TITLE: [Management of vomiting induced by polychemotherapy in Hodgkin's disease]

Sixty patients with Hodgkin's disease were treated with a polychemotherapy regimen including very emetic dacarbazine and adriamycin in addition to bleomycin and vincristine on 622 treatment days during the last 10 years. Nausea in 97% and severe vomiting in 91% were observed during the treatment causing considerable distress to patients which led to patient noncompliance in 26.6%. Among the used antiemetic agents haloperidol was less active than high-dose metoclopramide alone or with high-dose methylprednisolone which were extremely effective. The control and prevention of drug-induced emesis is a major problem for cancer patients and their oncologists. The present situation and the home tasks are also discussed on the basis of literature and their observations.

Since 1968, we have carried out cryoprostatectomy on prostatic hypertrophy and prostatic cancer patients. As for the method of monitoring during cryoprostatectomy, digital examination only was carried out at the beginning in order to confirm the range of freezing and to determine the freezing time. In 1972, we developed a cryoprostatectomy method under direct intravesicular observation using the Trocar Cystoscope and furthermore in 1980, we developed echoguided cryoprostatectomy using the transrectal linear electronic scanner. These new developments ensure the safety of cryoprostatectomy and furthermore, cryoprostatectomy can now be easily carried out. The above-mentioned procedures are described.

Two cases of systemic lupus erythematosus in association with primary gastric adenocarcinoma are presented. A pathological analysis of the two cases of gastric cancer does not support the hypothesis that lupus and its associated vasculitis is involved in the pathogenesis of gastric carcinoma. A critical review of the literature reveals numerous case reports suggesting the association of systemic lupus erythematosus with various malignancies; however, there is no definitive study which substantiates the association of gastric or other cancers with lupus. Future studies will be required to assess appropriately the possibility of an association between lupus and cancer.

TITLE: The postoperative classification for uterine cervical cancer and its clinical evaluation.

A postoperative classification for uterine cervical cancer has been made in consideration of the spatial spreading of cancer, biological malignancy of 120 cases which were treated with radical hysterectomy and pelvic lymphadnectomy. This classification corresponds extremely well to prognosis. The 5-year survival of the cases with prognostic index (P.I.) 9 or less was 96.1%, while those with P.I. above 10 showed 31.8%. In the Shinshu University School of Medicine clinic, this classification has become indispensable for decision of postoperative irradiation, selection of irradiation methods, and chemotherapeutic agents. Using this classification for individualized therapy, the survival rate was elevated in advanced cancer, class III or IV, with lymph node metastasis and P.I. above 10.

A case of early esophageal cancer, 2.5 cm in length, located just above the esophagogastric junction is presented. This cancer seemed to originate from the fundic gland of the esophagus, because it consisted of the submucosal invasion of well-differentiated adenocarcinoma which was almost completely covered with the squamous cell mucosa of the esophagus. This patient was referred to our clinic because of lower to be a small-cell carcinoma of the lung extending to the mediastinum. He died of the lung cancer one year after operation.

The pre-therapy serum levels of immunosuppressive acidic protein (IAP) were assayed in patients with cancer of the uterine cervix and ovarium using the single radial immunodiffusion method. These levels were normal in patients with early cancer of uterine cervix (O and Ia stage); in advanced cancer patients they were increased with cancer stage. The serum IAP levels in ovarian cancer were higher than in patients with advanced uterine cervical cancer.

The University of Geneva has notified the National Cancer Institute that work on nuclear transplantation done by one of its researchers, Karl Illmensee, is under review for alleged irregularities. Other scientists have had difficulty in reproducing the results of Illmensee's research, which was thought to have important implications for the cloning of mammals. Illmensee, an NCI grant recipient, had performed similar experiments as a visiting professor at Bar Harbor's Jackson Laboratory in collaboration with Peter Hoppe. An investigation into that work is now being planned.

Previous studies have suggested that certain occupational groups may be at increased risk for prostate cancer. A cancer registry-based case-control study was conducted to investigate possible associations between various occupations and the risk of prostate cancer among 1,239 case subjects and 3,717 age-matched control subjects. Elevated relative risk estimates were observed for farmers, mechanics, sheet metal workers, separating machine operators, and for men employed in several manufacturing industries. These findings are discussed in the context of previous studies, and suggestions for future research are provided.

Biological response modifiers (BRMs) strengthen the host defense mechanism/subsequently producing therapeutic effects against neoplastic and other diseases. Many kinds of BRM are applied for malignant skin tumors. Administration is both systemic and intratumoral, evaluable lesions can be measured in one, two and three directions. Furthermore there are also methods for judging the effects by clinical symptoms. These involve evaluation of macroscopic effect, from which it is possible histopathological judgement. Therefore, the evaluation of effects on skin cancer is very complex, and a new method is necessary. Accordingly we devised a method of judgement summarizing these evaluation procedures.

Incidence of telangiectasia and ulcer among 118 postoperated breast cancer patients, who were tangentially irradiated at Department of Radiology, Kyushu University from 1961 to 1980, followed more than 5 years were evaluated. Telangiectasia occurred in 5% at 20-35 Gy, in 50% at 50-55 Gy. Ulcer occurred in 5% at 50-55 Gy, in 50% at 70-75 Gy. Skin tolerance dose in Rubin's Table (1972) is correct.

The authors worked out methods of topometric investigation of cervical cancer patients during preparation for intracavitary irradiation using 252Cf fast neutron sources. The use of balloon catheters made it possible to determine a coefficient of projection enlargement in the point of interest, to determine accurately the distance between endostats, sources and contrasted organs of interest: the bladder and rectum. The results of the studies were used for individual design of intracavitary treatment as well as for the determination of radiation exposure of the pelvic organs in 38 patients. The patients received a total of 138 investigations.
