TITLE: MRI: a role in clinical trials.
PUBDATE: 20010601
The diagnosis and treatment of breast cancer has evolved significantly over the last 20 years. Breast-conserving therapy is replacing the Halstedian concept of "en bloc" resection. Difficulties in detection, pre- and postoperative planning and follow up continue to challenge the clinician. Women at high risk present a significant clinical dilemma. MRI technology in many of these areas is providing more information about detection, tumor size, extent, and response to treatment. The careful and thoughtful inclusion of MRI in clinical trials may help continue the advancement of breast cancer care. J. Magn. Reson. Imaging 2001;13:837-841.

The correlation between dose values from dose-volume histograms (DVHs) and dose values from dose-wall histograms (DWHs) of the rectum tissue of patient with uterine cervix cancer was determined. The minimum dose in 2 cm3 in the high-dose region of the DVH is a good estimate of the dose in the rectum wall.

TITLE: The role of mammography in breast cancer prevention.
PUBDATE: 19990901
Screening mammography has been shown to reduce breast cancer mortality by detecting small, nonpalpable, early-stage breast cancers. In 1998, several studies were published, reinforcing the value of screening mammography in women aged 40 years and older. Some studies focused on when screening should begin (age 40 years), while others analyzed the process of screening mammography, with particular attention to recall rates and false-positive studies. Other reports emphasized the limitations of mammographic interpretation, the use of computers to aid in mammographic diagnosis, and screening with ultrasound.

A family history of ovarian cancer confers an increased risk of ovarian cancer. We review the literature quantifying the familial risks associated with ovarian cancer and assess the evidence that the familial clustering of ovarian cancer is genetically determined. We then describe the known high-penetrance genes, the ovarian cancer risks attached to them and their contribution to ovarian cancer in families and in the general population. The evidence for the existence of other ovarian cancer genes is then considered.

PUBDATE: 20010201
Pain that is poorly responsive to opioid analgesics is challenging for physicians who deal with cancer patients. Numerous factors may influence analgesic response during the course of the illness. These include changing nociception associated with disease progression, the appearance of intractable side effects, the development of tolerance, the presence of neuropathic pain, the temporal pattern, the effects produced by the production of opioid metabolites, and many others. These factors influence the delicate balance between pain relief and opioid toxicity that must be achieved in cancer patients with pain.

Proteomics is a rapidly expanding discipline that aims to gain a comprehensive understanding of the expressions, modification, interactions, and regulation of proteins in cells. New high-throughput technologies, such as protein chips and isotope-coded affinity tag peptide labeling, coupled with classic technologies such as two-dimensional gel electrophoresis and mass spectrometry, complement genomic technologies, providing cancer researchers with powerful tools for cancer diagnosis and prognosis and for the identification of targets for therapy.

Lymphadenectomy in gastric cancer became vital and obligatory part of staging procedure. Therapeutic effect of lymphadenectomy with patient's prognosis improvement still remains uncertain. In subgroups, extensive lymphadenectomy enhance radicality of the procedure, enables to reach R0 resection in the area of tumour bed and hopefully improves survival. D2 lymphadenectomy is now becoming as a standard also in our country. Technique and possible complications of D2 lymphadenectomy are the matter of this article.

Three patients with lung cancer and shoulder pain for which no local cause could be found are described. All three benefited from a course of palliative radiotherapy to ipsilateral mediastinal disease remote from the site of the pain. It is suggested that the pain is referred from intrathoracic involvement of the phrenic nerve by cancer, and that palliative irradiation of the mediastinum should be considered if investigations fail to reveal a local cause for ipsilateral shoulder pain.

Cancer and psychiatric symptoms commonly co-occur. The frequency is about 50%, and those most important for urology are depression, anxiety, and probably post-traumatic stress disorder. There is a strong relationship between psychological distress and cancer pain. This review provides information on diagnostic and therapeutic strategies for psychiatric diseases, which are important for oncology patients in urology. Special advice is given for the doctor-patient relationship and communication.

The goal of this study was to asses the diagnostic value of hysteroscopy, cytology, ultrasonography and histopathology in various pathological states in endometrium. 250 patients with abnormal uterine bleeding were examined. The wide range of several diagnoses were achieved with 8 cases of cancer. Used methods were found to be complementary because 7 cases of cancer were recognised by histopathologic method, 6 by hysteroscopy and 5 by a cytological test (3 results were suspicious). To the risk group were qualified by ultrasonography all patients in postmenopausal age.

The staging of colorectal cancer continues to be an interesting and evolving process. Accurate staging can predict overall prognosis and helps to select appropriate treatment options. This article addresses the clinical and pathologic staging of colon and rectal cancer. The use of endorectal ultrasound in the preoperative clinical staging of rectal cancer is reviewed. The importance of surgical resection margins, lymph node retrieval rates, and tumor markers is discussed.

TITLE: Screening and surveillance for colorectal cancer.
PUBDATE: 19991001
Colorectal cancer is the second most common cause of cancer death among American men and woman. Currently available screening and surveillance techniques are effective in detecting early-stage colorectal cancer and its premalignant precursor lesion, the adenomatous polyp (adenoma). Removal of adenomas by colonoscopic polypectomy significantly reduces the incidence of colorectal cancer. Appropriate screening and surveillance recommendations should be based on the individual's colorectal cancer risk stratification. High-risk groups, such as patients with hereditary nonpolyposis colorectal cancer (HNPCC) and familial adenomatous polyposis (FAP), should be offered genetic counseling and specialized screening recommendations for colorectal and associated extracolonic malignancies.

TITLE: The role of radiotherapy in non-small cell lung cancer.
PUBDATE: 19991201
Most patients who receive a diagnosis of non-small cell lung cancer (NSCLC) have advanced disease and are not curable with surgery. Developments in the technology of radiation therapy (RT) have contributed to the broad utility of this treatment modality in both a curative and palliative capacity. Many patients at all stages, including those who are medically inoperable, may benefit from RT. Locally advanced NSCLC is treated commonly with combined modality therapy. Novel RT administration schedules and chemotherapy regimens for combined modality therapy are essential for improving the management of NSCLC. Additional benefits can be foreseen as new strategies for patient selection emerge.

Advances in the treatment of tumors have improved the life expectancy of cancer patients appreciably. As a consequence the incidence of bone metastases has also increased. Surgical interventions must achieve a maximal palliative effect with minimal morbidity and mortality. Major objectives are reduction of pain while preserving stability and function. Surgical treatment of primary and secondary bone tumors is complicated and thus also burdened with complications. However, interdisciplinary therapy comprising surgical resection and stabilization, radiotherapy and/or chemotherapy improves life expectancy and the quality of life of the cancer patient.

PUBDATE: 20000301
In this short article, we review the conceptual basis for chemoprevention of cancer, the proven clinical efficacy of this concept, and current trends to develop new chemopreventive agents based on understanding of their mechanisms of action. Four classes of new agents, namely selective inhibitors of cyclooxygenase-2, selective estrogen receptor modulators, rexinoids (retinoids that bind selectively to the receptors known as RXRs) and ligands for the peroxisome proliferator-activated receptor-gamma are discussed in detail. The importance of developing totally new classes of chemopreventive agents is stressed, with particular emphasis on the potential usefulness of new synthetic triterpenoids derived from naturally occurring molecules.
