Herein we report the results of a study on the mortality trends of testicular cancer in Spain from 1951-1983. The study showed that following a long period of increased mortality, overall mortality rates for all age groups have dropped in recent years. However, the mortality rate has increased for the young and middle-aged males and has dropped for the older age groups. A similar finding has been observed for other countries such as the United States. We have attempted to interpret these changes based on the risk factors for this type of cancer.

TITLE: Cancer of the male breast with prolonged survival.

A retrospective review of patients suffering from male breast cancer was carried out at the Shands Teaching Hospital of the University of Florida. Thirteen evaluable cases were analyzed. Three patients were in Stage I, two patients in Stage II, none in Stage III, and eight in Stage IV. Two of the patients with Stage IV disease have had remarkably prolonged survivals of 194 months and 128 months. Such prolonged survivals are unusual. It is possible that the biology of male breast cancer is different from the female, and the disease should be approached more optimistically than it has in the past.

Cancer of the gallbladder carries a grim prognosis, because the disease is usually too advanced and surgically incurable at the laparotomy. In our series, the curative resection rate is only 16.7%. However, palliative resection with postoperative irradiation have possible effects on the prolongation of survival periods. Cancer of the bile duct has better prognosis, than that of the gallbladder, because the early occurrence of obstructive jaundice often leads to curative resection. In our series, however, the curative resection rate is only 50.0%. More aggressive surgical therapy might improve the surgical figures. In addition, postoperative radiation therapy and immuno-chemotherapy may have possible effects as adjuvant therapy.

A 40-year-old male with advanced penile cancer, whose left inguinal node was ulcerated at the time of initial presentation, underwent multimodal therapy. Four cycles of chemotherapy were given from September 20, 1984 to February 8, 1985. Partial penectomy and left ilioinguinal lymphadenectomy with removal of left groin ejaculation were performed on February 20, 1985, followed by right ilioinguinal lymphadenectomy on March 20, 1985. The skin defect of the left groin was covered with tensor fascia lata myocutaneous flap. The patient is alive with no evidence of disease 30 months after the surgery.

The 1980 National Center for Health Statistics life tables for the U.S. black and white populations reveal a difference in life expectancy of 7 years between black and white males and 6 years between black and white females. Using cause-substituted life tables, we show that a number of causes of death contribute to the difference. The largest contributors are cardiovascular disease for both sexes and homicide and cancer for males.

The authors report a case of müllerian adenosarcoma in an 86 year old woman. This tumor is characterized by the association of proliferative benign epithelium with a sarcomatous mesenchyma. It was described for the first time in 1974 by Clement and Scully. It is a cancer of decreased malignancy, revealed either by isolated metrorrhagias, or by an increase of the size of the uterus in a post-menopausal woman. Its course shows the occurrence of local recurrences and pulmonary or peritoneal metastases. The only appropriate treatment is total abdominal hysterectomy with salpingo-oophorectomy.

If cytological investigation alone is performed 2500 examinations are needed for one case of screening-related survival. If these examinations are repeated on a yearly basis, the benefit related to this screening effort is less than 1 case of survival/1000 subjects/year.

Clostridium septicum bacteremia and its association with hematologic malignancy and colorectal cancer have been well recognized. Panwalker, in a recent review, discussed clostridial sepsis and other unusual infections associated with colorectal tumors, including streptococcal bovis and Bacteroides. He reports the coexistence of colorectal cancer and metastatic Clostridium septicum infections at multiple sites. We describe a case in which a patient with an occult cecal carcinoma develops Clostridium septicum sepsis and thoracic aortitis secondary to metastatic gas gangrene. This dramatic and unusual complication has not previously been documented. The necessity of colonic evaluation with Clostridium septicum bacteremia is discussed.
