PUBDATE: 19850701
Surgery has been considered to be a main treatment of urogenital cancer. However, surgery alone could not completely cure these patients. The prognosis was poor. Multi-disciplinary treatment has been adopted to improve these results. However, there are many problems to be solved, such as combination of these regimens. In addition, the criteria for evaluation of the therapy should be considered. Nevertheless, multi-disciplinary treatment has been one of the most potent procedures for these malignancies.

Since 1968, we have carried out cryoprostatectomy on prostatic hypertrophy and prostatic cancer patients. As for the method of monitoring during cryoprostatectomy, digital examination only was carried out at the beginning in order to confirm the range of freezing and to determine the freezing time. In 1972, we developed a cryoprostatectomy method under direct intravesicular observation using the Trocar Cystoscope and furthermore in 1980, we developed echoguided cryoprostatectomy using the transrectal linear electronic scanner. These new developments ensure the safety of cryoprostatectomy and furthermore, cryoprostatectomy can now be easily carried out. The above-mentioned procedures are described.

PUBDATE: 19820901
Two cases of systemic lupus erythematosus in association with primary gastric adenocarcinoma are presented. A pathological analysis of the two cases of gastric cancer does not support the hypothesis that lupus and its associated vasculitis is involved in the pathogenesis of gastric carcinoma. A critical review of the literature reveals numerous case reports suggesting the association of systemic lupus erythematosus with various malignancies; however, there is no definitive study which substantiates the association of gastric or other cancers with lupus. Future studies will be required to assess appropriately the possibility of an association between lupus and cancer.

TITLE: The postoperative classification for uterine cervical cancer and its clinical evaluation.
PUBDATE: 19831001
A postoperative classification for uterine cervical cancer has been made in consideration of the spatial spreading of cancer, biological malignancy of 120 cases which were treated with radical hysterectomy and pelvic lymphadnectomy. This classification corresponds extremely well to prognosis. The 5-year survival of the cases with prognostic index (P.I.) 9 or less was 96.1%, while those with P.I. above 10 showed 31.8%. In the Shinshu University School of Medicine clinic, this classification has become indispensable for decision of postoperative irradiation, selection of irradiation methods, and chemotherapeutic agents. Using this classification for individualized therapy, the survival rate was elevated in advanced cancer, class III or IV, with lymph node metastasis and P.I. above 10.

A case of early esophageal cancer, 2.5 cm in length, located just above the esophagogastric junction is presented. This cancer seemed to originate from the fundic gland of the esophagus, because it consisted of the submucosal invasion of well-differentiated adenocarcinoma which was almost completely covered with the squamous cell mucosa of the esophagus. This patient was referred to our clinic because of lower to be a small-cell carcinoma of the lung extending to the mediastinum. He died of the lung cancer one year after operation.

PUBDATE: 19830701
The pre-therapy serum levels of immunosuppressive acidic protein (IAP) were assayed in patients with cancer of the uterine cervix and ovarium using the single radial immunodiffusion method. These levels were normal in patients with early cancer of uterine cervix (O and Ia stage); in advanced cancer patients they were increased with cancer stage. The serum IAP levels in ovarian cancer were higher than in patients with advanced uterine cervical cancer.

The University of Geneva has notified the National Cancer Institute that work on nuclear transplantation done by one of its researchers, Karl Illmensee, is under review for alleged irregularities. Other scientists have had difficulty in reproducing the results of Illmensee's research, which was thought to have important implications for the cloning of mammals. Illmensee, an NCI grant recipient, had performed similar experiments as a visiting professor at Bar Harbor's Jackson Laboratory in collaboration with Peter Hoppe. An investigation into that work is now being planned.

CT is a non-invasive examination technique, which is able to demonstrate laryngeal and pharyngeal anatomy in a three-dimensional manner and give qualified information in addition to the clinical findings. In the classification of laryngeal and hypopharyngeal cancer, CT is superior to conventional radiological methods. CT complements direct and indirect laryngoscopy in showing tumour infiltration of surrounding structures.

PUBDATE: 19830101
67 breast cancers with a maximum diameter of up to 10 mm on the histological section were retrospectively analysed according to the primary situation for medical consultation and the way to cancer diagnosis. 46 women presented themselves with clinical signs. 19 women had an anamnestical risk, whereas only two women were without any risks or symptoms. 34 tumourspecific clinical signs are followed by 25 tumourspecific mammograms. Eight small invasive carcinomas were detected only under the microscope, two of them in mastectomy specimens. Even small cancers, therefore, are mostly diagnosed from the patients themselves. The contribution of mammography is tremendous. Intensified systematic histological examination of biopsies and mastectomy specimens is mandatory especially in high risk patients.

TITLE: Phase II study of procarbazine in small cell carcinoma of the lung.
PUBDATE: 19820201
Thirty-nine patients with small cell anaplastic carcinoma of the lung were given oral doses of procarbazine ranging from 100 to 200 mg/m2 on Days 1-14 every 4 weeks. All patients had previously received combination chemotherapy. No responses were observed in the 24 evaluable patients. In reviewing the literature, we were unable to find evidence that the inclusion of procarbazine in combination chemotherapy is beneficial in small cell anaplastic carcinoma. Accordingly, it is concluded that procarbazine should not be considered as a useful drug in the management of this cell type of lung cancer.

The authors analyze results of the surgical treatment of pulmonary cancer for 35 years (1947-1981). Operations were fulfilled in 2269 patients, in 1869 of them the operations were radical. The resectability increased from 59,2% (1947-1959) to 92,4% (1973-1981). The postoperative lethality decreased from 15,1% to 7,0%, at the period of 1980-1981 it was 3,7%. The specific weight of preserving operations, lobectomies, resection and plasty of bronchi included, increased from 3,9% to 59,7%. The ways for further improvement of results of the surgical treatment of pulmonary cancer are shown.

PUBDATE: 19850501
There are numerous, different recommended systems for staging cancer of the maxillary sinus. No single system is known to be better than another. We have developed a method of simultaneously staging a tumour of the maxillary sinus into each of six different TNM staging classifications. This enables a comparison of the various classifications and provides a number of advantages: staging can be done quickly and easily, and is automatically standardised; it assists careful documentation of tumour extent and it ensures comparisons of like with like, which should enable a more critical and useful appraisal of the results of treatment.

Myasthenic syndrome secondary to small cell lung cancer has been studied in five patients who achieved complete remission with chemotherapy. The relationship between response or relapse of the neoplasm to therapy and the activity of the myasthenic syndrome is variable. This inconsistent clinical correlation between the cancer and its paraneoplastic syndrome is examined in view of current data on the immunologic basis of myasthenic syndrome.

PUBDATE: 19851001
A decade ago, interest in "enhanced" natural radiation was focused primarily on localized industrial activities, e.g., uranium, radium, thorium and phosphate production. Recognition that all human activities influence natural radiation exposure, and that indoor radon exposure is particularly sensitive to such activities (building construction practices, occupant life-styles) has added a new dimension to the problem. Public health assessments must address the problem of high individual risks of lung cancer as well as substantial population risks that are only minimally ameliorated by reduction of high exposures. In the United States, Federal, State, and local government agencies are collaborating with the private sector in developing the information needed for public guidance. Voluntary standards relating to building construction practices are likely to be developed.

TITLE: Cancer in the elderly: basic science and clinical aspects.
PUBDATE: 19850201
The incidence of cancer increases progressively with age. Rearrangements of genomes have been found to accompany cellular aging. These factors, in concert with age-dependent alterations in immune function and host defense, may help to explain the increased risk of malignant disease in aged persons. The clinical presentation and natural history of neoplasia are also affected by aging. This conference reviews recent developments in these areas, examines the effects of drug use in the elderly and implications for management, and discusses current information on how age may influence the response of cancer to therapy.
