TITLE: [Enzymes of anabolic and catabolic nucleic acid pathways in human hepatomas, in liver of healthy persons, and in liver of patients with cancer of gastrointestinal tract]

The activities of enzymes catalyzing the formation of nucleic acid precursors, nucleoside kinases, as well as of those involved in the degradation of nucleic acids, were studied in nuclei of the liver of healthy persons, human hepatomas and the liver of patients with cancer of gastrointestinal tract. The activities of thymydine kinase and uridine kinase in the human hepatoma nuclear sap were found to increase 40- to 50-fold and 120- to 150-fold, respectively, as compared to those in normal human liver. The activities of DNase and RNase in the fraction of chromatin protein of human hepatomas, on the contrary, decreased almost to zero. As to the liver of patients with cancer of gastrointestinal tract, drastic alterations in the activities of nucleoside kinases and nucleases in the direction characteristic of tumors themselves were observed. This phenomenon is regarded as a manifestation of the systemic effects of the tumor on the host.

Experiments with Chinese hamster cells have shown that neutron irradiation does not result in repair of potentially lethal damage (PLD), i.e., that which can be influenced by changes in environmental conditions following irradiation. Since PLD is presumed to be repaired in tumors but not in normal tissues, this absence of differential sparing of tumor cells relative to normal tissues--a feature characteristic of irradiation with x rays--represents an advantage of neutrons in addition to their reduced oxygen effect. At a given dose, the difference in relative biological effectiveness (RBE) between tumors and normal tissues corresponds to a 5% increase in tumor dose with no concomitant increase in dose to normal tissues, which could be significant in cancer therapy.

Primary carcinoma of breast was treated in 967 patients from 1962 through 1972. Thirty-five of these patients had a second primary tumor of which 50% were discovered simultaneously. A family history for cancer was recorded in 26% of the patients with bilateral cancer. Patients found their tumor more often than the physician except in the simultaneous cases where the physician was more successful. Pathological examination showed 10% of the tumors were comedo, lobular carcinoma in situ, papillary, or tubular cancers. In the metachronous group, 50% of the axillae were involved on both sides. In the simultaneous cases the axilla was less frequently involved. Mammography, biopsies of the second breast, and prophylactic mastectomy in certain high-risk patients should improve survival rates.

We practiced a systematic endoscopy of the upper part of the aero-digestive tract on 160 patients suffering from a cancer in the mouth, the pharynx and the upper part of the larynx. This unit, going from the mouth to the cardia does really exist: it has the same epithelial coating, the same function and is liable to suffer the same damages. It is the seat of many well-known cancerous localizations that always need to be searched for on principle when face to face with any cancer. The endoscopic examination so conceived allowed the authors to find a cancerous association on the very level of the upper sero-digestive tract among 14 per cent of the patients; 4 out of these 14 per cent having oesophageal damages impossible to detect clinically on one hand, and often radiologically on the other hand. Morever, 3 out of 7 of lesions were tiny: from 1 to 3 millimetres, extending the limits of the diagnosis of the cancer of the aesophagus and perhaps leaving better therapeutic possibilities.

31P-NMR spectra were obtained from the perchloric acid extracts of normal lung and lung cancer tissues obtained at surgery, and from extracts of neoplastic cells cultured in vitro. The perchloric acid extract of lung cancer tissue gave rise to a signal whose chemical shift was 3.2 ppm at pH 8.0. This signal was not observed in the extract of normal tissue of the lung from the same patient. The compound giving this signal was identified as phosphorylcholine.

The analgesic effect of porcine calcitonin (PCT) was studied in 32 patients with cancer-induced bone destruction. Twenty patients (62.5%) showed some response among them 10 of 13 patients who had suffered from uncontrollable pain despite other treatment methods such as radiotherapy, and or the administration of other analgesics. The effect was observed about 10 days after the administration of the drug. Combined therapy with PCT and other modalities such as radiotherapy, chemotherapy and hormone therapy had a marked effect in controlling the pain.

Non-infected urine samples of twenty six preoperative bladder cancers were examined by urinary CEA radioimmunoassay and exfoliative cytology. There were 65% of urinary CEA positivity and 58% of cytological positivity (class III, IV, V). CEA measurements were positive in 8 of 11 negative cytology cases (class I and II). Exfoliative cytological examination failed to detect most of well differentiated transitional cell carcinomas, while urinary CEA study detected 4 of 6 such cases. Combination of both urinary CEA measurement and cytology resulted in 85% of positivity. Therefore we recommend routine use of this combination as a screening system of bladder cancer.

The risks are, of course, lower than those to smokers themselves; but smoking is self-inflicted, passive smoking is involuntary. Making various assumptions, the proportion of lung cancer cases among non-smokers that could reasonably be attributed to environmental tobacco smoke can be calculated to be about 20-30% in western countries. Thus, non-smokers in the society could benefit considerably from diminishing exposures to other people's smoke.

Over the past several years, we have utilized methotrexate, vinblastine, doxorubicin, and cisplatin (M-VAC) as definitive, neoadjuvant, and adjuvant therapy for various stages of transitional cell carcinoma with reduced toxicities. Currently, there is little information concerning the histopathologic changes after M-VAC therapy. We report on the histopathologic changes of bladder transitional cell carcinoma following M-VAC chemotherapy in our protocol for treatment of bladder cancer. The main histopathologic findings after M-VAC therapy are squamous metaplasia, necrosis, fibrosis, and persistent transitional cell carcinoma. In 2 cases, there were persistent adenocarcinoma and squamous cell carcinoma. The importance of these observations in terms of diagnostic and therapeutic implications is reviewed.

Analysis of 20,764 White and 882 Black bladder cancer patients diagnosed during 1978-85 indicates that Black patients were more likely than White patients to go untreated following diagnosis after adjustment for age- and stage-at-diagnosis, sex, and tumor histology (OR = 1.80, 95% CI = 1.33, 2.43). Treatment status was found to be a significant predictor of five-year survival after adjustment (treated/untreated odds ratio = 3.16, 95% CI = 2.08, 4.79). Results suggest that differences in initial therapy may contribute to the survival differential between Black and White bladder cancer patients.

The innervation of the human spleen was investigated by the fluorescence method for adrenergic nerve fibers (52 cases) and the enzyme histochemical method for cholinergic nerve fibers (27 cases). The spleens examined were removed by laparotomy chiefly for gastric cancer. Adrenergic nerve fibers were demonstrated at the medioadventitial junction and the media in the trabecular arteries, whereas they occurred at the medioadventitial junction in the central and penicillar arteries. They were not found in the trabecular and pulp veins and venules, sheathed capillaries, avascular trabeculae, white and red pulp, and capsule. Nerves containing only a few adrenergic fibers and nerves without adrenergic fibers occurred close to the trabecular arteries. It was also demonstrated that nerves close to the trabecular arteries contain cholinergic fibers. The present study first verifies histochemically that the human spleen is innervated doubly by adrenergic and cholinergic nerve fibers.

Different mammographic parenchymal patterns have been found to be associated with significantly different rates for the development of breast cancer in a screening program of self-referred women. These differences are qualitatively similar but of lesser magnitude than those in previous reports by Wolfe which were based on symptomatic women who had had a previous negative mammogram. In addition, this report indicates a small difference in the rate of breast cancer at first mammographic examination, using the same parenchymal classifications. These findings, coupled with other risk factors, may permit the concentration of mammographic screening on a smaller segment of the population at risk, thus improving the benefit to risk ratio.
