TITLE: A review of the therapy of paraneoplastic neurologic syndromes.

The therapy of paraneoplastic neurologic syndromes remains unclear and warrants a systematic review of the literature. Reports in English and foreign language literature were abstracted. Data were sorted by the primary paraneoplastic neurologic syndrome, the primary malignancy, and the methods of treatment. Neurologic improvement follows surgical, chemotherapeutic, and radiation treatments. Adjuvant immunosuppressive therapy with steroids, plasmapheresis, or immunoglobulin may help stabilize the progression of neurologic symptoms. Syndromes characterized by inflammation or neurotransmitter production without neuronal loss are most responsive to therapy. While treatment of the underlying cancer with appropriate surgery, chemotherapy, and radiation is recommended, the paraneoplastic syndrome should probably be managed with immunosuppressive therapy.

Fanconi anemia is a rare autosomal recessive disease characterized by multiple congenital abnormalities, bone marrow failure, and cancer susceptibility. The mean age of onset of anemia is 8 years, and the mean survival is 16 years. Death usually results from complications of bone marrow failure. Considerable progress in Fanconi anemia research has resulted from the recent identification and cloning of three Fanconi anemia genes. The current review describes the structure and function of the Fanconi anemia genes and describes the role of the encoded Fanconi anemia proteins in a cellular pathway controlling chromosome stability.

The objective of the work was to assess the impact of the shift of gastric cancer in an oral direction in the mediastinal lymph nodes. For this purpose a new miniinvasive lower mediastinoscopy was used in 30 patients. The method is safe and can be used also in other situations. Mediastinal metastases may occur in cancer in the distal and medium third of the stomach. They need not be present when cancer is in the upper third. The shift of gastric carcinoma did not influence the mediastinal nodes.

TITLE: Antibodies to endostatin in a multifocal glioblastoma patient.

Endostatin, a C-terminal fragment of collagen XVIII is involved in the regulation of neovascularisation in solid tumours in mice. However, few data are available on the concentration of endostatin protein in patients with cancer. Paradoxical results obtained in this way prompted us to investigate an antibody to endostatin. We detected antibodies to endostatin in the serum and in the tumour brain tissue of a patient with a multifocal glioblastoma, and in the serum samples from two patients with aggressive tumours. These data suggest that endostatin overexpression by tumour tissue might induce a humoral immune response.

Gastric carcinoma is one of the commonly diagnosed malignancies and remains an important cause of mortality in the Western hemisphere. Various sequences of genetic alterations may be responsible for the development of gastric cancer, as there exist two histological types: well-differentiated or intestinal-type and poorly differentiated or diffuse-type gastric cancers. While new genetic findings open new perspectives on carcinogenesis and molecular biology of gastric cancer, they have not been widely applied in routine diagnostic procedures, classification systems, disease monitoring, or prognostic assessment. We discuss the mechanisms molecular pathology in relation to their potential clinical relevance.

IMPLICATIONS FOR NURSING PRACTICE: An understanding of organ preservation in cancer treatment will allow oncology nurses to be more effective patient advocates by providing current information that can be integrated into patient care and education.

The components of an effective immune response have been elucidated in recent years. An understanding of the dysfunction of the immune response in cancer in one or more of these components has led to a variety of immunotherapeutic approaches. These therapeutic strategies are designed to stimulate dendritic cell proliferation, promote antigen uptake and processing, stimulate an effector cell response via direct antigen presentation, or target tumor cells via antibody therapy. Many approaches in prostate cancer have demonstrated successful induction of the desired immune response. Limited clinical success has also been seen.

Very few, if any, radiation induced cancers should appear among nuclear workers in the United States. The existing safety standards and lifetime doses received under the operation of those safety standards are such that less than 1% of the cancers that appear in nuclear workers should be related to their occupational radiation exposure. This small numbers of valid claims is a tribute to the effectiveness of the federal safety standards and to the ALARA professional philosophy of achieving excellence in radiation protection which has marked the field of health physics since its inception.

Opioids are a necessary and effective component of the management of chronic non-cancer-related pain in some patients. Careful structuring, monitoring, and documentation of care are important, but the therapeutic use of opioids is uncomplicated in the majority of patients using opioids and is gratifying for both the patient and the treating physician when it results in significant reduction in pain, improvement in level of function, and a higher quality of life. Opioid therapy is most often successful when combined with other pharmacologic and nonpharmacologic interventions as indicated by the type of pain and the context in which it occurs.
