TITLE: Lymphangitic spread of breast carcinoma: scintigraphic pattern with chest x-ray and computed tomography correlation.

Lymphangitic spread of metastases has distinct patterns on pulmonary scintigraphy and radiographic imaging. A 43-year-old woman with metastatic breast cancer presented with shortness of breath. Symptomatology was studied with ventilation and perfusion (V/Q) scintigraphy, chest radiograph, and computed tomography (CT). The scintigraphic pattern is consistent with prior descriptions of lymphangitic spread of tumor. The chest x-ray and CT were both performed within the prior 2 months and were confirmatory.

Yellow discoloration of the osteoid seams of the partly demineralised trabecular bone in the site of the osteolytic cancer metastasis was observed. Intralesional hemorrhage with massive breakdown of the red cells and local release of bilirubin caused characteristic colour changes of unmineralised osteoid. Mineralised osteoid had unchanged colour. No morphological changes of bone cells were recognised. Yellow osteoid developed as a result of local high bilirubin concentration in the neighbourhood of bone trabecullae with unmineralised osteoid seams.

YM-216391, a novel cyclic peptide, was isolated from the cultured mycelium of Streptomyces nobilis JCM 4274. It was purified by solvent extraction, silica gel and ODS flash column chromatographies, followed by preparative HPLC. YM-216391 dose-dependently inhibited the growth of human cervical cancer HeLa S3 cells with an IC50 value of 14nM. YM-216391 also showed potent cytotoxic activity against a human cancer cell line panel.

As risk factors of thrombosis except for above, abnormal protein C, abnormal protein S and abnormal antithrombin are often observed. Caucasians have frequently FV Leiden or abnormal FII as congenital thrombophilia, though those are not observed to Japanese. As an acquired risk factor for PE, there are cancer, high homocysteinemia, lipid abnormality and anti-phospholipid antibody syndrome. It is necessary to evaluate these risk factors for the prevention of PE.

Recent studies demonstrate a progressive decline in mitochondrial function during aging. Studies have established that the decline in mitochondrial function is due to the accumulation of mutations in mitochondrial DNA. These observations suggest that the mitochondrial dysfunction that accompanies aging may exert a major influence on carcinogenesis.

Two large recent studies indicate an increased risk of breast cancer with use of menopausal hormone therapy (HT), particularly combinations of estrogen and progestogen. These studies also go against previous theories of the types of breast cancer that would be associated with HT use. Emerging information regarding the influence of estrogen plus progestin on breast cancer risk adds further weight to the recommendation against use for chronic disease risk reduction and raises additional questions regarding risks of even short-term use for vasomotor symptoms associated with menopause.

The effects of fractions from hot aqueous extract, acetone extract and the folklore preparation of Clausena excavata were studied on mouse splenocyte proliferation. The fractions of hot aqueous and acetone extracts were found to be the most active. On the contrary, the fractions from the crude folklore preparation resulted less active. This result could partly explain the popularity of this plant in folk medicine as a remedy for cancer and HIV patients in the eastern part of Thailand.

Metastases, rather than primary tumours, are responsible for most cancer deaths. To prevent these deaths, improved ways to treat metastatic disease are needed. Blood flow and other mechanical factors influence the delivery of cancer cells to specific organs, whereas molecular interactions between the cancer cells and the new organ influence the probability that the cells will grow there. Inhibition of the growth of metastases in secondary sites offers a promising approach for cancer therapy.

We describe an unusual case of acute myocardial infarction (AMI) and cardiogenic shock in a 25-year-old woman. After coronary revascularization, a thyrotoxic storm developed and the patient's cardiac failure changed into a hyperdynamic circulation without obvious changes in the patient's vital signs. This report will focus on three issues: (1) the benefit of advanced hemodynamic monitoring in circulatory unstable patients, (2) the confounding effects of untreated hyperthyroidism on the circulation, and (3) previous cancer treatment as a cause of AMI in young persons.

Basal cell carcinoma arising from the nipple of the breast is an uncommon malignancy, with only a handful of cases reported in the literature. The diagnosis is, however, important because basal cell carcinoma in this location can mimic breast cancer, may behave aggressively, and there is a relative lack of consensus in its treatment. A case of basal cell carcinoma of the nipple is described in a 67-year-old man with a brief review of the literature.
