TITLE: [Effect of porcine calcitonin on pain caused by cancer-induced bone destruction]

The analgesic effect of porcine calcitonin (PCT) was studied in 32 patients with cancer-induced bone destruction. Twenty patients (62.5%) showed some response among them 10 of 13 patients who had suffered from uncontrollable pain despite other treatment methods such as radiotherapy, and or the administration of other analgesics. The effect was observed about 10 days after the administration of the drug. Combined therapy with PCT and other modalities such as radiotherapy, chemotherapy and hormone therapy had a marked effect in controlling the pain.

In clinical practice, a number of tests are conducted to determine the immune reactivity of the patients. The principal cell in immunological surveillance against cancer has been thought to be the T cell. However, multiple immunological methods to evaluate the T cell function have failed to confirm a major role for the T cell in immunological surveillance. The role of other potential effector cells such as NK cells, K cells, macrophages and granulocytes needs more intensive investigation. In this report the effects of anticancer agents on immune effector cells were analyzed and the possibility about the development of a reliable monitoring procedure for determining a potent treatment protocol was discussed.

From 1946 through 1982, 30 cases of distant malignant disease that metastasized to the thyroid were diagnosed at the Mayo Clinic. Although metastatic thyroid lesions are not uncommon, they generally do not pose a problem because they are small or are associated with evidence of obvious widespread metastatic disease. The most common sources of such lesions are the kidney, breast, lung, and lymphoid tissue. The thyroid metastatic lesion may appear many years after the identification and treatment of the primary cancer. Because the thyroid lesion may be the only important malignant disease remaining, adequate surgical treatment may prove to be life-prolonging or life-saving.

TITLE: Combined modes of imaging for liver hemangioma in cancer patients.

Twenty-nine consecutive patients with single or multiple liver hemangioma were assessed with more than one mode of imaging. Static scanning with 99Tcm was done in 21, ultrasonography in 28, CT in 29 and angiography in 27. Fine-needle percutaneous biopsy was unwisely performed in 7 cases with a single but singular complication, severe peritoneal lymphorrhea. Four laparotomies and 2 laparoscopies completed the investigations. Fourteen patients had a history of primary cancer, and 3 of these had a secondary coexisting with the angioma. CT was characteristic; ultrasonography was sensitive and perhaps specific enough for small angiomas. The 2 combined plus angio-CT greatly reduce the need for hepatic angiography.

Management of pain in the person with cancer is a high priority in nursing. Although the actual incidence and severity is not well documented, pain may be experienced at some point by the majority of persons with cancer. Pharmacologic management, by itself, is often not adequate. Nurses must become familiar with non-pharmacologic interventions, to be used alone or in combination with analgesics, for the successful management of cancer pain. This paper discusses various non-pharmacologic options and includes a nursing protocol. More research is warranted to better define those most likely to benefit from these interventions.

Metastasis to the heart and penis from a primary bronchial carcinoma is rare in almost all histological groups of cancer. Bronchial carcinoma is known to give rise to widespread haematogenous metastases, but there are certain sites where these metastases are more likely to occur--bones, brain, liver and adrenal glands. A case is reported in which undifferentiated squamous carcinoma of the bronchus spread to two extremely uncommon sites, the heart and the penis.

One hundred eighteen patients with lung cancer were retrospectively analyzed to determine whether postoperative radiotherapy (RT) improves survival. Patterns of treatment failure and three year NED (no evidence of disease) survival rates were assessed according to extent of tumor spread, histology, and treatment method. Patients with hilar or mediastinal node metastases were at higher risk of local failure compared to those with negative nodes. Postoperative RT reduced local recurrence and improved 3 year survival among patients with positive nodes. However, postoperative RT did not improve survival among those with negative nodes. Our data indicated that patients with positive hilar or mediastinal nodes may require postoperative RT to improve survival.

Eight clinical and paraclinical parameters were investigated in a retrospective study of 100 patients who underwent operations for adenocarcinoma of the endometrium and compared to the vaginal cytology prior to treatment. The over-all accuracy of vaginal cytology was 61%. In advanced stages, with increasing dedifferentiation of the tumour and especially in cases which later showed evidence of cervical invasion microscopically, the accuracy was higher. Previous pregnancies and deliveries showed no influence on the cytological accuracy. Positive vaginal cytology was the only positive diagnostic sign for cancer in only three of the 100 cases.

Of 2880 men and 1054 women who were found to be hepatitis B surface antigen positive by the Blood Transfusion Service in England and Wales between 1971 and 1981, more than 92% were traced to the end of 1983. 5 deaths from hepatocellular carcinoma had been reported in men, giving a relative risk compared with the male population of England and Wales as a whole of 42. There had been no deaths from liver cancer in women. In both men and women there was a greater than ten-fold increase in the risk of death from chronic liver disease. Secondary preventive action may be indicated.

Methylprednisolone was given to 8 patients receiving CDDP-containing chemotherapy. Of 8 patients, 7 patients achieved complete relief of nausea and emesis and 1 patient obtained partial elimination of symptoms. All these patients had consistent history of severe nausea and emesis before the methylpredisolone trial. At present, a potential effect of methylprednisolone on chemotherapy has not been known. In a case of testicular cancer the titer of tumor markers decreased straight on a table of logarithms.

In a study designed to characterize dietary patterns of vegetarian and nonvegetarian populations, chemical analysis of 3-day composite food samples showed lower fat content of food of vegetarians than that of nonvegetarians; pure vegetarians had the lowest. Unsaturated fatty acids were highest in the food of the pure vegetarian group. No significant difference was seen in percentage protein of the food consumed by male vegetarians and nonvegetarians while the food of female vegetarians was of lower protein content than that of the nonvegetarians. The relationship of these findings to the lower incidence of colon cancer in vegetarian populations remains to be determined.

Final evaluation of the Infusaid Pump system (Infusaid Corporation, Sharon, MA) for administration of regional chemotherapy must await completion of trials prospectively comparing survival of matched patients treated in this manner and patients treated by traditional systemic chemotherapy. A review of the current medical literature confirms our personal experience that response rates within the liver are high. Survival reports appear encouraging but this may represent an artifact in patient selection. An algorithm is presented to offer surgeons a reasonable and conservative means for selection of those colorectal cancer patients with metastatic involvement of the liver who are most likely to benefit from this system of drug delivery.

Incidence of eye cancer in persons 15 years of age and over in England and Wales was studied as a surrogate for eye melanoma data. Age-standardized registration rates for cancer of the eye in adults increased by half in both males and females from 1962 to 1977. This probably reflects a real increase in incidence of eye melanoma. There was a gradient of increasing incidence with decreasingly urban place of residence, but there was no gradient of incidence with latitude. In both sexes, incidence was highest in the Birmingham region. Proportional registration ratios were generally higher for nonmanual than for manual social classes and notably high for electrical and electronics workers.
