Biopsy of the uterine cervix from a 46-year-old woman who suffered from epigastric pain and weight loss showed metastatic adenocarcinoma. The primary site of the tumor was the stomach. At laparotomy, disseminated adenocarcinoma on the peritoneum and Krukenberg's tumor in the right ovary were found. A palliative partial gastrectomy, resection of the right ovary, and postoperative chemotherapy were performed. The possible mechanism of metastasis of extragenital cancer to the uterus is discussed.

Operative ultrasound examinations were performed using a 7.5-MHz transducer during nine operations for advanced gastric cancers and during six for early cancers. Normal stomach wall showed five layers that corresponded to histologic structures. Fourteen of 15 tumors, including nonpalpable early cancers, were localized with ultrasonography. In addition, both depth of penetration and lateral wall extension of the gastric cancers were precisely determined with this technique. Our preliminary results indicate that high-resolution operative ultrasonography may become a diagnostic aid during gastric cancer operations.

Total bladder replacement with tubular sigmoid colon and detubularized ileocecal bowel segments was performed on 17 patients after cystoprostatectomy for bladder cancer. There were few complications and patient acceptance was excellent. Daytime continence was achieved in most patients but voiding patterns were superior with detubularized segments. However, enuresis was common with both segment types. Total urinary continence (day and nighttime continence) was achieved in 7 patients with an artificial urinary sphincter that was activated only at night. Total bladder replacement is an acceptable form of urinary diversion after cystoprostatectomy in appropriate patients.

Infiltrating ductal carcinoma of the breast is histologically heterogeneous and may be subdivided into three main types (A, B, and C) which correlate with the likelihood of long-term survival. Examples of each type were tested against three monoclonal antibodies (H59, H71, and H72) prepared against the breast cancer cell line ZR-75-1. The reaction patterns of type B tumors were significantly different from those of types A and C with antibodies H59 and H71, but not with H72. Thus, the reactivity to these monoclonal antibodies, and presumably the antigen composition of the tumor cells, correlates with the histologic appearances and probability of long-term survival.

Although most causes of hematuria are benign, urinary tract bleeding may signal the existence of a life-threatening disease. Gross and microscopic hematuria share a common differential diagnosis, including urinary tract infection, urolithiasis and bladder cancer. Clinical evaluation may be guided by the patient's age, sex, medical history and physical examination. Intravenous pyelography or sonography is usually the first procedure performed, although cystoscopy is indicated in the face of active bleeding. Those patients who remain undiagnosed after a complete evaluation should be followed with routine urinalysis and cytology to allow early detection of malignancy.

In May 1987 and 1988, the American Cancer Society (ACS) sponsored Breast Cancer Detection Awareness (BCDA) programs throughout Wisconsin. These programs were made possible by the cooperative efforts of many ACS volunteers, hospitals, clinics, and physicians. The Wisconsin Radiological Society and the Division of Health were also involved in the planning efforts. In 1987, a total of 10,207 women were screened at 100 facilities, and 75 carcinomas were detected. In 1988, of 8,802 women screened at 102 facilities, 52 carcinomas were detected.

Thrombocytosis above 40.0 X 10(4)/mm3 occurred in five of six (83%) patients with malignant mesothelioma. In contrast, the incidence of thrombocytosis was 7.5% in patients with lung adenocarcinoma, 12.5% in squamous cell carcinoma, 5.1% in small cell carcinoma, and 41.7% in large cell carcinoma, respectively. The platelet count in large cell carcinoma was significantly higher than that in other cell types of lung cancer; however, the platelet count in malignant mesothelioma was much higher than that in large cell carcinoma. These results show that the incidence of thrombocytosis seems to be high in malignant mesothelioma, although the mechanism is thus far unknown.

Echocardiography may be used to demonstrate invasion of hilar lesions into the heart. We report a case in which echocardiography demonstrated nonresectability of a lung cancer which was confirmed via a Chamberlain procedure.
