Although intestinal metastases from lung cancer are not rare at postmortem studies, the development of clinically significant symptoms from the gastrointestinal metastases is very unusual. We report a case of small intestinal hemorrhage leading to intestinal perforation secondary to metastases from a large cell carcinoma of the lung in a 31-year-old man along with a review of the literature.

TITLE: A mathematical model of flow through the terminal lymphatics.

Proper understanding of the mechanisms of fluid absorption and flow through the terminal lymphatics is essential for the control of several pathological conditions such as edema, bedsores and cancer. A mathematical model of the terminal lymphatics was developed using the principles of mechanics. Computer simulation results substantiate the hypothesis that fluid absorption and flow through the terminal lymphatics occur due to suction mechanisms of the adjacent contractile lymphatic segments and due to periodic fluctuations in the interstitial fluid pressure. In addition, the results suggested that increasing the length of a terminal lymphatic vessel beyond a certain limit does not cause further increase in fluid flow into the terminal lymphatic.

Current therapy for primary brain tumors, metastatic brain tumors and leptomeningeal carcinomatosus is inadequate. Monoclonal antibodies and certain biological ligands such as transferrin and epidermal growth factor bind selectively to tumors but in and of themselves have little therapeutic activity. Linking these binding moieties to protein toxins yields new molecules, called immunotoxins, that display enormous cell-type specific toxicity in vitro and could be valuable agents for treatment of CNS cancer. This review discusses our development of immunotoxins to treat brain tumors after systemic, intrathecal and intratumoral injection.

204 pts with breast cancer were compared to 208 pts of similar age, with pathologies other than cancer, to assess the frequency of biological and social factors related to breast cancer, in Chilean females. The pts with breast cancer were in the 40 to 65 year old group, had a higher frequency of AB blood group, lower pregnancy and delivery rates and later pregnancies. Age of menarche and menopause, prolonged breast feeding, oral contraceptives and nutritional state were not related to a higher frequency of breast cancer. Further studies that include the analysis of previous breast pathology, diabetes, psychosomatic factors, alcohol and caffeine consumption, tobacco and others are suggested.

A case of prostate cancer presenting with malignant ascites as the sole clinical manifestation is reported. The clinical course simulated occult gastrointestinal malignancy. At the histologic level, the tumor had a signet-ring morphology. Detailed immunocytochemical and ultrastructural studies, however, confirmed the prostatic origin of this neoplasm. Signet-ring carcinoma of the prostate is a rare aggressive form of prostatic cancer which may present with a malignant effusion and may mimic gastrointestinal cancer.

About 10% of patients with newly diagnosed colorectal cancer will have locally advanced disease without gross evidence of distant or discontiguous intraabdominal metastases. Long-term cure or disease-free survival frequently follows extended resection for a locally advanced primary colorectal carcinoma. The use of adjuvant combined chemo-radiation therapy is still being evaluated. While complete resection of recurrent disease sometimes provides the best long-term survival when compared with other treatment modalities, few patients are suitable candidates for curative resection. In our experience, major extended resection for recurrent pelvic disease is largely palliative and is rarely associated with long-term survival.

Somatic mutations of von Hippel Lindau (VHL) tumor-suppressor gene have been identified in kidney cancers from North America and Japan. We studied VHL gene mutation in 31 kidney tumors from France. Of these tumors, 45% (14/31) displayed mutations, 60% of which occurred at A:T base pairs. The frequency and the spectrum of mutations identified in kidney tumors from Europe were similar to those reported for tumors from other geographical locations.

The Papanicolaou smear has been a remarkably effective tool in cancer prevention, but it is not a perfect test. Although the most important factor in failure of cervical cancer prevention is lack of adequate screening, other factors include problems with sampling, interpretation, and effective clinical follow-up. A small number of rapidly developing cervical cancers probably also arise in the interval between Papanicolaou smear screenings. Consequently, cervical cancer will develop in some women despite appropriate screening. This article will analyze some of the problems relating to diagnostic errors, which include abnormal cells that are few (< 100), small, or bland.

It is well documented that proteoglycans are involved in a wide range of pathological conditions. Recently published results in international journals provide new information on the role of proteoglycans in such conditions. A mutation in the gene encoding for a cell surface proteoglycan has been demonstrated in overgrowth syndromes. A proteoglycan has been isolated from urine and shown to induce cachexia in cancer patients. Furthermore, in both achondrogenesis and colon cancer, the reduced ability to sulphate proteoglycans is due to genetic defects in cellular sulfate transporters. Finally, fibrosis has been inhibited in glomerulonephritic mice by transferring the gene for decorin, a transforming growth factor beta-1 binding proteoglycan, into muscle tissue.

Pain is the most common symptom in patients suffering from cancer, in particular for advanced diseases. Unfortunately, one third of the patient do not benefit from optimal pain control. One must stress the importance of defining the etiology of pain and subsequently its pathophysiology. This thorough evaluation can help to formulate a proper strategy for a treatment of pain that could be adapted to the disease evolution. A multidisciplinary approach with respect to the WHO prescribing criteria is the most appropriate way to control severe cancer pain syndromes.
