Our own data, as well as those of many other workers, suggest that clinicians cannot depend on primary lesion Gleason scoring alone to predict the incidence of positive nodes in patients with prostatic cancer. We continue to find node involvement in 20 per cent or more of patients with Gleason 2 through 4 primary lesions. Furthermore, we continue to find that a large number (nearly 40%) of patients with Gleason 8 through 10 primary tumors do not have lymph node spread. Because of the significance of node status on patient prognosis and its potential impact on treatment decisions, we continue to advocate surgical lymph node staging in patients with localized prostate cancer.

Pleural effusions are common in the setting of lung cancer. The clinician must establish whether the effusion is malignant, ruling out the possibility of curative surgery; paramalignant, which may or may not rule out surgery; or unassociated with the cancer. A pleural effusion associated with lung cancer is an ominous finding, but a small percentage of patients in this setting will be candidates for curative surgery. When a malignant pleural effusion is diagnosed by cytology or histology, the clinician must decide on the most appropriate form of palliative therapy for the symptomatic patient. In the symptomatic patient with a reasonable life expectancy, chest tube drainage with the instillation of tetracycline hydrochloride appears to be the most effective and least morbid form of palliative therapy.

Forty-five percutaneous needle biopsies of pancreatic masses were performed in 41 patients in whom there was clinical or radiological concern for the diagnosis of cancer. Computed tomography (CT) was the imaging technique used in 41 of the 45 biopsies. An overall sensitivity of 85%, specificity of 100%, and accuracy rate of 88% were achieved, with serious complications in three of the 45 attempts (6.7%). These figures show that percutaneous pancreatic biopsies have the potential for greater accuracy than has been previously reported and are relatively safe when compared with surgery.

The case records of patients presenting with metastases from an unknown primary cancer (MUP) have been reviewed. Important prognostic variables were performance status and the presence of disease in more than one system. Patients of poor performance status and disease in multiple organs had a median survival of one month and 87% were dead within three months. Those patients of good performance status and disease apparent in only one organ had a median survival of seven months. Patients with carcinoma confined to lymph nodes in the high cervical region who received treatment with radiotherapy had 3- and 5-year survival rates of 26% and 17% respectively.

A phase II study of cis-diamminedichloroplatinum (CDDP, 80 mg/m2, every 3 weeks) was performed in patients with non-small cell lung cancer (NSCLC). The overall response rate to CDDP was 14% (6/42). In patients without prior chemotherapy, the response rate was 20% (2/10), and in patients with prior chemotherapy, the response rate was 13% (4/31). The major side effect was gastrointestinal toxicity. It was concluded that CDDP at a dose of 80 mg/m2 every 3 weeks is effective against NSCLC.

Among South African black women in rural areas, breast cancer is rare and remains very uncommon among urban dwellers in Soweto, Johannesburg. Since time of menopause is an influencing factor in breast cancer in white women, mean times were determined in series of 1850 and 1255 black women in rural and urban areas, respectively. Using probit analysis, respective means were 49.5 (SD 4.7) years, and 48.9 (SD 4.2) years. Values thus differ little from those reported for white women. Obviously case control studies are needed to learn of the bearing of menopausal age on breast cancer proneness in this very-low-risk population.

Diagnostic delay was measured in 455 consecutive patients who had large bowel cancer and it was found to be less pronounced than in a previous study in the same hospitals. However overall corrected five year survival remained the same. The duration of symptoms was not correlated with the stage of the disease, nor with five year survival. The pattern of disease resembled other Western series, with an overall corrected five year survival rate of 33.2%. Prognosis was significantly better in women and worse in patients with an elevated ESR on presentation. Intestinal obstruction due to carcinoma did not adversely affect prognosis.

Eleven hypercalcemic patients affected by various kinds of cancer were treated with dichloromethylene diphosphonate (Cl2MDP), 400 mg diluted in 500 ml of saline solution, i.v., during 3 h. Drug administration normalized plasma calcium levels in all treated patients, within 72 h for patients with lung cancer; within a longer period for other kinds of cancer. Moreover, Cl2MDP also reduced calciuria in treated patients. The use of the drug was not followed by side effects and was also well tolerated in patients with kidney insufficiency.

A double-blind controlled study of the effects on semen quality of 4 alpha-adrenergic agents with and without antihistamines was conducted in a patient who had failure of emission secondary to retroperitoneal lymphadenectomy for testis cancer. No other controlled studies are available. All of the alpha-adrenergic drugs allowed the patient to produce an ejaculate. The effects were similar. The addition of antihistamine did not improve the semen quality. Four days of treatment were consistently more effective than a single dose, especially for sperm motility. A pregnancy resulted. It is concluded that long-term treatment with alpha-adrenergic drugs is indicated in men with failure of emission secondary to retroperitoneal lymphadenectomy.

The possibility of a cancer risk associated with fluoridation of public water supplies is re-examined using mortality data for 35 US cities, 20 with fluoridated water supplies and 15 with non-fluoridated water. Crude cancer death rates, and mortality ratios standardized for age, sex, and ethnic group are examined, using four alternative sets of standard rates and three different combinations of pericensal years' deaths. Changes in cancer mortality before and after fluoridation in the fluoridated cities are compared with changes in the non-fluoridated cities over the same time period. In none of the analyses have differences in mortality trends been found that could not be due to chance alone. Thus, these results do not support the suggestion of an association between fluoridation of water supplies and cancer mortality.

The incidence of deep venous thrombosis of the legs (DVT) was studied in 119 critically ill patients by 125I-labeled fibrinogen scanning; the efficacy of low-dose heparin prophylaxis was assessed in a randomized, double-blind study. DVT occurred in 29% of control patients and in 13% of patients receiving heparin 5000 U subcutaneously twice daily. DVT was found mainly in men and was associated with circulatory impairment, respiratory failure and recent vascular or cancer surgery. In a comparison study of medical patients, DVT occurred in 10% untreated and 2% treated. In conclusion, the critically ill are at high risk of venous thromboembolism and low-dose prophylaxis is warranted in those who have no hemostatic impairment.

In a series of 405 patients with epidermoid carcinoma of the head and neck, 52 patients (13%) developed multiple epidermoid carcinomas of the upper aerodigestive tract. Subsequent primary tumors developed with nearly equal frequency in the head and neck, lung, and esophagus. Forty of the patients developed a subsequent primary carcinoma within five years of the first. Thirty-eight (73%) of the patients who developed a subsequent primary carcinoma survived less than two years from its diagnosis. The majority (34/50) of the patients died from the subsequent primary carcinoma. We recommend that panendoscopy and an esophagogram be performed on initial evaluation of patients with head and neck cancer. A thorough reexamination is warranted if new symptoms or signs develop.
