TITLE: Primary sclerosing cholangitis associated with liver atrophy.

Of 27 patients with primary sclerosing cholangitis, 4 were found on investigation and subsequently at operation to have gross lobar atrophy. The disease was particularly severe in the hilar region. Three of the four patients were asymptomatic prior to the onset of jaundice. The presence of atrophy precipitated laparotomy to exclude cancer in two cases. Marked disparity in size between liver lobes precluded a hilar approach to relieve extrahepatic obstruction in two patients. Prolonged follow-up excluded coincident malignant disease. Selective liver atrophy should be considered part of the disease spectrum in primary sclerosing cholangitis.

Three hundred fifteen cases of adenocarcinoma were analyzed for differences between lesions in the right colon, left colon, and rectum for the period 1978 to 1983. The right colon cancers accounted for 1/3 of the cases and were associated with a higher incidence of anemia. The incidence of obstruction was the same between the right and left colon but lower for the rectal lesions. Left colon and rectal cancers had a higher incidence of gross bleeding than right colon cancers, but all had a high rate of occult blood in the stool. The Duke stage of the lesions, which was similar throughout the colon and rectum, is a reliable prognostic indicator.

The area of serosal cancerous invasion and intraperitoneal free cancer cells were examined in 360 gastric cancer cases. The positivity rate of intraperitoneal free cancer cells was 20% in the group with under 20 cm2 of serosal invasion, 59% with 20 cm2 to 30 cm2, and 76% over 30 cm. The postoperative survival rate was significantly lower in the group with free cancer cells than in the group without them. Prognosis was better in case with the area of serosal invasion was smaller. Hematogenous metastasis was frequent under 20 cm2, but over 20 cm2, peritoneal metastasis predominated.

TITLE: Thickness as prognostic aid in upper aerodigestive tract cancer.

A retrospective review of records and microscopic slides was carried out on 151 patients, encompassing nine specific upper aerodigestive tract cancer sites, correlating depth of invasion with node metastases and outcome. In this preliminary descriptive study, thickness was found to be more closely related to node metastasis and to survival than was surface diameter in middle-stage tumors, particularly in tongue, floor of the mouth, buccal mucosa, gum, and soft palate sites. Thickness, depth of penetration, and bone involvement appear to represent the degree of general tumor aggressiveness better than does surface extent. A substitution of thickness measurement for surface diameter (T) in the present TNM staging system is suggested.

Some of the breast cancer chemotherapy trials conducted during the past 25 years were reviewed. In general, most authors noted a significantly improved disease-free survival and overall survival in premenopausal patients but not in postmenopausal patients. Considering all patients, most authors found a significant increase in the disease-free survival but not in overall survival. In one study, the relapse rate increased with time. In the first three years, the relapse rate in the control group greatly exceeded that in the treatment group, but as the years went on, this trend reversed, and an increasing percentage of the treated group had relapse. More time is required to draw firm conclusions.

The study originates from the arousal of suspicions that, in connection with loading and unloading work involving the use of trucks, exposure to diesel exhaust fumes may be carcinogenic. This report describes a register study in which a broad mapping of the cause-of-death pattern and cancer morbidity has been carried out through the matching of locally compiled personal data with those from a central register, the Cause-of-Death Register, and the Cancer Register. The results obtained thus far show a low total mortality--irrespective of cause of death--but a tendency towards an excess risk for cancer morbidity, mainly due to lung cancer.

Eight cases of primary Non-Hodgkin's lymphoma (NHL) of the breast are reported. Almost all presented as a painless movable mass without nipple retraction, edema in the overlying skin, satellite skin nodules or bloody nipple discharge. Most of the cases belonged to B cell lymphoma. Wide local excision or simple mastectomy combined with chemotherapy or radiotherapy were adopted in the treatment of these patients. The average survival was 28 months. Since prognosis of NHL is worse than that of breast carcinoma and is easily misdiagnosed as breast cancer, it is important to take vigilance in the differential diagnosis.

A case-control study was conducted in Utah between 1984 and 1987 to evaluate the effects of nutrient intake on risk of developing ovarian cancer. Detailed dietary intake information was available from 85 first primary ovarian cancer cases and 492 population-based controls. Calories, fat, protein, fiber, and vitamins A and C did not appreciably alter the risk of developing ovarian cancer. However, high intake of beta-carotene appears to confer protection against ovarian cancer (odds ratio = 0.5, 95% confidence interval 0.3-1.0) after adjusting for age, number of pregnancies, and the body mass index of weight/height.

There is virtually no screening for endometrial cancer after the menopause. The methods so-far available (cervico-vaginal smear or endouterine specimens) are either unreliable or else difficult to perform. The progestogen tests are, in contrast, easy for any doctor to carry out, acceptable to patients, fairly reliable and low cost, which should make their widespread use possible. The administration of a progestogen to a post-menopausal woman should not produce bleeding unless the endometrium is hyperplasic or proliferative. 1 366 tests of this type were carried out and resulted, amongst other discoveries, in the detection of 5 cases of asymptomatic cancers of the endometrium and 2 cases of ovarian cancer.

TITLE: [A case of resected metastatic adrenal carcinoma from gastric cancer]

A case of a metastatic adrenal carcinoma is presented. A 67-year-old female underwent total gastrectomy for an adenocarcinoma of the stomach before 52 months. A mass was indicated at the left supra-renal region by computed tomography. The mass was successfully resected by an upper abdominal para-aortic lymph node dissection. Histologically, the mass was confirmed as a metastatic adrenal carcinoma with a lymph node metastasis from the gastric cancer. Metastatic adrenal carcinomas and local lymph node dissections for treating metastatic carcinomas are discussed.

TITLE: Failure of chemotherapy to prolong life in patients with metastatic prostate cancer who have failed androgen deprivation.

Using historical controls, we analyzed the effect of chemotherapy on survival of patients with metastatic prostate cancer who failed androgen deprivation. The survival of 178 patients with metastatic prostate cancer who receive palliation only after failing hormonal therapy was compared with the survival of 27 patients who received chemotherapy on NPCP Protocol 1500. Survival was measured from the initiation of androgen deprivation. The mean survival after chemotherapy was 8.4 months which was slightly less than the group receiving palliation only. At this time, chemotherapy has not been able to improve survival in patients with metastatic prostate cancer.

Serum HSV-2 antibody in male from high and low incidence areas of cervical cancer was assayed by enzyme-immuno-slide method. The results show that the geometric mean titer of the serum HSV-2 antibody in the male from high incidence area (413.5) is significantly higher than that from the low incidence area (210.4) (P less than 0.01). It indicates that the high cancer incidence might be related to the high HSV-2 infection rate in the spouses of the cervical cancer patients.

The effect of PPM therapy, consisting of cisplatin, peplomycin and mitomycin C, was evaluated in 15 cases of squamous cell lung cancer. Ten partial responses were achieved in primary cases and the response rate was 66.7%. Nephrotoxicity was well controlled with a continuous infused drip and FOM. The nausea and vomiting were reasonably well controlled with methylprednisolone and metoclopramide. Severe interstitial pneumonitis occurred in 5 cases (33.3%). PPM therapy is considered to be a very useful combination chemotherapy for squamous cell lung cancer but careful attention must be paid to pulmonary toxicity.

Gastric juice and urine samples from consecutive patients who underwent endoscopy for upper GI tract complaints were examined for the presence of mutagens. Patients endoscopically and histologically diagnosed as having either chronic atrophic gastritis (CAG) or gastric cancer (GC) had higher than normal levels of mutagens in their gastric juice and urine. The gastric juice pH of these patients was also elevated and, in the case of the CAG patients, contained detectable levels of nitrites. No correlation was however found between gastric mutagen levels and urinary mutagen excretion in the individuals examined.
