TITLE: Tobacco smoke in the workplace: an occupational health hazard.

Tobacco smoke, which contains over 50 known carcinogens and many other toxic agents, is a health hazard for nonsmokers who are regularly exposed to it while at work. Involuntary exposure to tobacco smoke annoys and irritates many healthy nonsmokers. Serious acute health effects are probably limited to the one fifth of the population with pre-existing health conditions that are aggravated by exposure to tobacco smoke. The consequences of long-term exposure include decreased lung function and lung cancer. Existing air quality standards for workplaces do not directly specify an acceptable level for tobacco smoke. The evidence on the composition of tobacco smoke and on the health hazards of involuntary exposure suggests that there may not be a "safe" level for such exposure.

TITLE: Malignant lymphomas in road transport workers.

The relative risks of malignant lymphomas among road transport workers were investigated using the National Cancer Registry in England and Wales. Drivers of buses and coaches (OCC unit 120) had increased risks for both Hodgkin's and non-Hodgkin's lymphomas and the drivers of other road passenger vehicles (OCC unit 121) for Hodgkin's lymphomas only. Although the increased risks did not show statistical significance, they corresponded to and reinforced previously described findings based on occupational mortality.

TITLE: Interferon--the clinical experience.

Recent trials with interferon, in particular those using pure, recombinant interferon, have provided us with improved data (compared with those obtained in early studies) and show that interferon does hold some promise as a therapeutic agent. When used in the treatment of viral disease, interferon has shown itself to be quite effective in reducing viral titres. While not as effective against cancer, interferon has limited tumour growth, reduced the appearance of metastases and led to a general improvement in patient condition in many cases. This article discusses some of the recent results obtained with interferon in the treatment of viral disease and cancer.

TITLE: Epidemiology of cancer of the middle ear cleft.

Cancer of the middle ear is an uncommon tumor which has a very poor prognosis. A population-based analysis shows that there are two marked differences between men and women for middle ear cancer in England and Wales: (1) the incidence rate in men continues to rise with advancing age, but in women the rise is proportionately much less after 75 years of age; and (2) the mortality rate in men decreases after 70 to 75 years of age, whereas the mortality for women continues to rise with advancing age. The significance of these differences is not yet clear. No other epidemiologic reports related to middle ear cancer has been found in the literature.

Renal function was studied in patients with laryngeal cancer during radiation therapy performed under the conditions of mean fractionation in the presence of metronidazole radiosensitization up to the summary focal dose of 20 Gy. As compared to the findings before treatment an increase in the level of residual nitrogen, urea, uric acid and a decreased amount of creatinine in the plasma were noted. Decreased clearance of the urea, elevated Ambard's urosecretory constant were observed. The nature of these shifts was preserved even after the termination of subsequent radiation therapy up to the summary focal dose of 32 Gy without using the radiosensitogen. There is a possibility of a direct toxic effect of metronidazole on the renal tissue regardless of the state of oxygenation.

The pseudotumor cerebri, a neurological syndrome clinically characterized by headaches, vomiting and bilateral papilledema, occurred in two patients, previously subjected to total thyroidectomy for differentiated thyroid carcinoma, after initiation of levothyroxine replacement therapy. In patients with thyroid cancer, subjected to thyroidectomy and then thyroid hormone replacement therapy, the possible development of pseudotumor cerebri syndrome should be considered and differentiated from CNS symptoms due to brain metastases.

Two major TNM classification systems are currently in use for staging cancer of the head and neck region, i.e. UICC and AJC. This causes confusion when attempting to exchange information and evaluate treatment results between Australia and certain overseas centres. The two systems are described and the effect of any differences on altering survival data is assessed by reviewing 234 cases of squamous carcinoma of the larynx, hypopharynx, oropharynx and oral cavity. The direct correlation between the two systems is established. Shortcomings of the systems are demonstrated and important variables not included in either TNM system are discussed.

The Wolfe classification of mammographic parenchymal patterns has been applied to 9036 women attending a Breast Cancer Screening Unit. Overall, two thirds fell into high risk categories (P2 and DY). The incidence of DY decreases with age, while the low risk categories (N1 and P1) increase with age. P2 remains constant over most of the age range. The implications of these findings are discussed and comparison is made with other series.

Because of a high cardial risk of the patient, the tumor was resected as late as 23 months after the first diagnosis. TITLE: Predeposit autologous blood transfusions in patients undergoing irradiation and radical cystectomy.

We have used blood withdrawn electively and frozen for later use by the donor in 25 patients with bladder cancer who were treated by preoperative irradiation and radical cystectomy. Patients donated 1 to 5 units of blood (average 3.4 per patient). We transfused 10 patients with only autologous blood, whereas 15 required some homologous blood in addition to their own banked blood. The program was well tolerated and repeated blood donation did not compromise the effectiveness of preoperative irradiation. Our preliminary data show that this program reduces the dependence on volunteer donor homologous blood transfusions for cystectomy, thus, decreasing the potential risk of an adverse reaction and avoiding seasonal blood bank shortages.

Aclacinomycin-A was evaluated in a phase II trial for advanced head and neck cancer. A weekly infusion of 65 mg/m2 was used. Eighteen patients were entered. Fifteen patients were evaluable for response and toxicity. One was evaluable for toxicity only and two died before completion of a full course of therapy. One patient without previous chemotherapy achieved a PR in a neck node for a short time. Six patients had stable disease with a median to progression of 6 weeks. Major toxicity encountered was bone marrow suppression. Aclacinomycin-A appears to have no activity in squamous cancer of the head and neck.

Two cases of carcinoma of the urachus are reported, 1 involving the dome of the bladder and 1 occurring beneath the peritoneum of the anterior abdominal wall below the umbilicus at the apex of an elongated bladder. Both patients died shortly after diagnosis, a fact which emphasizes the poor prognosis of this group of patients. The poor prognosis and need for more than local resection of the tumour may not be widely appreciated. Tumours arising above the bladder have a different mode of presentation from those arising within the bladder wall. Bladder wall tumours are most frequently mucus-secreting adenocarcinomas; tumours arising above the bladder may be more variable in their histological type, and include squamous and undifferentiated cancer. A simple classification into vesicle and supra-vesical tumours is proposed, to replace an early classification into 7 groups.

TITLE: A phase II evaluation of adriamycin and cis-platinum in hormone resistant prostate cancer.

Twenty-five patients with metastatic prostate cancer were treated with a combination of Adriamycin 50 mg/m2 and cis-platinum (CDDP) 50 mg/m2 every three weeks. Response was evaluated using radioisotope bone scan, serum acid phosphatase levels, and clinical status. Response rates of 6% bone, 21% acid phosphatase, and 24% clinical status were noted. Major toxicity was gastrointestinal (due to CDDP). Treatment was well tolerated even in patients with extensive bone metastases and prior irradiation. Using the response criteria described here, patients with metastatic prostate cancer without measurable soft tissue disease are eligible for Phase II and III study.

A high frequency radiator has been developed for the treatment of prostate cancer by hyperthermia. The applicator produces a deep seated hot spot. The radiator has an outer diameter of 20 mm and an insertable length of about 175 mm. A high frequency cylindrical slot antenna inside the applicator is cooled by water. The frequency used is 433.9 MHz, one of the frequencies allowed for medical therapy in the F.R.G. A control system regulating the power output of the radiator avoids damage to the tissue around the prostate, especially the rectum mucosa and the tissue between rectum and prostate. About 60 experiments with male dogs proved that local heating of the prostate is possible with the help of the developed applicator without any damage in the surrounding tissue.

Studies of samples from a larger patient population confirmed the consistency of expression of the previously reported markers for malignancy in normal-appearing intermediate cervical cells in samples from patients with abnormal cytology (moderate dysplasia and severe dysplasia/carcinoma in situ). Based on samples of only 30 cells per case, a false-negative rate of 10% to 30% was estimated. The expression of the marker features thus provides a clear indication of uterine abnormal cytology; the lack of expression, however, does not entirely rule out the possibility of uterine abnormalities. The use of larger sample sizes and better staining protocols could further enhance the usefulness of marker feature studies in the prescreening for cancer.
