A cross-over, single blind trial of MST and MSS in relieving pain of advanced cancer patients is described. Forty-nine cases completed the study. Both MST and MSS gave equally excellent pain relief in the patients as judged by the pain score of VAS and pain rating scale by a nurse, and the duration of sleep. The daily effective dose was 55.5 +/- 19.7 mg for MST and 61.0 +/- 11.0 mg of MSS.

Systematic inflammatory response syndrome, SIRS, is one of the biological responses to surgical and anesthetic invasions. SIRS itself is a defense response, but excessive SIRS may result in post-operative failure of major organs. There is evidence that inflammatory cytokines play an important role in SIRS. Therefore it may be treated by regulating these inflammatory cytokines. Changes in inflammatory cytokines during the perisurgical period were discussed in relation to post-operative organ injury in patients who had undergone highly invasive procedures such as open heart surgery or resection of esophageal cancer to explore the possibility of anticytokine therapy.

A high-performance immunoaffinity chromatographic technique has been developed for the measurement of recombinant human granulocyte colony stimulating factor in human patients receiving this agent, following neutropenia, arising from cancer chemotherapy. The technique employs a short, biocompatible polymer column packed with minute, antibody-coated glass beads. This system was applied to the analysis of recombinant human granulocyte colony stimulating factor in three different human body fluids. A reasonable degree of correlation was achieved when comparing the immunoaffinity technique to a conventional immunoassay, although the immunoaffinity technique displayed greater specificity.

Thrombosis of intracranial sinuses and veins is a rare but potentially lethal syndrome. It often affects young adults, but occurs in all ages. It is frequently seen in association with hormonal changes (puerperium, pregnancy and the use of oral contraceptives), inflammatory bowel disease, cancer, and various states of hypercoagulability, or as a consequence of infection. The clinical picture is extremely variable and the diagnosis can be difficult unless one specifically searches for it. The combination of magnetic resonance imaging and magnetic resonance angiography is a powerful and non-invasive tool for diagnosing the syndrome. Treatment with heparin (and for the septic form antibiotics) improves the outcome.

A procedure that measures the amplification of oncogenes in human cancer cells is described. The cells were obtained by fine-needle biopsy to allow repeated sampling from individual metastases. A drawback was the low number of cells obtained, but this could be overcome by using a slot-blot hybridization technique to measure gene amplification. Two patients with mammary cancer (primary tumors or metastases), analyzed for the levels of amplification of the oncogene erb-B2, are described in detail. This technique is suitable for analyzing alterations occurring during cancer progression and for identifying subgroups of mammary cancer with different characteristics.

Even though the incidence of gastric carcinoma is decreasing, the prognosis remains poor. A review of 88 patients with advanced gastric cancer was evaluated by univariate and multivariate analysis to determine prognostic factors. Univariate analysis showed that both "curative" resection (P = 0.006) and adjuvant chemotherapy (P = 0.02) were important therapy variables. These factors were not independent when evaluated by multivariate analysis. However, when they were combined and re-evaluated by multivariate analysis, the combination of "curative" surgery and adjuvant chemotherapy significantly improved survival in advanced gastric cancer (P = 0.04).

Resistance of tumors to a variety of chemotherapeutic agents presents a major problem in cancer treatment. The gene responsible for multidrug resistance, termed mdr1, encodes a membrane glycoprotein (P-glycoprotein) that acts as a pump to transport various cytotoxic agents. The P-glycoprotein has been shown to bind anticancer drugs and several resistance-reversing agents including calcium channel blockers, and to be an ATPase. The P-glycoprotein was found to function in the blood-brain barrier. The physiological function of the P-glycoprotein in relation to therapy is discussed.

Investigation of extrarenal primary tumors occurring in Eker rats late in life (at 2 years) additionally revealed pituitary adenomas (17/31 = 54.8% (carrier) vs 7/32 = 21.9% (non-carrier siblings of affected animals), P < 0.01), sarcomas of the spleen (21/31 = 67.7% vs. 0/32 = 0%, P < 0.001) and sarcomas (of probable stromal origin) of the uterus (8/17 = 47.1% vs. 0/15 = 0%, P < 0.01). Thus, the Eker rat might provide a novel animal model of cancer predisposition syndromes.

Electrophysiological studies demonstrated very small compound muscle action potentials in response to supramaximal nerve stimulation, limited tetanic and postexercise facilitation, and prolonged prominent postexercise exhaustion (40-60% of baseline value) persisting up to 20 minutes. Pseudoaneurysms of the extracranial carotid arteries are rarely seen following irradiation for cancers of the head and neck. We present a patient with a pseudoaneurysm of the common carotid artery following a radical neck dissection and irradiation for thyroid carcinoma 20 years earlier. Following oesophagoscopical examination, a pseudoaneurysm of the right common carotid artery ruptured into the piriform sinus. The common carotid artery was embolized with multiple coils and the bleeding was halted. The relationship between the carotid artery aneurysm and irradiation, and the treatment of carotid artery aneurysm, is discussed.

In most patients presenting with acute pancreatitis, the cause can be established on the basis of initial history, physical examination, laboratory studies, and abdominal sonography. Patients with unexplained pancreatitis at that point are often considered to have idiopathic disease. However, a cause and, often, effective treatment to prevent recurrent pancreatitis are possible in many of these patients if an aggressive diagnostic approach is taken to discover undiagnosed hyperlipidemia, occult gallstones, abnormalities of the bile and pancreatic ducts, sphincter of Oddi dysfunction, pancreatic cancer and other tumors, and cystic fibrosis (in children and young adults).
