TITLE: Primary sclerosing cholangitis associated with liver atrophy.

Of 27 patients with primary sclerosing cholangitis, 4 were found on investigation and subsequently at operation to have gross lobar atrophy. The disease was particularly severe in the hilar region. Three of the four patients were asymptomatic prior to the onset of jaundice. The presence of atrophy precipitated laparotomy to exclude cancer in two cases. Marked disparity in size between liver lobes precluded a hilar approach to relieve extrahepatic obstruction in two patients. Prolonged follow-up excluded coincident malignant disease. Selective liver atrophy should be considered part of the disease spectrum in primary sclerosing cholangitis.

Three hundred fifteen cases of adenocarcinoma were analyzed for differences between lesions in the right colon, left colon, and rectum for the period 1978 to 1983. The right colon cancers accounted for 1/3 of the cases and were associated with a higher incidence of anemia. The incidence of obstruction was the same between the right and left colon but lower for the rectal lesions. Left colon and rectal cancers had a higher incidence of gross bleeding than right colon cancers, but all had a high rate of occult blood in the stool. The Duke stage of the lesions, which was similar throughout the colon and rectum, is a reliable prognostic indicator.

TITLE: [Problems and present status of surgical treatment for early gastric cancer]

We investigated 81 cases resected in our department about 10 years. The following conclusions were obtained. The R2-operation in lymph node dissection should be made even for early gastric cancer. Early gastric cancer with the unclear border, uneven surface or induration should be operated as advanced cancers. Because the positive rate of lymphatic permeation in these cases is high and the hematogenous metastasis is not rarely found. We also sent out a questionnaire to 430 hospitals which were attached to the Japanese Research Society for gastric cancer. We do believed that we should not easily performed the non operative treatment for early gastric cancer, because we could be only obtained the exact information after the histological examination of resected specimens.

A retrospective review of laryngeal cancer in Auckland over the period 1965-1979 has revealed a generally poorer prognosis for supraglottic cancer than for glottic cancer. The use of radiotherapy for supraglottic cancer and for advanced glottic cancer has not been as successful as primary surgery but has yielded good results in early glottic carcinoma. Salvage surgery after failed radiotherapy was significantly more successful for glottic tumours than supraglottic. Stomal recurrence was more likely if a patient had required an emergency tracheotomy for airway obstruction.

TITLE: Multivariate analysis of prognostic factors for operable rectal cancer.

Previous reports have established that several variables are of prognostic importance in rectal cancer. The information from a Medical Research Council trial in seventeen centres in the United Kingdom was used to assess the simultaneous contribution of several of these factors in a multivariate analysis. In the preoperative assessment mobility of the tumour and the number of quadrants involved were predictive of the likelihood of a curative resection being possible. Among patients who had curative resections, prognosis was influenced by Dukes' classification, height of tumour, histological grade, and venous invasion. A scoring system based on these criteria was constructed and applied to this group of patients. It successfully defined five prognostic groups with 5-year survival rates ranging from 80% to 19%.

The area of serosal cancerous invasion and intraperitoneal free cancer cells were examined in 360 gastric cancer cases. The positivity rate of intraperitoneal free cancer cells was 20% in the group with under 20 cm2 of serosal invasion, 59% with 20 cm2 to 30 cm2, and 76% over 30 cm. The postoperative survival rate was significantly lower in the group with free cancer cells than in the group without them. Prognosis was better in case with the area of serosal invasion was smaller. Hematogenous metastasis was frequent under 20 cm2, but over 20 cm2, peritoneal metastasis predominated.

Since the introduction of medical lasers in 1960, the skin has been a major site for applied laser research and therapy. Argon and carbon dioxide lasers have been the most widely used in clinical practice, and many diseases that were impossible or difficult to manage are now being effectively treated with these. Exciting developments today involve other systems, including tunable dye, ruby, neodymium-YAG, and low-energy lasers. Photodynamic therapy for cancer may provide a selective method of destroying clinically inapparent malignant cells. Lasers now seem to have become an integral part of the dermatologic surgeon's armamentarium.

A retrospective review of records and microscopic slides was carried out on 151 patients, encompassing nine specific upper aerodigestive tract cancer sites, correlating depth of invasion with node metastases and outcome. In this preliminary descriptive study, thickness was found to be more closely related to node metastasis and to survival than was surface diameter in middle-stage tumors, particularly in tongue, floor of the mouth, buccal mucosa, gum, and soft palate sites. Thickness, depth of penetration, and bone involvement appear to represent the degree of general tumor aggressiveness better than does surface extent. A substitution of thickness measurement for surface diameter (T) in the present TNM staging system is suggested.

Some of the breast cancer chemotherapy trials conducted during the past 25 years were reviewed. In general, most authors noted a significantly improved disease-free survival and overall survival in premenopausal patients but not in postmenopausal patients. Considering all patients, most authors found a significant increase in the disease-free survival but not in overall survival. In one study, the relapse rate increased with time. In the first three years, the relapse rate in the control group greatly exceeded that in the treatment group, but as the years went on, this trend reversed, and an increasing percentage of the treated group had relapse. More time is required to draw firm conclusions.

Synthetic development of derivatives and analogs which have stronger activities and lower characteristic toxicities than parent antitumor antibiotics such as daunomycin, adriamycin, etc., have been illustrated namely: (1) syntheses of 4'-O-derivatives such as 4'-O-tetrahydropyranyl-adriamycin; (2) syntheses of N-alkyl derivatives such as 3'-deamino-3'-morpholino-adriamycin; and (3) syntheses of 11-deoxy-adriamycin. Notably, 4'-O-tetrahydropyranyl-adriamycin having a (2" R)-configuration has been confirmed to have stronger activity and lower toxicity than adriamycin by Phase II study. New, naturally occurring anthracycline antibiotics (decilorubicin, arugomycin, ditrisarubicin, etc.) have also been reviewed.

Cisplatin binds in vitro with vitamin B12 coenzymes at physiologic pH and temperature. We have hypothesized that cisplatin-induced neurotoxicity is due to an in vivo inactivation of the cobalamin coenzymes. Vitamin B12 metabolism was studied in patients treated with cisplatin for advanced cancer. Serum levels of B12 and B12-binding capacity remained normal during cisplatin therapy. Excessive urinary excretion of methylmalonic acid and homocystine, which are both metabolic correlates of cellular cobalamin deficiency, was not demonstrated after cisplatin administration. Thus, we find no evidence to implicate cellular B12 deficiency as a mechanism of cisplatin neurotoxicity.

TITLE: Mortality and cancer incidence among Swedish dock workers--a retrospective cohort study.

The study originates from the arousal of suspicions that, in connection with loading and unloading work involving the use of trucks, exposure to diesel exhaust fumes may be carcinogenic. This report describes a register study in which a broad mapping of the cause-of-death pattern and cancer morbidity has been carried out through the matching of locally compiled personal data with those from a central register, the Cause-of-Death Register, and the Cancer Register. The results obtained thus far show a low total mortality--irrespective of cause of death--but a tendency towards an excess risk for cancer morbidity, mainly due to lung cancer.
