Management of the chronic pain of cancer is a common and difficult problem. In addition to a medical examination of the patient, it is necessary to perform a psychological assessment of his premorbid personality, current mental status, and coping mechanisms to devise an individualized approach to his pain. The mainstay of cancer pain control are the narcotics, which differ primarily in potency and duration of action. Nonnarcotic analgesics are equianalgesic with the less potent narcotics. Antipsychotic drugs are useful as tranquilizers, antiemetics, and analgesic potentiators. Antidepressants and hypnotics permit the patient a more normal life-style. Stimulants such as cocaine and amphetamines both potentiate narcotic analgesia and reduce narcotic-induced somnolence and respiratory depression. Tetrahydrocannabinol offers no advantage over traditional analgesics. With care and patience, the physician can render practically any cancer patient pain-free.

Cancer mortality in Hamburg, calculated per 10,000 living inhabitants of each age group of men and women for the years 1873 to 1978 proved to be essentially constant after eliminating sources of statistical error. Calculation or organ involvement by the same method for 1933 to 1978 showed that the increase in mortality for one type of location is compensated by a decrease elsewhere, so that altogether the constancy remains. This behavior can be explained if it be assumed. This behavior can be explained if it be assumed that a certain constant percentage of the population is prone to cancer and - possibly due to the action of exogenous noxae - becomes ill when the necessary age is reached. The nature of a determining factor which is most probably present is not known. Comparison with international figure shows that the results of the Hamburg Study can be considered a model case.

We investigated cancer risk among mothers of 2365 children who were diagnosed with cancer between 1973 and 1989 in Sweden. From the date of birth of the child until 31 December 1989, 38 cases of cancer were diagnosed among the mothers. The expected number of cases, according to national rates, was 30.9. Cancer of the thyroid was the only site showing a significantly increased risk among the mothers (observed = 6, expected = 1.2, P < 0.01), but two cases were medullary cancer associated with an inheritable syndrome, and inherited by their children.

Two nuclear families showing characteristics of the SBLA syndrome are described wherein progeny of breast cancer-affected mothers manifested early childhood malignant neoplasms. These observations have led us to postulate a novel type genetic-environmental interactive model which incorporates Knudsen's "two-hit" hypothesis as a partial explanation for the exceedingly early onset of cancer in the subject progeny. Given the assumption that the first hit was germinal with transfer of the deleterious SBLA gene at conception, we postulate that the second or somatic-hit occurred early on in utero. This may have involved a complex mechanism of one or more factors including tumor cell products, tumor specific antigens, immunosuppression, de-repressed oncogene, or an activated oncogenic virus via a transplacental communicable phenomenon. The testing of this new hypothesis dealing with carcinogenesis in the SBLA syndrome should employ immunologicalgenetic parameters concurrently in fetuses and mothers.

In the period from 1961 to August, 1975, 2,222 microlaryngoscopic examinations were carried out at the ENT Department of the University. A correlation was seen between the increase in the number of examination in the last few years and the increase in the number of malignant tumours of the larynx, while malignant growths in the hypopharynx were found to be only slightly increased. An anlysis of 1,027 microlaryngoscopies carried out under anaesthesia over the last 2 2/3 years showed almost 50% cancers and precancers. In 5% of the case at most more rare laryngeal findings were also seen. Of these, the granular cell tumour, plasmocytoma, oncocytic cystadenoma and amyloid-tumour of the larynx, but also tuberculous laryngitis, are represented. The modern endoscopic examinations and the fundamental bioptic histomorphological examination of proliferations allow an exact differentiation of these changes in the tissues of the endolarynx. In this connection, a valuable technical aid is the v. Stuckard low-power magnifying laryngoscope, that has been in use for the past six months.

The incidence of extrahepatic gastrointestinal metastases from breast cancer is reported in the literature only as necroscopy studies (6-18%); they usually originate from lobular or a mixed ductal-lobular subtype. Nonspecific presenting symptoms, death of the patients caused by other more frequent metastases, and variable radiographic features mimicking primary neoplasms cause a clinical underestimation of this pathology. We report here a case of rectal metastasis from an invasive ductal carcinoma (IDC). This is to our knowledge, the first recorded instance of an anal metastasis from IDC.

Combined-modality therapy consisting of radiation therapy and 5-fluorouracil plus mitomycin remains the standard treatment for localized anal canal cancer, permitting preservation of organ function and achieving high response and survival rates. Current trials are evaluating the ability to deliver higher doses of radiation therapy over shorter intervals. Currently, the most common indication for radical surgery is failure of chemoradiation. Metastatic disease is less responsive to chemoradiation treatment. Neoadjuvant and adjuvant strategies are being evaluated, and additional drugs including capecitabine, irinotecan, and oxaliplatin are being assessed in treatment of localized and advanced disease.

Circulating carcinoembryonic antigen (CEA) levels and their relation to liver function test values were studied in 29 jaundiced patients with benign extrahepatic biliary tract obstruction and inflammation. During the obstructive and inflammatory phase, 15 (52%) of the patients had CEA levels greater than 2.5 ng/ml. Elevated CEA levels were associated more frequently with common bile duct stones (and cholangitis) than with gallbladder stones (and cholecystitis) alone, although this difference was not statistically significant. The former often had values greater than 5.0 ng/ml. The highest values were found in two patients with liver abscesses. T'HE CEA levels returned to normal following relief of obstruction in seven of ten patients and increased in two patients who had progressive inflammation. Serum alkaline phosphatase and bilirubin levels were significantly higher in the patients with elevated CEA levels (p smaller than .05). Serum alkaline phosphatase levels showed a significant positive correlation with CEA levels (p smaller than .02). Patients with obstructive jaundice and elevated CEA levels do not necessarily have cancer.

A 57 years old man was operated for aortic valve replacement. During the operation a mediastinal lymph node was biopsed with the result of undifferenciated large cell carcinoma with neuroendocrine features. Three months after surgery the patient died of non-cardiac related reasons, postmorten examination was not performed. Although the primary tumor could not be assessed, probably the lung was the original location. The histological classification of lung cancer is exposed, with special reference in the undifferenciated large cell type with neuroendocrine features.
