TITLE: Improved genomic/nuclear DNA extraction for 8-hydroxydeoxyguanosine analysis of small amounts of rat liver tissue.

Using two different commercially available extraction kits, genomic/nuclear DNA could be recovered from rat liver samples as small as 10 mg. Background 8-hydroxydeoxyguanosine levels in such DNA were low and stable at 0.26-0.30 +/- 0.01-0.03/10(5) guanine residues. The minimum tissue wet weight required for the accurate 8-hydroxydeoxyguanosine analysis was 25 mg. The present results indicate that routine and reliable assessment of the involvement of oxidative DNA damage in the development of various diseases, including cancer, is feasible using a variety of tissue sources.

Breast symptoms are appropriately evaluated by a breast-oriented history and by the diagnostic triad of clinical breast examination, FNA, and mammography. In the case of a palpable dominant mass, concordance of the diagnostic triad yields a reliable clinical diagnosis. If there is not concordance, or if there is any doubt about the diagnosis either on the part of the physician or the patient, open surgical biopsy provides the definitive histologic diagnosis. The goal in clinical practice is to detect nonpalpable cancers by ordering screening mammography for all patients eligible by age, history, or both. The long-term cancer-free survival of women treated for nonpalpable breast cancer is excellent.

The authors collected dietary intake data about the food and nutrient intake of 49,501 male health professionals. Edentulous participants consumed fewer vegetables, less fiber and carotene, and more cholesterol, saturated fat and calories than participants with 25 or more teeth. These factors could increase the risks of cancer and cardiovascular disease. Mean differences in intake ranged from 2 to 13 percent, independent of age, smoking, exercise and profession. Longitudinal analyses suggest that tooth loss may lead to detrimental changes in diet.

Breast cancer occurring during pregnancy is a rare clinical situation. A multidisciplinary approach is needed involving an obstetrician a medical oncologist and a surgeon. It should be treated according to the same principles applied in non pregnant patients. Abortion or termination of pregnancy don't improve survival. Decisions regarding abortion should be based on the desires of the patient and on therapeutic necessities. Surgery if necessary is always possible, radiation therapy should be avoided by reason of fetal toxicity, chemotherapy if absolutely necessary is possible after the first trimester.

Audit of a general surgical practice in the rural Victorian town of Echuca identified 28 new patients with breast cancer between September 1992 and August 1995 (10% of those with breast conditions). Ovarian cancer is the leading cause of death from gynecologic cancer. Despite aggressive cytoreductive surgery and platinum-based chemotherapy, the 5-year survival for patients with clinically advanced ovarian cancer is only 15 to 20 percent, although the cure rate for stage I disease is usually greater than 90 percent. These statistics provide the primary rationale for ovarian cancer screening. This overview of the current literature serves to show that mass screening for ovarian cancer is far from being established and fraught with management and procedural dilemmas. The reasonable assumption being that there is little evidence to support widespread screening of large populations of women who do not have familial or genetic risk factors for ovarian cancer.

TITLE: Successful oocyte donation after stage 1C serous ovarian cancer.

A stage 1C serous epithelial cancer was discovered and treated during laparoscopy performed for infertility investigation. An immediate in-vitro fertilization (IVF) was scheduled to obtain frozen embryos. Oncological treatment was then completed by radical surgery (with uterine conservation) and chemotherapy. Two years later, the thawed embryos were unsuccessfully transferred. However, following oocyte donation, the patient became pregnant and delivered two babies. From this observation, the authors discuss the strategy required in cases of early ovarian cancer associated with infertility.

The results of a series of 3,596 patients affected by lung cancer and submitted to multimodality treatment are reported. Long-term survival at 5 and 10 years of resected patients was related to cancer staging: 5 years after surgery 63% of treated patient died because of metastasis and only 12% because of local recurrence, while after 10 years metastases were observed in 40% and local recurrence almost in none.

Gene amplification is an important manifestation of genetic instability in cancer. Recently, the study of gene amplification has been greatly facilitated by the development of the molecular cytogenetic techniques of comparative genomic hybridization and chromosome microdissection. We describe in this brief overview a combined approach using both techniques, which allows the identification of chromosomal regions of gene amplification and provides entry point clones for target gene identification. This molecular/cytogenetic approach consisting of chromosome microdissection and comparative genomic hybridization should be valuable in identifying novel amplified genes important in neoplastic development and progression.
