlying this complex syndrome. The therapeutic approach is planned in three mayor phases which may be employed alone or in combination, following an accurate evaluation of the pathophysiology and the clinical pattern in every single patient. The first phase includes multifocal pharmacological therapy by nonnarcotic drugs in order to affect at different levels the physiopathological mechanisms of cancer pain. The second phase is indicated when nonnarcotic drugs cannot achieve complete pain relief; neurosurgical procedures (nerve blocks, rhizotomies, cordotomies, ecc...) are employed in this phase. The pharmacological treatment must be continued and associated to surgery. The third phase includes hypophysectomy, deep brain stimulation, psychosurgery and/or narcotic drug therapy, which are the last step in management of terminal cancer pain when all treatments have been ineffective. The results of this therapeutic program in 188 patients affected by pain of malignant origin are reported and discussed.

TITLE: Postabsorptive urinary hydroxyproline test in patients with metastatic bone disease from breast cancer.

Postabsorptive urine hydroxyproline (HYPRO) excretion ("spot" HYPRO test) was compared with the 24-hour urine excretion of HYPRO in 45 patients with breast cancer and nine normal subjects on a low-gelatin diet. A good correlation was observed between the results of these two tests in both groups. Patients with skeletal metastasis showed a higher spot HYPRO value than those without bone involvement and the number of lesions by bone scanning correlated with the values of spot HYPRO. In 20 of 22 patients with bone metastasis followed up for three to 19 months, spot HYPRO values correlated well with the evolution of bone disease. We conclude that the spot HYPRO test is a simple, inexpensive, and accurate method for the diagnosis and follow-up of patients with skeletal metastasis from breast cancer.

TITLE: Unusual presentations of carcinoma of the right colon.

Two unusual presentations of carcinoma of the right colon are described. One of the two patients presented with a right gluteal abscess and the other presented with a colocutaneous fistula to the anterior abdominal wall at the site of a previous appendectomy scar. Despite extensive local tumor invasion of adjacent structures in both cases, all the regional lymph nodes were free of cancer and the tumors were of low-grade malignancy. A review of the literature indicates that bulky colonic tumors with extensive local invasion and negative mesenteric lymph nodes have a relatively good prognosis if adequate resection is performed.

TITLE: Breast cancer mortality among PVC fabricators.

A case-control analysis of breast cancer deaths among PVC fabricators is presented. This study is an extension of a cross-sectional mortality study of deaths occurring among 17 PVC fabricators during the years 1964 to 1973. Relative risk estimates were derived and tested using the Mantel-Haenszel procedure. Although no statistically significant relative risks were found, a least significant relative risk analysis indicated that the underlying relative risk would have to be on the order of three to one in order to be detected. There does seem to be reasonable assurance that very large increases in risk for breast cancer do not exist among these PVC fabricators.

All of the tubular adenocarcinomas of early gastric cancer in humans showed gentle transitions from surrounding normal to neoplastic epithelium. In mammary carcinomas in C3H/He mice, known as viral tumors, there were also transitional appearances from normal to cancer cells. Furthermore, it was possible in rats to trace gradations between normal fibrocytes and sarcoma cells in marginal areas of tumors induced subcutaneously by 3-methylcholanthrene (MCA). It cannot be explained by the mutation of a few somatic cells that transitions from normal to neoplastic cells were observed not only in viral tumors in mice but also in human carcinomas and in chemically induced tumors in rats. It seems well to think of the mode of origin of all types of tumors as an excessive cellular regeneration to make up for the functional disturbance of cells invaded by oncogenic viruses.
