In spite of the fact that fluorinated pyrimidines have been in extensive clinical use for more than 25 years, the optimal dose, route of administration and schedule have yet to be determined. An adequate course is considered to be one that produces either an objective response or mild toxicity. However, modification of dose schedule have shown considerable variation in rate of response, drug toxicity, and drug mortality. In contrast to its rapid clearance from blood, 5-fluorouracil may persist for prolonged period in cancer tissue. More detailed measurements of their metabolites in cancer tissue may lead to an improved understanding of 5-fluorouracil treatment.

TITLE: Lung cancer detected during a screening program using four-month chest radiographs.

Ninety-two lung cancers were detected in the Mayo Lung Project in patients undergoing chest radiography every four months for screening. Fifty patients had a peripheral nodule, 16 had a perihilar nodule, 20 had hilar or mediastinal enlargement, and six had pneumonitis. The peripheral cancers grew slowly. Ninety per cent were visible in retrospect for months or even years. Despite this, 70% of the peripheral cancers were classified as postsurgical American Joint Committee (AJC) Stage 1. The central cancers grew rapidly, usually presenting as hilar or mediastinal enlargements after normal findings on the previous radiograph obtained four months earlier. Most were classified as AJC Stage 3 tumors.

TITLE: Pancreatic cysts caused by carcinoma of the pancreas: a pitfall in the diagnosis of pancreatic carcinoma.

Pancreatic cysts were diagnosed on computed tomography (CT) in 6 (8%) of 73 histologically proven pancreatic carcinomas. In three of a total of seven cases studied, the initial CT diagnosis was pancreatitis. The CT features of cysts secondary to pancreatic carcinoma were identical to those of pseudocysts in pancreatitis. All cases had masses of varying sizes suggestive of carcinoma or pancreatitis. Even in retrospect, two cases were difficult to differentiate from pancreatitis with pseudocysts. Our results indicate that whenever pancreatic cysts are encountered in patients of cancer age without a history of pancreatitis, further examinations including percutaneous aspiration biopsies should be performed to exclude malignancy.

The indications for laparoscopy have been greatly reduced by the development of ultrasonography. However, laparoscopy is still indicated in secondary cancer of the liver, as it can provide histological proof of the diagnosis by means of biopsies taken under direct vision. It is therefore more effective than the non-invasive morphological examinations, but, in the future, it may be replaced by ultrasound guided biopsies. In general, laparoscopy only confirms a diagnosis which is very probable on the basis of clinical and laboratory data. Furthermore, it is very unlikely that laparoscopy will reveal small metastases; this is due to the multicentric nature of hepatic metastases, rather than to a deficiency in the investigation itself.

A periodical and histological study on the histogenesis of experimental stomach carcinoma induced by N-methyl-N'-nitro-N-nitrosoguanidine in rats suggested that cancer progression occurred from the submucosal focus under the condition of existing adenomatous hyperplasia. This carcinoma manifested particular atypia, combined hyperplasia of the smooth muscle, indistinct borderline from the surrounding normal mucosa. These were considered to be specific histological findings in experimental carcinoma. In 3 of 273 gastrectomized patients with gastric carcinoma the same histological findings were obtained as in experimental gastric carcinoma; 35 patients had submucosal heterotopic glands similar to the experimentally induced rat foci or adenomatous hyperplasias seen in the experimental carcinoma. These results suggest the same histogenesis for human stomach cancer and the experimental carcinoma.

Total rosette forming cells (TRFC) and high affinity rosette forming cells (HARFC) were enumerated in 534 patients with carcinoma of oral cavity, uterine cervix or breast, and the results were compared with adult controls. The HARFC levels were found to be significantly decreased in cancer patients, and this decrease was more pronounced in advanced clinical stages. The HARFC levels returned to normal in patients who had a clinical cure but remained low in patients having residual disease. Enumeration of HARFC may therefore be useful in assessing the cell mediated immunity in cancer patients.

The value of the subrenal capsule assay in predicting the response of advanced ovarian cancer to combination chemotherapy was studied prospectively. The population consisted of 24 patients with advanced or recurrent disease. Using a three-grade interpretation of drug response in the assay (sensitive, intermediately sensitive, and resistant), an over-all predictive accuracy of 79% was achieved. All objective clinical responses and acquired drug resistance could be predicted by the assay. It was concluded that the assay is a promising method for predicting response of patients with ovarian cancer to chemotherapy.

A simple, safe, rapid and accurate technique is described for cytological diagnosis of ulcerated or nodular rectal cancer done while performing digital rectal examination or sigmoidoscopy. In 21 cases, there were 17 true positives and no false positives for cancer. There were two true negatives and one false negative for rectal cancer. It is important to exclude prostatic cancer if, in the male, the lesion is anterior and situated at or near the level of the prostate gland, and this can be done using an immunoperoxidase technique which will identify cancer cells of prostatic origin.

In a case-control study of all new cases of laryngeal cancer in Denmark from 1980 to 1982, 326 cases and 1134 community selected controls participated. Questionnaires were used to obtain information on education, occupation, and number of occupational exposures as well as smoking and drinking habits. High risk ratios for laryngeal cancer were found for semiskilled and unskilled workers, workers exposed to dust, out of doors workers, drivers, and people working in the cement industries and port services. The study hypothesis was that exposure to chromium or nickel increases the incidence rate of laryngeal cancer. No support for this was found concerning chromium, but exposure to nickel had a statistically significant risk ratio of 1.7.

The irradiation of radical resectable lung cancer is unambiguously not as effectively as surgery, and also the preoperative radiation with high tumour doses has not demonstrated any benefit in survival rates. Tumours of the superior sulcus could be an exception in both treatment modalities. As shown in randomized studies one could suggest some advantage employing postoperative irradiation of patients especially with mediastinal node metastases. The addition of chemotherapy to high dose irradiation with curative intent in inoperable non-small cell lung cancer has not been shown to increase survival. The combined chemo-radiotherapy of small cell lung cancer significantly reduced the incidence of relapse and prolonged the disease free interval but did not affect long-term survival.

We describe a 62-year-old man who presented with digital necrosis as part of a clinical picture which resembled Wegener's granulomatosis. However, an open lung biopsy revealed adenocarcinoma of the lung. To our knowledge he is only the 5th reported patient with lung cancer presenting with digital necrosis. The unusual association of digital necrosis and internal malignancy is discussed.

The effect of tamoxifen on serum levels of basal prolactin and basal and stimulated growth hormone was assessed in 10 women with advanced breast cancer prior to and after 1 and 8 weeks of treatment. Tamoxifen had no effect on basal levels of either hormone or on insulin-stimulated growth hormone. Two of 4 patients undergoing arginine provocation testing had a partial response to tamoxifen and both exhibited marked diminution of growth hormone stimulation which was not seen in the non-responders.
