Preneoplastic lesions are more common than clinical cancer and define a population at increased risk for the development of malignancy. Recent studies suggest that the immune system has the capacity to recognize these lesions, and enrichment of preneoplasia-specific immune effectors can be detected in the tumor bed of some preneoplastic lesions such as monoclonal gammopathies. Here, I discuss the promise and challenges of harnessing the immune response against preneoplasia. Approaches to boost the natural host response to these lesions may have a major impact on reducing net cancer burden.

Testicular cancer is curable by conventional cytotoxic drugs. Despite these excellent results, a group of patients remain incurable with standard chemotherapy, including patients who progress despite high-dose salvage chemotherapy, patients with malignant transformation of teratomas and patients with late recurrence. Tyrosine kinase receptors constitute a group of proteins which includes EGFr (Epidermal Growth Factor receptor) and c-Kit. The recent possibility of treatments targeted to the molecular level, particularly using agents directed against c-Kit and EGFr raise new hopes for the effective treatment of refractory testicular cancer.

In in vitro test systems, chrysotile is markedly toxic, causes chromosomal aberrations, and is capable of inducing morphological and preneoplastic transformation. In carefully designed animal experiments, chrysotile produces lung cancer and mesothelioma as effectively as do the amphiboles tested. Human population studies do not refute these experimental results. Chrysotile asbestos is carcinogenic to humans, especially for the induction of lung cancer and mesothelioma in exposed populations. For cancers of other sites, with the exception of laryngeal and possibly gastrointestinal cancer, the evidence for association with exposure to all forms of asbestos, including chrysotile, is not yet adequate for evaluation.

Ablative surgery for pyriform sinus cancer often leaves a defect which cannot be closed primarily. Many techniques for hypopharyngeal reconstruction have been described but no single technique can be recommended for use in all situations. The prevertebral fascia flap described in this article provides an alternative to hypopharyngeal repair when a more complex technique is contra-indicated. It is a one stage procedure, fashioned with locally available tissue, and technically simple. Its use in two anecdotal cases is described herewith.

The value of screening for prostate cancer remains unclear. Although digital rectal examination, transrectal ultrasonography, and determination of serum prostate-specific antigen levels may lead to early detection of a malignancy, these procedures have never been shown to reduce disease-specific mortality from prostate cancer. Unfortunately, several potential errors found in uncontrolled trials may suggest benefit from screening where none exists. Only a large, randomized, controlled clinical study demonstrating decreased mortality from prostate cancer can prove that screening is beneficial. Until such a study is performed, patients should be informed of both the potential benefits and the risks of screening and treatment.

The purpose of this study was to clarify the indication of prophylactic hepatic arterial-infusion chemotherapy (PHAIC) after hepatic resection for liver metastases from colorectal cancer. Sixty-one patients underwent curative hepatectomy, and 27 of them were treated with PHAIC using implantable port. According to clinicopathological factors of primary colorectal cancer and liver metastases, the prognoses were analyzed. In conclusion, PHAIC was not useful for patients who had distant lymph node metastases (paraaortic lymph node) from primary cancer.

The therapy of paraneoplastic neurologic syndromes remains unclear and warrants a systematic review of the literature. Reports in English and foreign language literature were abstracted. Data were sorted by the primary paraneoplastic neurologic syndrome, the primary malignancy, and the methods of treatment. Neurologic improvement follows surgical, chemotherapeutic, and radiation treatments. Adjuvant immunosuppressive therapy with steroids, plasmapheresis, or immunoglobulin may help stabilize the progression of neurologic symptoms. Syndromes characterized by inflammation or neurotransmitter production without neuronal loss are most responsive to therapy. While treatment of the underlying cancer with appropriate surgery, chemotherapy, and radiation is recommended, the paraneoplastic syndrome should probably be managed with immunosuppressive therapy.
