TITLE: Adult cancer risks among the mothers of children with cancer.

We investigated cancer risk among mothers of 2365 children who were diagnosed with cancer between 1973 and 1989 in Sweden. From the date of birth of the child until 31 December 1989, 38 cases of cancer were diagnosed among the mothers. The expected number of cases, according to national rates, was 30.9. Cancer of the thyroid was the only site showing a significantly increased risk among the mothers (observed = 6, expected = 1.2, P < 0.01), but two cases were medullary cancer associated with an inheritable syndrome, and inherited by their children.

TITLE: Pregnancy coexisting with serous papillary adenocarcinoma involving both uterus and ovary.

Less than 100 cases have been reported involving coexistent pregnancy and endometrial cancer of epithelial ovarian cancer. Only one case, histologically an endometrioid adenocarcinoma, demonstrated simultaneous involvement of endometrium and ovary. We report an additional case involving both uterus and ovary coexisting with pregnancy but of a serous papillary histology. A viable preterm infant was delivered. The mother is currently without evidence of disease on cisplatin-based chemotherapy.

Since linkage of the familial breast-ovarian cancer gene (BRCA1) to chromosome 17, the critical region has been narrowed to 1.0-1.5 Mb by recombination studies, a detailed physical map has been constructed and much of the region has been cloned in yeast artificial chromosome, bacteriophage P1 and cosmid vectors. The focus now lies on identifying the genes housed within the BRCA1 region and scanning them for oncogenic mutations.

Primary human cells which keep in contact with each other show heat resistance, but cancer cells do not. Cytologic methods for detection of early cancers of the uterine cervix, lung and various other organs are discussed. The scraping smear method using a spatula is more effective than the cotton swab or vaginal pool smear method for detection of preinvasive intraepithelial lesions, such as, carcinoma in situ and dysplasias of various degrees of the uterine cervix. The use of sputum specimens pooled for three to five days is recommended for cytologic examination in population screening of lung cancer. Good cytopreparatory techniques, suitable screening and cytodiagnostic classifications of malignancy are also described and emphasized, especially, the importance of properly fixed cytologic material for correct cytopathological diagnosis.

A 68-year old woman underwent transarterial oily chemoembolization using Lipiodol with epirubicin hydrochloride and mitomycin C for treatment of multiple hypervascular hepatic metastases of gastric cancer. The tumors showed a good fixation of Lipiodol with a complete biologic response. The tumor has been well controlled for eight months by only one chemoembolization on follow-up study. Transcatheter oily chemoembolization may be an effective treatment not only for hepatocellular carcinoma, but also for hypervascular liver metastases from gastrointestinal malignancies.

Serum levels of adenosine deaminase (ADA), 5-nucleotidase (5'-NT) and alkaline phosphatase (ALP) were studied in 25 patients of carcinoma breast and 25 normal subjects. Adenosine deaminase was found to be the better probable parameter for the detection of cancer and to assess the development of various stages of cancer whereas 5'-nucleotidase had only diagnostic significance. Serum alkaline phosphatase levels were important for assessing the spread of cancer at secondary sites. After mastectomy a significant decrease was found in the levels of serum ADA and 5'-NT whereas no variations were found in case of serum ALP.

Clostridium septicum bacteremia is frequently associated with hematologic and colonic malignancies and neutropenia. It frequently produces 'metastatic' gangrene with excessive mortality. Standard therapy usually includes surgical debridement and antibiotics. We present a patient with metastatic breast cancer treated with high-dose chemotherapy and bone marrow transplantation. She was treated successfully with antibiotics alone despite developing Cl. septicum bacteremia and gas in hepatic metastases. The pathophysiology of this infection is reviewed.
