Although intestinal metastases from lung cancer are not rare at postmortem studies, the development of clinically significant symptoms from the gastrointestinal metastases is very unusual. We report a case of small intestinal hemorrhage leading to intestinal perforation secondary to metastases from a large cell carcinoma of the lung in a 31-year-old man along with a review of the literature.

204 pts with breast cancer were compared to 208 pts of similar age, with pathologies other than cancer, to assess the frequency of biological and social factors related to breast cancer, in Chilean females. The pts with breast cancer were in the 40 to 65 year old group, had a higher frequency of AB blood group, lower pregnancy and delivery rates and later pregnancies. Age of menarche and menopause, prolonged breast feeding, oral contraceptives and nutritional state were not related to a higher frequency of breast cancer. Further studies that include the analysis of previous breast pathology, diabetes, psychosomatic factors, alcohol and caffeine consumption, tobacco and others are suggested.

A case of prostate cancer presenting with malignant ascites as the sole clinical manifestation is reported. The clinical course simulated occult gastrointestinal malignancy. At the histologic level, the tumor had a signet-ring morphology. Detailed immunocytochemical and ultrastructural studies, however, confirmed the prostatic origin of this neoplasm. Signet-ring carcinoma of the prostate is a rare aggressive form of prostatic cancer which may present with a malignant effusion and may mimic gastrointestinal cancer.

Our unit has not investigated quality of life measurements specifically, and therefore I would like to make some general comments on the presentations by Professor Selby and Dr. Lindley. There are a number of influencing factors which affect the quality of life of cancer patients. An effective anti-emetic reduces nausea and vomiting and this could have an impact on patient well-being during the period of cytotoxic treatment. The question is, 'Do new, more effective anti-emetics improve the overall quality of life of cancer patients?' In order to show this we need to define very carefully the patient population under consideration.

From 1984 to 1990 38 patients had an operation for carcinoma of the gallbladder and bile ducts. The unfavorable prognosis of this type of cancer was improved by a combined treatment, consisting of surgical tumor reduction and implantation of a transhepatic drainage, followed by external radiation and intracatheter brachytherapy. Nine patients were treated in this manner, seven died 6-67 months after operation, two are still alive 7 and 13 months postoperatively.

The lower subregion of the south of Thailand has an overall higher incidence and mortality associated with oesophageal cancer than does the upper subregion. The increased incidence in the lower south is more marked among females than among males, but the male/female incidence ratio in the lower subregion is still high (3.7:1). There is evidence also of an increased incidence in the eastern provinces compared with that in western provinces. Available data on occupation and district of residence provide no clues to the cause of the distinct geographic pattern of incidence within the southern region.

The role of ultrasound in the diagnosis of prostate cancer is still evolving. We used ultrasound to examine 202 patients with palpably abnormal prostates. Of 151 patients undergoing biopsy, ultrasound-guided biopsy was comparable to digitally directed biopsies in cancer detection (92% vs 90.7%). The two methods are complementary and should both be considered in patients with palpably abnormal prostates.

Breast cancer is a serious disease in terms of incidence and mortality for older women. Older women are likely to receive substantial benefit from mammography because of their increased risk for the disease. However, most women older than 65 years are not getting regular mammograms. There are a number of barriers to breast screening for older women, but perhaps none is more important than the lack of a physician recommendation. Tailored programs are needed if the promise of breast cancer screening is to be realized for older women. Such programs should reflect the heterogeneity of older women. In addition, physicians must be collaborators because of their central role influence.

A retrospective study of one decade of rib biopsy in four hospitals in Nashville, Tenn, showed 61 biopsies were done in 60 patients. The typical patient was a male in his seventh decade. Preferred operative technique was open biopsy with general anesthesia. One half of the patients had metastatic malignancy; most of the known primary tumors were lung cancer. About one fifth of specimens were normal ribs. Biopsy was done in nine of these because of false-positive scintigraphy. Accurate preoperative chest wall localization is critical in order to minimize intraoperative decision-making problems. Yield of rib biopsy should be increased by more careful clinical observation, including critical evaluation of bone scans, avoiding overinterpretation of physical findings and observing for healing of possible rib fractures.
