In the period from 1961 to August, 1975, 2,222 microlaryngoscopic examinations were carried out at the ENT Department of the University. A correlation was seen between the increase in the number of examination in the last few years and the increase in the number of malignant tumours of the larynx, while malignant growths in the hypopharynx were found to be only slightly increased. An anlysis of 1,027 microlaryngoscopies carried out under anaesthesia over the last 2 2/3 years showed almost 50% cancers and precancers. In 5% of the case at most more rare laryngeal findings were also seen. Of these, the granular cell tumour, plasmocytoma, oncocytic cystadenoma and amyloid-tumour of the larynx, but also tuberculous laryngitis, are represented. The modern endoscopic examinations and the fundamental bioptic histomorphological examination of proliferations allow an exact differentiation of these changes in the tissues of the endolarynx. In this connection, a valuable technical aid is the v. Stuckard low-power magnifying laryngoscope, that has been in use for the past six months.

Circulating carcinoembryonic antigen (CEA) levels and their relation to liver function test values were studied in 29 jaundiced patients with benign extrahepatic biliary tract obstruction and inflammation. During the obstructive and inflammatory phase, 15 (52%) of the patients had CEA levels greater than 2.5 ng/ml. Elevated CEA levels were associated more frequently with common bile duct stones (and cholangitis) than with gallbladder stones (and cholecystitis) alone, although this difference was not statistically significant. The former often had values greater than 5.0 ng/ml. The highest values were found in two patients with liver abscesses. T'HE CEA levels returned to normal following relief of obstruction in seven of ten patients and increased in two patients who had progressive inflammation. Serum alkaline phosphatase and bilirubin levels were significantly higher in the patients with elevated CEA levels (p smaller than .05). Serum alkaline phosphatase levels showed a significant positive correlation with CEA levels (p smaller than .02). Patients with obstructive jaundice and elevated CEA levels do not necessarily have cancer.

On the basis of theoretical and experimental research work on substrate supply, cell kinetics and therapy mechanisms in the intercapillary region, therapeutic steps are formulated which act on cancer cells situated at a greater distance from the capillaries and, therefore, against the genesis of cancer recurrence as such.

TITLE: Frequency of antibody to a virus-induced tumor-associated antigen (AG-4) in Japanese sera from patients with cervical cancer and controls.

Microquantitative complement fixation was used to detect antibody to AG-4, a tumor-associated herpesvirus type 2-induced antigen, in sera from Japanese patients with cervical neoplasia. The prevalence of antibody to AG-4 in cervical cancer patients is 47% as compared to 7% in control women. That AG-4 antibody is less prevalent in Japanese than in American cancer patients reflects the frequency of genital herpesvirus type 2 infections in Japan. These findings confirm the association of AG-4 with cervical cancer in a proportion of Japanese patients with neoplastic disease; they implicate factors other than herpesvirus type 2 in another proportion of cervical cancer patients. The possibility that herpesvirus type 1 is one of these factors is considered.

This paper discusses the hospitalization of aviators at the Naval Regional Medical Center, San Diego, Ca. The areas covered include cancer, trauma, cardiac cases, alcoholics, and the effect of hospitalization on the flying status of aviators. The variety of diagnoses accumulated in 2 years was suprising. Further follow-up of cardiac cases must be pursued, especially post-coronary artery bypass. Alcoholism as a diagnosis in hospitalized patients may be a larger problem than elicited in this series. Hopefully this paper will not be just a statistical study. We should think of prevention and care to decrease the number of aviators who have their wings clipped due to medical problems.

C3H-A-vy and C3H-A-VY-FB mice transferred from our colony to the laboratory of Dr. J. R. Sabine in Australia had a markedly reduced incidence of both hepatomas and mammary tumors. Sabine and co-workers (J Natl Cancer Inst 50: 1237-1242, 1973) attributed the low incidence of tumors in their laboratory compared to the high incidence in this laboratory to the fact that we routinely use red cedar shavings in the bedding, whereas they use sawdust, predominantly Doublas fir. Results of tests in this laboratory showed that the difference in occurrence of tumors could not be attributed directly to either a difference in diet or bedding. There was no evidence that the cedar shavings were carcinogenic. The animals in Australia on bedding without cedar shavings were not as healthy as our animals, in that they did not grow as well and were infested with ectoparasites; apparently these factors caused the reduced occurrence of tumors.

The suppression of tumor-specific cell-mediated cytotoxicity by human immunoregulatory alpha-globulin (IRA), by a peptide fraction derived from IRA, and by IRA-like peptides from the serum of cancer patients was studied in a syngeneic murine tumor-host system. Splenic lymphocytes from tumor-immunized mice were cytotoxic specific tumor cells in vitro as measured by the [125]-iododeoxyuridine release microcytotoxicity assay. However, this effect was significantly depressed if 1.25 to 5 mg of IRA per ml were added to the cultures. Pooled lyophilized normal human serum protein was inactive. IRA peptide and IRA-like peptide fractions from cancer patients were also highly suppressive of cell-mediated cytotoxicity at much lower concentrations (0.05 to 0.5 mg/ml). Control human serum peptide, which failed to inhibit the induction of hemolytic plaque-forming cells in sheep erythrocyte-injected mice, had no effect on cell-mediated cytotoxicity. IRA and IRA-like peptide fractions were not cytotoxic to the effector lymphocytes or to the target cells at the concentrations used.

TITLE: Migration inhibition with various cell fractions in human colorectal cancer.

The unseparated leucocytes, separated mononuclear cells and granulocytes of six control subjects and nine patients with colorectal cancer have been studied by a direct cell migration inhibition technique. A migratory index was calculated from the migration in the presence and absence of a perchloric acid extract of large bowel tumours. In 10% homologous AB serum, no significant migration inhibition occurred with any of the cells from control subjects. Five of the nine cancer patients showed significant inhibition with their unseparated leucocytes, seven of seven with their mononuclear cells, and none of nine with their granulocytes. In 10% autologous serum, some controls exhibited migration inhibition with their unseparated leucocytes and their granulocyte fraction, but not with the mononuclear cell fraction. Migration inhibition was also now apparent in the granulocyte fraction of the cancer patients. It is concluded that, with a soluble tumour antigen preparation, a mononuclear cell population increases the sensitivity of the direct migration inhibition test and that autologous serum may interfere directly with the migration of granulocytes, by an action not dependent upon the release of inhibitory factors from sensitized lymphocytes. This could explain some of the inconsistencies of the assay when using an unseparated leucocyte population.

Three types of colitis are now recognized. In the late 1950's and early 1960's, 51 per cent of our patients having colectomy had classic chronic ulcerative colitis. Of our patients operated on at that time, 39 per cent had Crohn's colitis and 10 per cent had colitis of indeterminate type. These three types of colitis should be differentiated because of the high risk of cancer of the colon in patients with long-standing chronic ulcerative colitis. We have not seen an increased risk of carcinoma associated with Crohn's colitis. We performed resection with anastomosis in approximately one third of patients with Crohn's colitis, but recurrent disease developed in two thirds. In contrast, recurrent Crohn's disease developed in only 5.7 per cent of patients having colectomy with ileostomy. Toxic megacolon associated with Crohn's colitis has been seen only in the early clinical stage of the disease. From the standpoint of management and prognosis, attempts to differentiate between the two major types of colitis are appropriate. We have outlined some of the basic clinical and pathologic differences between these two types of colitis.

A low-dose, three-drug regimen, C.M.F. (cyclophosphamide 50 mg, p.o., days 1-14; methotrexate, 25 mg, and 5-fluorouracil, 500 mg, i.v., days 1 and 8; cycled every 28 days) was used in 46 consecutive chemtherapy-eligible women (41 previously hormonally treated) with recurrent breast cancer. Thirteen percent of the patients had complete regressions (C.R.); 33% had partial regressions (P.R.); 26% stabilized; and 28% progressed. In evaluating response by sites of metastases, lymph nodes (30%), lung nodules (22%), and subcutaneous deposits (2/3) had the highest incidence of C.R.; 46-71% of patients with lymph node, lung, subcutaneous, liver, breast, or peritoneal disease showed C.R. or P.R. Skin and pleural disease responded in 30% of patients whereas no patients had radiographic healing of bony metastases. The toxicity was minimal: 7% gastrointestinal, 26% marrow-suppressive, and 7% infectious. This low-dose C.M.F. regimen resulted in regression resulted in regression rates similar to higher dose C.M.F. protocols, which use approximately twice these drug dosages with commensurate toxicity.

TITLE: Cervical cancer control: a study of morbidity and mortality trends over a twenty-one-year period.

Data from a 21-year period are presented to evaluate the effects of a mass cytologic screening program on uterine cancer morbidity and mortality in Louisville, Jefferson County, Kentucky. The success of screening was greatest in the younger age groups. There was a fall-off after age 45 years, especially in those age 60 years or older. Women at highest risk for cervical cancer, in the low socioeconomic quartile, had a better initial screening rate than the two middle-income quartiles, and had the highest rate of all women for subsequent rescreening. The greatest decrease in both morbidity and mortality was in women under the age of 50 years. Women age 30-39 and 50-59 years benefitted the most, as measured by mortality, with a decrease of 70.8 and 69.0%, respectively. There was no change in mortality rates for those age 70 years or older. Although the average annual age-adjusted rate increased from 13.2 to 15.3/100,000 women over the 21-year period.

Estrogen use for treating symptoms of menopause and in oral contraception is under growing attack Recent findings have shown negative correlations between oral contraceptive (OC) use and myocardial infarction, elevated risk of breast cancer, endometrial cancers among wo men between 21 and 39 years of age (especially among those using sequent ial pills) and benign tumors of the liver. Studies have also linked prolonged estrogen therapy in menopausal women with increasing incidence of endometrial cancer. An estimated 5.6-fold increase in endometrial cancer risk has been figured for women who have been using estrogen from 1 to 4.9 years. Consequently, the FDA has announced plans to label all estrogen products with strong restrictive warnings. There is a possibility that sequentials will be removed completely from the market The increasing number of reports of various effects of estrogen still await resolution.

Tumor blood flow, measured by an isotope dilution technique, is only 2% to 15% of that of the surrounding tissue. This sluggish circulation differentiates cancer from normal tissue and forms the basis for a new therapy that entails the transfer of radiofrequency (RF) energy for heating tissues locally. The heated tissue is cooled by the circulation of blood, which carries away the heat. Impaired perfusion, as in cancers, impedes cooling. Heating by RF elevated the temperatures of animal and human cancers by 5 to 9.5 C above that of healthy tissue. The heat eradicated the animal cancers without destruction of normal tissue. Radiofrequency therapy produced tissue necrosis or substantial regression of cancer in 21 patients.

In an attempt to improve the survival rates in women with advanced ovarian adenocarcinoma (abdominal spread), they were treated with sequential therapy. This consisted of operation followed by chemotherapy, second-look exploratory laparotomy, and finally whole-abdomen irradiation. Only those patients who had a complete clinical response to chemotherapy and subsequently underwent a second operation and irradiation were evaluated. Seventy-five per cent of the patients had recurrent cancer within the treated area after sequential therapy was completed. Morever, of four patients with no gross residual cancer after the second operation, three had cancer recurrences within the treated area. It is concluded that methods of treating advanced ovarian adenocarcinoma other than sequential therapy must be found.

TITLE: Preneoplastic lesions of the human mammary gland transplanted into the nude athymic mouse.

Morphologically normal lobules and atypical lobules postulated precancerous to ductal carcinoma were transplanted to test their biological behavior. Supravital staining disclosed the 1 to 4-mm microorgans. "Cleared" mammary fat pads of nude mice were optimal transplantation sites. Of the total of 217 transplants from 19 cancer-associated and 13 non-cancer-associated breast, 151 survived after 2 to 27 weeks. Of 61 surviving normal-appearing lobules from cancer-associated breasts transplanted without prior in vitro maintenance, 20 (30%) dedifferentiated, and of 48 surviving lobules from noncancerous breasts, 11 (20%) dedifferentiated. Fifteen of 28 histologically normal-appearing lobules (60%) obtained from cancer-associated breasts after age 50 dedifferentiated. Thirty of 36 atypical lobules isolated from the breast tissue were obtained from cancer-associated breasts and the 5 of those that dedifferentiated came from cancer-associated breasts. Twenty of 22 (90%) dedifferentiating transplants from cancer-associated breasts showed a vascularization response, whereas 3 of 7 (43%) from noncancer-associated breasts did so. If dedifferentiation in this experimental setting is indicative of a precancerous potential, the data on normal-appearing lobules obtained from cancer-associated breasts from women over age 50 suggest that these lobules carry the greatest precancerous potential. Such lobules probably belong to a type persisting after menopause and they may be hormonally autonomous. Such lobules might undergo further atypia in vivo and, eventually, cancerous transformation.
