OBJECTIVE: To explain the reason for the toxicity and ways to avoid it. DISCUSSION: The transdermal fentanyl patch is proving to be a safe, well tolerated and effective treatment modality in the control of cancer pain, but care has to be exercised when using this drug, especially at the time of initiation, as the consequences may be disastrous for the patient.

The phenomenon of aging is distinct from processes associated with advanced age known to increase risk of diseases, such as cancer. Furthermore, the process of aging is not necessarily related to phenomena such as in vitro replicative senescence; however, any unifying hypothesis of aging must account for all age-dependent phenomena, including senescence. It is proposed that apoptosis forms the ultimate protective process for preservation of phenotypic fidelity in multicellular organisms since it is the process by which the organism detects damage and replaces the defective cell. Time-dependent degeneration of apoptosis control is the rate-limiting step in the process of aging.

Since severe bradycardia (17 beats.min-1) was detected by preoperative Holter monitoring, a temporary pacing catheter was inserted. She underwent mastectomy under general anesthesia using propofol combined with thoracic epidural anesthesia. She also received postoperative thoracic epidural block. Her perioperative heart rate was 80-105 beats.min-1 and the rhythm was sinus. There was no marked perioperative cardiovascular derangement.

1alpha,25-dihydroxyvitamin D(3) (1alpha,25 (OH)(2)D(3)) is a naturally occuring organic compound showing strong calcemic activity and anti-proliferative/differentiation-inducing activity. 1alpha,25 (OH)(2)D(3) is expected to be a therapeutic agent for metabolic bone diseases and cancer treatment. To enhance its selectivity, specificity, and safety as a therapeutic agent, it is required to separate its calcium-regulating activity from its non-calcium-regulating activity. For this reason, a variety of vitamin D analogues have been developed. The analogues with modification of A-ring are promising candidates for this purpose.

Most chemical carcinogenic agents are industrial. About 4% of all cancers have an occupational origin, but the percentage is higher in exposed populations. Nonsteroidal antiinflammatory drugs (NSAIDs) are a group of compounds with similar therapeutic and side effects. Their therapeutic effects depend on blockade of prostaglandin synthesis through enzyme cyclooxygenase (COX) inhibition. Two isoforms of the enzyme cyclooxygenase have been identified: COX-1 and COX-2. Selective COX-2 inhibitors i.e. meloxicam, nimesulid, etodolac or highly selective COX-2 inhibitors i.e. celecoxib, rofecoxib have antiinflammatory and analgesic properties with less or no gastrointestinal or other NSAIDs-typical adverse effects. Highly selective COX-2 inhibitors may also be active in colonic polyposis, colorectal cancer and Alzheimer's disease.

TITLE: [Subacute neuropathy, multiple cancers, and Gougerot-Sjögren syndrome: contribution of anti-Hu antibody]

We report the case of a patient presenting a subacute, predominantly sensory neuropathy. The work up revealed a Sjögren's syndrome and a breast carcinoma. The presence of anti-Hu antibodies, identified by Western Blot using purified recombinant HuD protein, and the absence of the Hu antigen in the breast carcinoma ruled out the responsibility of the Sjögren's syndrome or breast carcinoma. In this context, the most likely diagnosis was a subacute neuropathy associated with small cell lung cancer, which was indeed discovered 3 years later.

A cytotoxic steroidal glycoside was isolated from Chlorophytum malayense Ridley and its structure was characterized as a known compound, neohecogenin 3-O-beta-D-glucopyranosyl(1-->2)-[beta-D-xylopyranosyl(1-->3)]-beta-D- glucopyranosyl(1-->4)-beta-D-galactopyranoside (chloromaloside A). The structural identification was performed using 2D-NMR and LC/MS/MS analysis. The previous, erroneously assigned 1H-NMR spectral data were revised whereas the published 13C-NMR spectral assignments were confirmed. This compound showed in vitro cytotoxicity against several human cancer cell lines.

During long-term fasting, gluconeogenesis from amino acids was thought to lessen, when ketone bodies from lipolysis became a major fuel source. Thus, muscle mass is conserved. However, recent studies show that this adaptation does not occur in chronic undernourishment. In cancer, chronic undernutrition without disease, and HIV infection, carbohydrate utilization is high. Enhanced hepatic glucose production occurs in active inflammatory bowel disease and in underweight cancer patients. Repletion of tissue after undernutrition is energetically inefficient because of enhanced diet induced thermogenesis (following anorexia nervosa) and decreased fat, and increased protein, oxidation (in tuberculosis).

However, all physicians treating patients with germ cell tumors need to be aware of the toxicities of chemotherapy. Because many patients are cured of their disease, both short-term and long-term toxicities have been identified. The short-term toxicities are myelosuppression, nausea and vomiting, pulmonary toxicity, nephropathy, and neuropathy. The longer term toxicities often discussed are ototoxicity, impairment of fertility, persistent neuropathy or ototoxicity, cardiovascular diseases, and secondary malignancies, which are all described in this review.

Colorectal cancer is the second most common cause of cancer death in the UK. Prompt investigation of suspicious symptoms is important, but there is increasing evidence that screening for the disease can produce significant reductions in mortality. High quality surgery is of paramount importance in achieving good outcomes, particularly in rectal cancer, but adjuvant radiotherapy and chemotherapy have important parts to play. The treatment of advanced disease is still essentially palliative, although surgery for limited hepatic metastases may be curative in a small proportion of patients.
