TITLE: Adult cancer risks among the mothers of children with cancer.

We investigated cancer risk among mothers of 2365 children who were diagnosed with cancer between 1973 and 1989 in Sweden. From the date of birth of the child until 31 December 1989, 38 cases of cancer were diagnosed among the mothers. The expected number of cases, according to national rates, was 30.9. Cancer of the thyroid was the only site showing a significantly increased risk among the mothers (observed = 6, expected = 1.2, P < 0.01), but two cases were medullary cancer associated with an inheritable syndrome, and inherited by their children.

A survey of recent Texas dental school graduates regarding their preparedness for addressing the oral health care needs of cancer patients in their practices suggests several trends. Many graduates (26%) do not feel adequately prepared to manage and provide oral care for cancer patients. Over 95% of the dentists surveyed expressed interest in locally sponsored continuing education courses on the oral health care of cancer patients. Clearly, both predoctoral and postdoctoral dental education must recognize the need for formalized instruction on the oral cre of cancer patients if general dentists are to provide high-quality, comprehensive dental care for these patients.

Less than 100 cases have been reported involving coexistent pregnancy and endometrial cancer of epithelial ovarian cancer. Only one case, histologically an endometrioid adenocarcinoma, demonstrated simultaneous involvement of endometrium and ovary. We report an additional case involving both uterus and ovary coexisting with pregnancy but of a serous papillary histology. A viable preterm infant was delivered. The mother is currently without evidence of disease on cisplatin-based chemotherapy.

Since linkage of the familial breast-ovarian cancer gene (BRCA1) to chromosome 17, the critical region has been narrowed to 1.0-1.5 Mb by recombination studies, a detailed physical map has been constructed and much of the region has been cloned in yeast artificial chromosome, bacteriophage P1 and cosmid vectors. The focus now lies on identifying the genes housed within the BRCA1 region and scanning them for oncogenic mutations.

Sequential hemibody radiotherapy (SHBI) is currently being evaluated in clinical trials as adjuvant treatment in cancer patients who are potential long-term survivors. The potential late adverse effects of this therapeutic modality on the bone marrow may place patients at risk for the complications of aplasia, leukaemia, and myeloproliferative syndromes. We describe a case of acute non-lymphocytic leukaemia and myelofibrosis following SHBI for prostate carcinoma. The possible role of radiation in the induction of these abnormalities is discussed.

Massive obesity leads to nonspecific skin disorders. Skin folds are more numerous and deeper in the obese subject, and can become the site of various disorders. Hence the difficulty of diagnosis and treatment is increased. Acanthosis nigricans should be recognized, but not solely attributed to obesity, since other causes, particularly cancer, may be involved. Finally, leg ulcerations, a frequent consequence of the venous insufficiency inherent in obesity, should not lead to overlook other underlying causes, particularly in diabetic patients.

One hundred patients with advanced cancer referred to a palliative care service were evaluated for psychosocial distress using a standard questionnaire. A high incidence of emotional upset, financial distress, and home-care problems was found. Psychosocial distress is common among patients (and their families) with advanced cancer and may have a significant impact on provision of optimal care. Community services are often inadequate. A dedicated palliative care service employing a multidisciplinary approach can best provide care. We make specific recommendations about how to combat psychosocial distress using specific administrative and management techniques.

Dietary sodium saccharin is associated with bladder tumors when fed at high levels to the male rat. Under these conditions urinary pH, sodium concentration, and volume are elevated and proliferative changes are present in the urothelium. Extensive epidemiological studies have shown that saccharin does not increase the risk of bladder cancer in humans and laboratory investigations have shown that sodium saccharin is not mutagenic and does not bind to DNA. Recent research indicates that the urothelium in male rats is damaged under conditions of high urinary pH and sodium levels by a mechanism that involves alpha 2u-globulin and possibly silicate crystalluria. These studies and their implications for human health risk are reviewed.

We investigated the heat-sensitivity of normal human cells and cells derived from human cancers (cancer cells) heated at 43 degrees C under different culture conditions. Primary human cells which keep in contact with each other show heat resistance, but cancer cells do not. This does not correlate with the total cellular amount of heat shock protein 72, but rather with accumulation of the protein in the nucleus after heating.
