Cancer surgery has existed as an elective treatment modality for more than 150 years, yet it is relatively unsophisticated in light of the advances seen in other medical disciplines. Using the nuclear medicine technique of lymphoscintigraphy (i.e., mapping lymph nodes using a radioactive isotope), the surgeon can perform a sentinel lymph node biopsy that targets only the lymph nodes that first drain a primary tumor site. This procedure can provide accurate staging information without risking the potential morbidity associated with lymph node dissection. This type of precise biopsy is made possible by using a gamma-detecting instrument coupled with an ultrasensitive probe designed for intraoperative use.

The authors mailed a pilot survey to 500 randomly selected general dentists. Although most dentists reported their knowledge of oral cancer to be current, their actual knowledge of oral cancer risk factors and signs and symptoms was inconsistent. Conflicting data were also found between their level of knowledge and their provision of oral cancer examinations. Most respondents agreed that patients should undergo an oral cancer examination annually, yet many do not provide this service.

In eukaryotes, 3-hydroxy-3-methylglutaryl coenzyme A (HMG-CoA) reductase is a key enzyme that catalyses the synthesis of a precusor of cholesterol as well as non-sterol isoprenoids, mevalonate. The regulation of the enzyme activity occurs at the transcription, post-transcription, translation and protein degradation levels. Cholesterol contributes to the generation and development of atherosclerosis while non-sterol isoprenoids play a role in regulation of cell proliferation, signal transduction and generation of cancers. At present, the enzyme is the target of several drugs effective against atherosclerosis.

Pruritus associated with malignancy may be one of the most bothersome symptoms in advanced cancer. Its control is still difficult to achieve and is a challenge to palliative medicine specialists. We describe five patients suffering from pruritus of different etiologies who responded rapidly to administration of paroxetine, a serotonin reuptake inhibitor, in a dose-dependent manner. Two patients experienced transient but severe nausea and vomiting. We suggest that paroxetine's antipruritic effect may be explained by rapid downregulation of the 5-HTs receptors, which may have an important role in the generation of pruritus and pain.

Recent discoveries of many new genes have made it clear that there is more to the human Y chromosome than a heap of evolutionary debris, hooked up to a sequence that happens to endow its bearer with testes. Coupled with the recent development of new polymorphic markers on the Y, making it the best-characterized haplotypic system in the genome, this gives us new opportunities to assess its role in disease and selection, through association studies with phenotypes such as infertility and cancers. However, the peculiar genetics of this bizarre chromosome means that we should interpret such studies particularly cautiously.

We report that alpha-tocopheryl succinate, a vitamin E analogue with pro-apoptotic properties, selectively kills cells with a malignant or transformed phenotype, i.e. multiple haematopoietic and carcinoma cell lines, while being non-toxic to normal, i.e. primary and non-transformed cells. OBJECTIVE: To study the relationship between the microvascular density(MVD) and the prognosis in laryngeal cancer. METHOD: Microvessel quantitation in 31 cases of laryngeal cancer was done with factor VIII-related antibody and SABC method. RESULT: The quantitation of microvessels in laryngeal cancer were negative of histologic grade and type, but were significantly different (P < 0.05) between metastasis (10.23 +/- 0.78) and without metastasis(6.38 +/- 0.33) two years after operation. CONCLUSION: The angiogenesis of laryngeal cancer is correlated with the lymph node metastasis and prognosis.

The prognostic value of the expression of the cyclin-dependent kinase inhibitor p21 and the p53 tumour suppressor gene was examined using immunohistochemistry in 60 patients with laryngeal cancer. Multivariate analysis using Cox's proportional hazard method, showed that p21 expression (P = 0.02) and advanced T stage (P = 0.003) significantly predicted survival. It was concluded that p21 expression may be a useful prognostic indicator in laryngeal cancer.

Barrett's esophagus is a premalignant lesion of the esophagus that arises as an abnormal tissue response to epithelial injury from gastroesophageal reflux. Barrett's esophagus has previously been considered an irreversible lesion that required life-long surveillance to prevent malignant transformation. Recently, combination therapy with pharmacologic or surgical control of acid reflux combined with endoscopic delivery of a mucosal injury appears to have the capability of reversing superficial Barrett's tissue, and perhaps deeper tissue as well. Whether Barrett's esophagus is cured and cancer/dysplasia prevented by these techniques will require long-term follow-up of these patients.

The results of three published randomized controlled trials of fecal occult blood testing (FOBT) provide unequivocal proof of the principle that screening reduces mortality from colorectal cancer (CRC). However, several interesting questions remain in interpreting and applying the results of the clinical trials, including: how well does FOBT screening work (i.e. how much can CRC mortality be reduced), how does it work, when is it worthwhile and worthwhile doing, and how can technique be optimized? The answers to these questions have important practical and clinical implications.
