We present a long-term survivor with isolated recurrent tumor in the incisional scar that developed 9 months after a curative resection for primary lung adenocarcinoma. She underwent an excision of the tumor followed by systemic chemotherapy, and is currently alive with no evidence of disease 69 months after the recurrence. Isolated incisional recurrence after complete resection of lung cancer is a rare phenomenon and may be curable using multimodal treatment.

Hickman catheters have previously been conventionally placed by surgical dissection. This usually performed by experienced surgeons and is carried out under general anaesthesia. We report our preliminary experience in Hickman catheter placement by percutaneous technique in twenty patients. We outline the implantation methods and complications encountered by this technique. The procedure is relatively simple provided the operator is skilled in central venous cannulation. The chief advantages are that the procedure can be done under local anaesthesia and results in less trauma compared with surgical dissection. Such an alternative in catheter insertion would promote wider usage of Hickman catheters in cancer patients.

A panel of seven chromosome 11p13 restriction fragment length polymorphisms (RFLPs) detected by five DNA probes is described. Two alleles were identified for each polymorphism, and Mendelian segregation of alleles was observed. Allele frequencies range from 0.13/0.87 to 0.44/0.56. This panel of 11p13 RFLPs will be useful for linkage studies investigating the role of 11p13 genes in Wilms' tumor, aniridia, and genitourinary anomalies. Additionally, these RFLPs will be important tools for studying tumor-specific 11p13 alterations in Wilms' tumor and other cancers.

We have used different measures of mortality in the same group of 438 patients presenting consecutively with histologically proven adenocarcinoma of the prostate, in order to assess the values and deficiencies of each method. Flow cytometry offers a relatively simple method to determine the DNA content of colorectal cancers. However, the technique can not discover minor changes in karyotype which may characterize near-diploid tumors, nor will it detect very small populations of aneuploid cells. In an attempt to improve the detection of such cells, we incubated biopsy material prior to analysis. This method revealed an aneuploid subpopulation in one of two biopsies studied. This simple protocol may ultimately lead to a more accurate assessment of the ploidy in colon cancer.

It is concluded that four sets of findings are valid: (1) diminished adrenal androgen production, probably genetic, in women with premenopausal breast cancer; (2) ovarian dysfunction (luteal inadequacy plus increased testosterone production) in breast cancer at all ages; (3) increased 16 alpha-hydroxylation of estradiol in breast cancer at all ages; and (4) evidence that prolactin is a permissive risk factor for breast cancer, and that the pregnancy-induced decrease in prolactin levels may account for the protective effect of early pregnancy against breast cancer.

Infections caused by MRSA in otorhinolaryngology were evaluated in this study. From October 1990 to the end of September 1991, MRSA was detected in one hospitalized patient and 14 outpatients of the otorhinolaryngology department. MRSA was detected in the tonsils of five patients other patients--including four with cancer and other underlying diseases. In two of the four patients with chronic otitis media, otorrhea was stopped by disinfecting external ear toxin. Osteomyelitis of the upper jaw in neonates and infants must be treated in consideration of MRSA, as it is a hospital infection caused by cephem-resistant Staphylococcus.

We describe a new concept for female bladder replacement introducing an ileocecal pouch with the intact appendix vermiformis used as an orthotopic neourethra. Based upon the results of preceding experimental studies this procedure was performed on a woman with the diagnosis of focal invasive bladder cancer. Eight months after the operation the patient is completely continent and back to work. This new type of urinary diversion with improved body image can be offered to female patients undergoing cystectomy for bladder cancer.

The process of programmed cell death, or apoptosis, is now recognized as an important component of multistep carcinogenesis. Traditionally, interventions used in the treatment of cancer patients were predicated on enhanced rates of proliferation in the tumor cells. It is now appreciated that tumor growth may, in many circumstances, be more a function of reduced rates of cell death rather than enhanced proliferation per se. Our understanding of the molecular regulation of apoptotic cell death is leading to the design and implementation of treatment strategies based on cell death induction.
