TITLE: [Fortuitous diagnosis of an undifferentiated large cell carcinoma during cardiac surgery]
PUBDATE: 19990701
A 57 years old man was operated for aortic valve replacement. During the operation a mediastinal lymph node was biopsed with the result of undifferenciated large cell carcinoma with neuroendocrine features. Three months after surgery the patient died of non-cardiac related reasons, postmorten examination was not performed. Although the primary tumor could not be assessed, probably the lung was the original location. The histological classification of lung cancer is exposed, with special reference in the undifferenciated large cell type with neuroendocrine features.

PUBDATE: 20010901
The Women's Health Alliance Intervention Study is a community-based intervention study designed to determine if county-based coalitions can increase breast and cervical cancer screening rates among women aged 40 and older in four rural Wisconsin counties. This article describes the intervention process and demonstrates the feasibility of rural volunteer coalitions to promote breast and cervical cancer screening in communities. An accompanying article in this issue describes the effectiveness of these interventions.

Li-Fraumeni syndrome(LFS) and its relation to the p53 gene and p53 gene transgenic and targeting mice are reviewed. LFS is a hereditary cancer-prone syndrome mainly with germ line mutation of p53 gene. p53 deficient mice, especially heterozygous mice, may serve as a model for human LFS, although there are several discrepancies between them. Carcinogenesis experiment using p53 deficient mice may be important to clarify the role of p53 in tumorigenesis in vivo.

PUBDATE: 19981001
The current surgical standard of care for the management of invasive breast cancer is the complete removal of the cancer with documented negative margins by either mastectomy or lumpectomy followed by complete axillary lymph node dissection. These procedures can be associated with significant morbidity which leads to increased hospitalizations, increased overall costs and considerable discomfort and life-style changes for the patients. With sentinel node identification and biopsy, patients with probable negative lymph nodes may avoid axillary dissection, and easily be treated as outpatients.

The role of cytoreductive nephrectomy in the management of metastatic renal cancer remains controversial. Recent trials, like SWOG 8949 have suggested the usefulness of this approach at least in selected patients with good performance status and other favorable indicators. The timing of cytoreductive nephrectomy has also been controversial and remains so to this time.

We examined the effects of rhHPO on the cell growth and DNA synthesis of both rat primarily cultured hepatocytes and hepatic carcinoma cell line in vitro by MTS and 3H-TdR in corporation methods. It was indicated that rhHPO is an important stimulating factor of regeneration, which may be developed as a potential drug for the treatment of severe hepatic diseases. We also found an inhibitory effect of rhHPO on the DNA synthesis of lung cancer cell lines GLC-82 in vitro, which might provide a valuable indicator for the study of its specificity and mechanisms.

TITLE: Limbic encephalitis and antibodies to Ma2: a paraneoplastic presentation of breast cancer.
PUBDATE: 20000801
A patient with atypical medullary breast cancer is described who presented with symptoms of limbic encephalitis. The patient's serum and CSF contained antibodies that reacted with the nervous system and the tumour. These antibodies recognised Ma2, a neuronal protein related to paraneoplastic limbic and brainstem encephalitis in men with testicular tumours. This report highlights the importance of testing for paraneoplastic antineuronal antibodies in cases of unexplained limbic encephalitis and suggests screening for breast cancer in women with antibodies predominantly directed to Ma2.

In 1998 and 1999 three new charity foundations were established in the Netherlands, covering the same areas as the Dutch Heart Foundation, the Netherlands Cancer Foundation and the National Rheuma Foundation respectively. Analysis of the set-up of the three new foundations, of their philosophy, their constitutions and the background of the persons involved shows a remarkably uniform pattern, making it very likely that the set-up of these alternative foundations is a part of the marketing strategy of some Dutch wholesalers in orthomolecular drugs. More legal regulation of charity foundations is seriously needed.

The standard surgical treatment for stage I, II, and IIIa non-small cell lung cancer (NSCLC) is lobectomy with mediastinal lymph node dissection. In our series of 214 patients with N2 disease the 5-year survival rate was 23%. Systematic mediastinal lymph node dissection provides further prognostic information on the N factor. Among NSCLC patients with a pathological diagnosis of N2, there are significant differences in the 5-year survival rate based the 3 factors of clinical N factor, N1 station metastasis, and N2 station metastasis.

From pregnancy to breast cancer screening and osteoporosis prevention, women have a need for a gender-specific set of health services. Dialogue that once occurred only between women and their physicians has evolved into debate between representatives from the insurance industry and state legislators. Therefore, women's health issues have risen on the legislative agenda, illustrated by the increased volume of bills introduced and enacted during the past five years.

The development of brain metastases in patients with advanced cancer is associated with increased morbidity and diminished longevity. Palliative treatment modalities focus predominantly on improving quality of life. Well-established beneficial treatments include the use of corticosteroids, radiotherapy, and surgery. Management issues that require further exploration and clarification include the dosages of glucocorticoids, the prophylactic use of anticonvulsants and the indications for radiosurgery.

TITLE: Benign oral disease as a cause of false-positive iodine-131 scans.
PUBDATE: 19981101
Post-thyroidectomy I-131 whole-body scintigraphy is an important aspect in the management of patients with thyroid cancer. The scan is used to detect residual thyroid tissue and distant functioning metastases. Extrathyroid I-131 accumulation does not always denote metastatic disease, and several potential sources of false-positive I-131 scans have been identified. This report presents four cases of benign oral disease as a cause of false-positive I-131 uptake. Benign oral disease has not been documented previously as an important source of false-positive I-131 whole-body scans.
