The production of superoxide and nitric oxide individually has been associated with the development of several diseases but only recently has it been realised that interactions between them may also be important in disease pathology. The central hypothesis which is emerging is that the balance between nitric oxide and superoxide generation is a critical determinant in the aetiology of many human diseases including atherosclerosis, neurodegenerative disease, ischaemia-reperfusion and cancer. These ideas are discussed in this short overview and placed in the context of the current and future status of therapies which could modulate the balance between nitric oxide and superoxide.

Calcifications are found in many women undergoing mammography. The majority of these calcifications are benign. Calcifications suggestive for malignancy prove to be cancer in about 30% of cases after biopsy. Study and personal experience should result in ready recognition of characteristically benign calcifications. Calcifications suggestive for malignancy should be biopsied and the ratio of malignant over benign biopsies should be as high as possible. The purpose of this paper is to present a practical approach to the evaluation of breast calcification, by means of a survey of the pattern of calcifications in breast pathology. We describe six types of typically benign calcifications and six criteria that entail an increased risk of malignancy.

Data from collaborative trials clearly indicate that the primary effect of adjuvant therapy is to delay death from breast cancer by months or years. However, adjuvant chemotherapy does not appear to prolong the survival of women aged 50 years or older, and tamoxifen has not yet been shown to improve the survival of women younger than age 50. In three trials involving node-negative patients, all showed a highly significant improvement in disease-free survival despite different chemotherapeutic regimens and durations of regimens.

Celiac sprue, or nontropical sprue, is associated with an increased incidence of digestive tract malignancy. We report a patient with celiac sprue who developed two primary adenocarcinomas of the jejunum > 2 years apart. The second cancer was asymptomatic and detected by surveillance computed tomography. Small-bowel cancers are unusual, and frequency of follow-up has not been established, so that it is important to have a high index of suspicion for these malignancies in celiac patients with vague gastrointestinal symptoms.

Laser blood-flow measurements were made on the tumor surface of 18 human oral cancer tumors growing on the shank of nude mice during and up to 6 h after photodynamic therapy (PDT) to detect changes in microcirculation. In eight animals, the same procedure was done without PDT. Simultaneously, the skin microcirculation in the contralateral leg was measured to detect systemically caused changes in microcirculation. Two days later, specimens of the tumor region were taken and analyzed histologically. Our results suggest a mainly direct effect of PDT on the tumor cells rather than tumor necrosis caused by decreased microcirculation.

Ataxia-telangiectasia is a complex syndrome that includes a very high cancer risk in children with a progressive cerebellar ataxia, the onset of which occurs in early infancy. Ocular telangiectasiae often do not appear until several years after the ataxia. The most common type of malignancy is lymphoma, usually of the B-cell type. Leukemias also occur. Failure to diagnose ataxia-telangiectasia in an infant with lymphoma or leukemia may result in radiation therapy with conventional dosages, which is contraindicated in ataxia-telangiectasia patients.

Palliative therapy for rectal carcinoma includes control of bleeding, tenesmus, mucous discharge, obstruction and pain. These local symptoms can cause serious morbidity for patients with locally advanced or metastatic rectal cancer. As an alternative to surgical resection for palliation, the authors applied endoscopic laser fulguration in 15 patients with rectal tumours; 14 experienced immediate relief of symptoms. At 6.4 months' follow-up, two patients had recurrent tenesmus and two required a colostomy. A high degree of patient acceptance makes laser fulguration for rectal tumours an important modality, for palliation; however, control of tenesmus may be temporary.

A very high incidence of cancers (10%) is recorded in patients homozygous for ataxia telangiectasia (AT). From 1970 to 1987, 35 children were investigated in the department of pediatric neurology in Lille. Three developed a malignancy (one hepatic tumor, one Hodgkin's disease and one non Hodgkin's lymphoma). Constitutional chromosome fragility and immune deficiency are the main features of AT. The first one is probably linked to the pathogenesis of malignancies. Moreover, cancer therapy has to take these features into account.

Congenital hypertrophy of retinal pigment epithelium (CHRPE) has been shown to be a frequent extracolonic manifestation of adenomatous polyposis coli (APC). The presence of CHRPE in patients with adenomatous polyps from families with cancer family syndrome suggests possible involvement of the APC gene locus in syndromes associated with less florid polyp formation than seen in APC. It also emphasises that caution must be exercised in using the presence of CHRPE clinically as a marker for APC in isolated at-risk individuals.

Pure squamous carcinoma is a rare pancreas cancer histological variety. Moreover the association of malign humoral hypercalcemia and neoplasia of the pancreas exocrine portion is extremely rare. The case of a patient with pure squamous pancreas carcinoma and hypercalcemia is discussed. The absence of bone metastasis allowed to classify it within the malign humoral hypercalcemia syndrome. It is the first case described in the spanish scientific literature and the third in the international literature of an epidermoid pancreas carcinoma associated to malign humoral hypercalcemia.

TITLE: Clinical application of radioimmunoimaging in the detection of tumors--current status in China.

In this article, the development of radioimmunoimaging (RII) in China is reviewed. Since 1984 conventional heterologous specific antibodies (Abs), such as anti-carcinoembryonic antigen (CEA) and anti-alphafetoprotein (AFP) Abs, have been used to detect cancers of the colon, ovary, lung, liver, etc. Later on, Abs against specific tumor cell-associated antigens have been produced in many institutions and applied clinically, with quite satisfactory results (detection rate ranging from 80-90%). Recently, human monoclonal antibodies (McAbs) are also being prepared. Finally, the existing problems to be overcome are discussed.
