TITLE: [Management of vomiting induced by polychemotherapy in Hodgkin's disease]

Sixty patients with Hodgkin's disease were treated with a polychemotherapy regimen including very emetic dacarbazine and adriamycin in addition to bleomycin and vincristine on 622 treatment days during the last 10 years. Nausea in 97% and severe vomiting in 91% were observed during the treatment causing considerable distress to patients which led to patient noncompliance in 26.6%. Among the used antiemetic agents haloperidol was less active than high-dose metoclopramide alone or with high-dose methylprednisolone which were extremely effective. The control and prevention of drug-induced emesis is a major problem for cancer patients and their oncologists. The present situation and the home tasks are also discussed on the basis of literature and their observations.

UFT at a dose of 600 mg was given per os every day for more than four weeks. Among 12 evaluable patients, one patient with adenocarcinoma of the lung showed partial response. The response rate for UFT was 8.3%. Toxic effects included anorexia (31%), nausea (15%), liver disorder (15%), and pigmentation (8%).

Previous studies have suggested that certain occupational groups may be at increased risk for prostate cancer. A cancer registry-based case-control study was conducted to investigate possible associations between various occupations and the risk of prostate cancer among 1,239 case subjects and 3,717 age-matched control subjects. Elevated relative risk estimates were observed for farmers, mechanics, sheet metal workers, separating machine operators, and for men employed in several manufacturing industries. These findings are discussed in the context of previous studies, and suggestions for future research are provided.

Biological response modifiers (BRMs) strengthen the host defense mechanism/subsequently producing therapeutic effects against neoplastic and other diseases. Many kinds of BRM are applied for malignant skin tumors. Administration is both systemic and intratumoral, evaluable lesions can be measured in one, two and three directions. Furthermore there are also methods for judging the effects by clinical symptoms. These involve evaluation of macroscopic effect, from which it is possible histopathological judgement. Therefore, the evaluation of effects on skin cancer is very complex, and a new method is necessary. Accordingly we devised a method of judgement summarizing these evaluation procedures.

Incidence of telangiectasia and ulcer among 118 postoperated breast cancer patients, who were tangentially irradiated at Department of Radiology, Kyushu University from 1961 to 1980, followed more than 5 years were evaluated. Telangiectasia occurred in 5% at 20-35 Gy, in 50% at 50-55 Gy. Ulcer occurred in 5% at 50-55 Gy, in 50% at 70-75 Gy. Skin tolerance dose in Rubin's Table (1972) is correct.

The authors worked out methods of topometric investigation of cervical cancer patients during preparation for intracavitary irradiation using 252Cf fast neutron sources. The use of balloon catheters made it possible to determine a coefficient of projection enlargement in the point of interest, to determine accurately the distance between endostats, sources and contrasted organs of interest: the bladder and rectum. The results of the studies were used for individual design of intracavitary treatment as well as for the determination of radiation exposure of the pelvic organs in 38 patients. The patients received a total of 138 investigations.

A radioimmunoassay for the detection of human immunoglobulin response is described which is suitable for the screening of patients undergoing immunotherapy with murine monoclonal antibodies. The use and the sensitivity of the assay are illustrated in the case of patients undergoing diagnostic imaging for ovarian cancer with the monoclonal antibody, OC125. The method offers the distinct advantage of quantitating human anti-murine antibody in serum containing a high concentration of antigen recognized by the injected murine antibody. In addition, the assay may be generalized to screen patients undergoing immunotherapy with a variety of murine or other monoclonal antibodies.

Results of reoperations for recurrent cancer of the large bowel in 164 patients are analyzed. Radical operations were performed in 110 patients (67.0%). The main operations used were resections of the malignized segment of the bowel (48 patients) and abdomino-perineal extirpation of the bowel with the anastomosed portion of the colon or its stump after the Hartmann operation (38 patients). Postoperative complications were noted in 27.6% of the patients. Lethality after radical operations was 10%. The follow-up analysis has shown that of all the discharged patients 40.7% survived more than 3 years, 31%--more than 5 years.

Palladium salt has been used for some time in experimental therapy protocols; with this in mind, we carried out a study of the effect of tetramine dichloro-palladium (soluble salt) upon kidney cells. Using an electron microprobe, we were able to detect the presence of palladium associated with sulfur and iron in the lysosomes of the proximal tubule cells. Our results were compared with those obtained using Cis-diaminedichloro-platinum (Cis-DDP), an anti-cancer drug used in the treatment of diverse tumors. The mechanism of intralysosomal concentration of palladium as a non soluble salt associated with sulfur appeared to be related to local sulfatase activity. Finally, iron concentration appeared to be related to the inhibition process of erythropoiesis.

The degree of hormone-dependence of endometrial cancers may be evaluated by the presence and the rate of estradiol and mostly progesterone receptors in an endometrial fragment. The most differentiated tumors have a high receptor content, while anaplastic cancers are most of the time without them. They must be systematically determined regardless of other prognostic factors enabling to evaluate the prognosis and guide the treatment. It is presently facilitated by the method of immunochemical titration on histological section, available for estrogen receptors, disclosing the spatial and functional distribution of hormono-dependent cells.
