TITLE: Estimation of radiation risk from screening mammography: recent trends and comparison with expected benefits.

On the basis of recent epidemiologic studies, the National Institutes of Health in 1985 provided a new estimate for radiation risk to the breast that employed a relative risk model and acknowledged greater dependence on age at exposure. Lifetime risks from a single mammogram may be calculated from this estimate and are lower than those based on the previous 1977 National Cancer Institute estimate. Possible years of life expectancy lost from annual mammography beginning at age 40 years may also be calculated and are negligible compared with estimates for years of life expectancy gained from such screening.

Estrogen-related cardiovascular dysfunction was noted in 23 out of 30 patients with prostatic cancer (PC). Coronary subjects with PC suffered from cardiac pain evident on ECG necessitating correction by effective doses of coronary active drugs. PC patients with essential hypertension exhibited frequent headache, progressive edema of the legs, drastic hypertensive reactions. It is held that estrogen therapy for prostatic cancer should be preceded and monitored by therapeutic evaluation responsible for optimal conditions to prevent and early diagnose cardiovascular complications.

Human retrovirus-like particles related to mouse mammary tumor virus (MMTV) are secreted in a steroid-dependent manner by the breast cancer cell line T47D. We report the successful large scale production and purification of these particles from culture supernatants of T47D cells and describe the experimental conditions established for this purpose. Thus, mg amounts of particles were produced by large scale culturing of T47D cells in an autoharvesting roller bottle system and purified by differential centrifugation and continuous flow ultracentrifugation on density gradients with a 50% recovery and a 350-fold enrichment.

In a retrospective study including 946 cases with colorectal cancer we analysed age, sex, distribution and stage. There was a significant higher prevalence in women and in all tumor stages women were older than men. The frequencies of Dukes C and proximal colon cancer was significantly higher in women. In older patients we found an increasing incidence of proximal cancer. Instead of tumor stage the age, sex and distribution showed no influence on the outcome.

From January 1982 to December 1989, we experienced seven multilocular cystic renal cell carcinomas (MLCRCCs) in 36 asymptomatic renal cancers incidentally diagnosed, and none in 23 symptomatic renal cancers (p less than 0.05). No multiocular cystic nephromas (MLCNs) appeared in either group. Though MLCRCCs have been considered to be, in general, extremely rare, they do occur in asymptomatic renal cancers. A multiloculated renal mass with thick septum discovered in an adult by ultrasonography or CT scan should be suspected as being a MLCRCC rather than a MLCN.

The animal data are consistent with this mechanism, because a high proportion--about half--of all chemicals tested (whether natural or synthetic) are indeed rodent carcinogens. We conclude that at the low doses of most human exposures, where cell killing does not occur, the hazards to humans of rodent carcinogens may be much lower than is commonly assumed.

An automatic needle biopsy system, Bioptycut, has been evaluated for use in the outpatient biopsy of 107 patients with suspected operable breast cancer. Of the 107 patients, 96 proved to have malignancy. The sensitivity of the test was 65%, and the specificity 100%. Bleeding problems in the cancer patient may result from the effects of the tumor on hemostatic mechanisms or from the treatment of the tumor by cytotoxic and other agents. Among the tumor-related bleeding problems are disseminated intravascular coagulation, primary fibrinolysis, thrombocytopenia, acquired platelet dysfunction, and circulating inhibitors or anticoagulants. Disseminated intravascular coagulation in most solid tumors is associated with hypercoagulability, whereas in acute promyelocytic leukemia bleeding is the most common presentation. Treatment-related bleeding disorders include the common problem of thrombocytopenia secondary to myelosuppressive chemotherapy as well as the interesting microangiopathic hemolytic anemia syndrome associated with mitomycin C and other agents.

A 49-year-old man with liver metastasis from rectal cancer was treated with cisplatin (CDDP) alone. Cisplatin was administered intravenously 3 times at a dose of 50 mg (31 mg/m2). He achieved an excellent PR (94% decrease) as determined by CT scanning, and plasma CEA level decreased from 37 to 2.2 ng/ml. The side effects were nausea, vomiting, and reflux esophagitis, but neither leucopenia nor renal dysfunction was observed. As a result, a radical operation could be undertaken.

The first two cases of breast metastases from anaplastic carcinoma of the nasopharynx are presented. In both cases the breast metastasis was solitary, but there was no delay in confirming their metastatic nature since both lesions manifested after the documentation of disseminated disease. The difference in histology, since 99% of our nasopharyngeal carcinoma patients have anaplastic carcinoma of poorly differentiated carcinoma subtype, makes it easy to differentiate them from primary breast cancer by cytology or histology.
