TITLE: Hemoccult screening for colorectal cancer.

A community hemoccult screening project for colorectal cancer resulted in the processing of 18,198 specimens. Positive test results were reported in 3 percent of the total, which ultimately resulted in the identification of 20 colorectal malignancies (0.1 percent) and 53 benign polyps (0.3 percent). The number of malignancies and polyps identified was disappointingly low. Our findings would challenge the concept of unsupervised mass screening from the viewpoint of cost-effectiveness; however, because of the numerous voluntary services and supplies, this particular project was useful in educating the public about early colon cancer detection.

In a pilot study, researchers at The Swiss Group for Clinical Cancer Research (SAKK) treated advanced lymphoma patients with a combination of DDP/VP-16/prednisone. Overall response rate was 34%. Currently, the new platinum derivatives are being studied. Meanwhile, various pilot studies are being conducted in non-Hodgkin's lymphoma (NHL) patients to evaluate the combination, VP-16/bleomycin/procarbazine/ARA-C. Preliminary results are encouraging. It is hoped that this will be an effective salvage regimen for NHL patients and also for use in an alternating regimen along with a well-established first-line treatment.

TITLE: Blood loss with use of the Shaw scalpel for the treatment of oral cancer.

The Shaw scalpel was used in seven radical operations for oral cancer. The amount of bleeding and postoperative exudate and the occurrence of postoperative complications were compared with that from 12 operations performed with the conventional steel scalpel. The blood loss during the radical neck dissection performed with the Shaw scalpel was 39% of the control value, and no blood transfusions were necessary. There was no increase in the amount of postoperative exudate in the Shaw scalpel group compared with the control group. Skin incision with the Shaw scalpel was accompanied by superficial wound dehiscences in three patients, which healed without any treatment.

A retrospective analysis of 1001 patients with invasive breast cancer showed a difference in survival between patients with different blood groups. Analysis of time from operation to local recurrence and to general recurrence reinforced this finding. The difference between blood groups became increasingly significant after accepted prognostic factors were allowed for. Patients at particularly high risk of early death or general recurrence were those with blood groups B and AB, those with group AB having a greater relative local recurrence rate.

Thirty seven cases of carcinoma of skin treated with electron-beam irradiation over a period of three years have been analysed retrospectively. Majority of lesions were in head and neck and basal-cell carcinoma was the commonest histological type. Of twenty-one patients followed, seventeen (81%) achieved complete remission and remained disease free for a period of one to three years. Cosmetic results were excellent. Role of electron-beam in the management of these superficial cancers and its advantage over the conventional radiation therapy methods have been highlighted. Radiation therapy utilising electron-beams form a very useful modality in the management of skin cancers, especially those arising in head and neck.

The concentrations of thyrotropin, thyroglobulin, calcitonin and carcinoembryonic antigen were raised in the blood of thyroid cancer patients. The occurrence of relapses of regional and distant metastases was accompanied by a sharp increase in the levels of thyrotropin, thyroglobulin and carcinoembryonic antigen permitting the recommendation of these tests for the diagnosis of distant metastases in the absence of local spread of a process. Simultaneous determination of the levels of thyroglobulins and carcinoembryonic antigen was the most informative for defining metastatic involvement of the osseous system.

The membrane fatty acid composition of cancer cells can be modified either in culture or during growth in animals without disrupting basic membrane or cellular integrity. Only fatty acids are affected; no changes occur in membrane cholesterol, phospholipid, or protein content. There are changes in membrane physical properties and certain cellular functions, including carrier-mediated transport, receptor binding, ion channels, and eicosanoid production. Fatty acid modification also can enhance the sensitivity of the cells to hyperthermia and Adriamycin. This technique provides a new approach to understanding the membrane properties of neoplastic cells. Membrane fatty acid modification also may be of potential value as a therapeutic approach designed to augment the cytotoxicity of other antineoplastic therapies.

The ability of antiandrogens to antagonize androgen effects in androgen responsive tissues is well established. Antiandrogens may diminish in vivo or in vitro proliferation of some androgen responsive cancer cells without causing cessation of multiplication. These model studies are representative of clinical experience in treatment of human prostate cancer with antiandrogen therapy. Recent studies in the AXC/SSh rat prostate cancer model show that these cancer cells elaborate polypeptide growth factors which stimulate their proliferation. If growth factor production by these cells is androgen independent, this may provide an explanation for failure of androgen ablation or antiandrogen treatment to effectively halt prostate cancer cell proliferation.

Primary hemochromatosis is a genetic disorder rarely recognized in childhood; its long-term consequences include cirrhosis and liver cancer. We report a family with primary hemochromatosis affecting three generations, including a 7-year-old child and a 29-month-old child; these are the youngest children with primary hemochromatosis yet reported. The pathophysiology, genetics, and clinical findings of this disorder are reviewed. Serum ferritin and transferrin saturation are useful screening tests; definitive diagnosis, however, depends on determination of hepatic iron content. A plan for evaluating and treating affected patients is proposed. Physicians caring for children must learn to recognize this potentially treatable disorder.

Serum tissue polypeptide antigen (TPA), CEA and IAP were measured simultaneously in cervical cancer, corpus cancer and ovarian cancer. TPA levels were increased (more than 110 U/1) in 39% (32/82) of the cervical cancer and in 46% (6/12) of the corpus cancer patients, respectively. In ovarian cancer, TPA levels were elevated (146 U/1 or higher) in 64% (13/22). The positive rate of CEA was somewhat lower than that of TPA and LAP levels were as high as TPA. Moreover, TPA levels were correlated in clinical course. In immunohistochemical examination, TPA was present in cancer cells and absent in normal tissue.

This paper reports the results of the PCS Outcome Surveys for radiation therapy treatment in six disease sites: cervix, Hodgkin's disease, seminoma of the testis, anterior two-thirds of tongue and floor of mouth, larynx and prostate. The survey reflects data obtained for patients treated in 1973. Results for early stages of disease are generally favorable in cancer of the cervix, prostate, testis, larynx and Hodgkin's disease. Control of disease in anterior two-thirds of tongue and floor of mouth was less favorable, but data suggest that more aggressive radiation therapy may contribute to better outcome for this site.
