31P-NMR spectra were obtained from the perchloric acid extracts of normal lung and lung cancer tissues obtained at surgery, and from extracts of neoplastic cells cultured in vitro. The perchloric acid extract of lung cancer tissue gave rise to a signal whose chemical shift was 3.2 ppm at pH 8.0. This signal was not observed in the extract of normal tissue of the lung from the same patient. The compound giving this signal was identified as phosphorylcholine.

TITLE: [Effect of porcine calcitonin on pain caused by cancer-induced bone destruction]

The analgesic effect of porcine calcitonin (PCT) was studied in 32 patients with cancer-induced bone destruction. Twenty patients (62.5%) showed some response among them 10 of 13 patients who had suffered from uncontrollable pain despite other treatment methods such as radiotherapy, and or the administration of other analgesics. The effect was observed about 10 days after the administration of the drug. Combined therapy with PCT and other modalities such as radiotherapy, chemotherapy and hormone therapy had a marked effect in controlling the pain.

The population burden of lung cancer due to environmental tobacco smoke is significant because a large fraction of the population is exposed. The risks are, of course, lower than those to smokers themselves; but smoking is self-inflicted, passive smoking is involuntary. Making various assumptions, the proportion of lung cancer cases among non-smokers that could reasonably be attributed to environmental tobacco smoke can be calculated to be about 20-30% in western countries. Thus, non-smokers in the society could benefit considerably from diminishing exposures to other people's smoke.

The principal cell in immunological surveillance against cancer has been thought to be the T cell. However, multiple immunological methods to evaluate the T cell function have failed to confirm a major role for the T cell in immunological surveillance. The role of other potential effector cells such as NK cells, K cells, macrophages and granulocytes needs more intensive investigation. In this report the effects of anticancer agents on immune effector cells were analyzed and the possibility about the development of a reliable monitoring procedure for determining a potent treatment protocol was discussed.

TITLE: Histopathologic changes of transitional cell carcinoma of bladder after M-VAC chemotherapy.

Over the past several years, we have utilized methotrexate, vinblastine, doxorubicin, and cisplatin (M-VAC) as definitive, neoadjuvant, and adjuvant therapy for various stages of transitional cell carcinoma with reduced toxicities. Currently, there is little information concerning the histopathologic changes after M-VAC therapy. We report on the histopathologic changes of bladder transitional cell carcinoma following M-VAC chemotherapy in our protocol for treatment of bladder cancer. The main histopathologic findings after M-VAC therapy are squamous metaplasia, necrosis, fibrosis, and persistent transitional cell carcinoma. In 2 cases, there were persistent adenocarcinoma and squamous cell carcinoma. The importance of these observations in terms of diagnostic and therapeutic implications is reviewed.

TITLE: Black/white differences in non-treatment of bladder cancer patients and implications for survival.

Analysis of 20,764 White and 882 Black bladder cancer patients diagnosed during 1978-85 indicates that Black patients were more likely than White patients to go untreated following diagnosis after adjustment for age- and stage-at-diagnosis, sex, and tumor histology (OR = 1.80, 95% CI = 1.33, 2.43). Treatment status was found to be a significant predictor of five-year survival after adjustment (treated/untreated odds ratio = 3.16, 95% CI = 2.08, 4.79). Results suggest that differences in initial therapy may contribute to the survival differential between Black and White bladder cancer patients.

From 1946 through 1982, 30 cases of distant malignant disease that metastasized to the thyroid were diagnosed at the Mayo Clinic. Although metastatic thyroid lesions are not uncommon, they generally do not pose a problem because they are small or are associated with evidence of obvious widespread metastatic disease. The most common sources of such lesions are the kidney, breast, lung, and lymphoid tissue. The thyroid metastatic lesion may appear many years after the identification and treatment of the primary cancer. Because the thyroid lesion may be the only important malignant disease remaining, adequate surgical treatment may prove to be life-prolonging or life-saving.

TITLE: Combined modes of imaging for liver hemangioma in cancer patients.

Twenty-nine consecutive patients with single or multiple liver hemangioma were assessed with more than one mode of imaging. Static scanning with 99Tcm was done in 21, ultrasonography in 28, CT in 29 and angiography in 27. Fine-needle percutaneous biopsy was unwisely performed in 7 cases with a single but singular complication, severe peritoneal lymphorrhea. Four laparotomies and 2 laparoscopies completed the investigations. Fourteen patients had a history of primary cancer, and 3 of these had a secondary coexisting with the angioma. CT was characteristic; ultrasonography was sensitive and perhaps specific enough for small angiomas. The 2 combined plus angio-CT greatly reduce the need for hepatic angiography.

Although the actual incidence and severity is not well documented, pain may be experienced at some point by the majority of persons with cancer. Pharmacologic management, by itself, is often not adequate. Nurses must become familiar with non-pharmacologic interventions, to be used alone or in combination with analgesics, for the successful management of cancer pain. This paper discusses various non-pharmacologic options and includes a nursing protocol. More research is warranted to better define those most likely to benefit from these interventions.

Bronchial carcinoma is known to give rise to widespread haematogenous metastases, but there are certain sites where these metastases are more likely to occur--bones, brain, liver and adrenal glands. A case is reported in which undifferentiated squamous carcinoma of the bronchus spread to two extremely uncommon sites, the heart and the penis.

TITLE: Simultaneous cloacogenic carcinoma in dizygotic twins.

Dizygotic twins developed cloacogenic carcinoma of the anus almost simultaneously. The patients, although separated from the time they were 20-years-old, had very similar life styles. There are several reports in the medical literature of synchronous tumors in mono and dizygotic twins. It is recommended that if a cancer diagnosis is made in one twin, the other undergo workup to exclude the presence of a tumor with similar histology. The establishment of state or national twin registries would provide valuable information regarding the role of genetic and environmental factors in the development of not only cancer but also of various nonmalignant disorders.

One hundred eighteen patients with lung cancer were retrospectively analyzed to determine whether postoperative radiotherapy (RT) improves survival. Patterns of treatment failure and three year NED (no evidence of disease) survival rates were assessed according to extent of tumor spread, histology, and treatment method. Patients with hilar or mediastinal node metastases were at higher risk of local failure compared to those with negative nodes. Postoperative RT reduced local recurrence and improved 3 year survival among patients with positive nodes. However, postoperative RT did not improve survival among those with negative nodes. Our data indicated that patients with positive hilar or mediastinal nodes may require postoperative RT to improve survival.

The ability of liposomal carriers to act as immuno-adjuvants for carcinoembryonic antigen (CEA) has been evaluated. Liposomal incorporation was consistent with association with the aqueous phase of the liposome, little if any of the protein being associated with the phospholipid bilayer. The incorporation of CEA within liposomal carriers resulted in immunological recognition in mice at doses (0.1 microgram) significantly less than required in Freund's complete adjuvant (25 micrograms), maximal responsiveness being found with liposomal-CEA preparations containing a lipophilic immunoadjuvant, N-acetyl-muramyl-L-alanyl-D-isoglutamyl-glyceryl-dipalmitate. Such liposomal formulations may have utility as immunoadjuvants for cancer immunotherapy.

TITLE: Hepatic squamous cell carcinoma with hypercalcemia in liver cirrhosis.

Primary squamous cell carcinoma of the liver is exceedingly rare and has previously been reported in association with hepatic teratoma, hepatic cyst or hepatolithiasis. This paper describes an autopsy case of squamous cell carcinoma which developed with hypercalcemia in a cirrhotic liver. This cancer was characterized histologically, immunohistologically and ultrastructurally, and was found to exhibit immunofluorescence positivity for anti-epidermal keratin monoclonal antibody, together with the presence of tonofilaments scattered sparsely in the cytoplasm of the cancer cells.
