A 34-year-old female patient with breast cancer metastasizing to bone had a remission after a course of treatment with medroxyprogesterone acetate (MPA) 1,200 mg/day, but again had a metastasis to the lungs. The patient was then instituted on a combination therapy with UFT 400 mg/day, and MPA 1,200 mg/day. As a result, the pulmonary metastasis disappeared, along with the consolidation of the bone involved.

Squamous cell carcinomas of the esophagus, while still frequent in many locations throughout the world, now comprise a smaller percentage of esophageal cancers in the United States than was previously believed. Such tumors occur most often in black men and in association with tobacco and alcohol abuse. Adenocarcinomas of the distal esophagus represent an increasing proportion of esophageal tumors. These neoplasms arise in columnar epithelium (ie, Barrett's esophagus) or extend into the esophagus from their site of origin in the proximal stomach. Esophageal adenocarcinomas appear more often in white than black subjects and in men more than women. The clinical features and diagnostic approaches to squamous cell tumors and adenocarcinomas of the esophagus are similar.

Homologous desensitization of responses to bombesin in small cell lung cancer cells can be mimicked by protein kinase C-activating phorbol esters. We show that phorbol ester-induced desensitization can be blocked by prior treatment with the protein kinase C inhibitor staurosporine or prolonged pretreatment with phorbol esters. However, under these conditions, homologous desensitization to bombesin persists. These data indicate that phorbol ester-induced desensitization is distinguishable from bombesin-induced homologous desensitization. Protein kinase C is not a dominant component in the homologous desensitization to bombesin in small cell lung cancer cells.

Innovative strategies using games, computers, preceptors, and elective courses have allowed schools of nursing to include more comprehensive coverage of oncology-related topics. Issues concerning clinical oncology nursing opportunities for undergraduate studies still need to be clarified. Undergraduate students must be afforded the legitimacy of their status as novices in nursing and in the specialty of oncology. Didactic and clinical experiences should result in the knowledge and skills needed to develop professionally from novice to expert.

Surgical staging documented extrauterine disease in 27.9% of 168 patients with apparent early-clinical-stage uterine cancer. An analysis of operative time (78 +/- 21 min), blood loss (332 +/- 160 cc), and surgical site infection risks (4.7%) indicated little additional risk of lymphadenectomy. The long-term risk of lymphocyst (1.3%) or lymphedema (0.7%) was small. The histologic information obtained from staging was utilized to rationally guide the need for adjunctive teletherapy. The overall risk of recurrence (median follow-up, 26 months) with surgical Stage I disease was 2.6%.

I reexamined the relations of family history of breast cancer in first-degree relatives, mammographic features of breast tissue, and breast cancer risk, using data from three case-control studies. Subjects included 1,047 cases with newly diagnosed breast cancer and 2,329 controls. Family history of breast cancer showed only a weak relation to mammographic features. In addition, while family history and mammographic features were each related to breast cancer risk, associations of these two factors with risk appeared additive. In contrast to previous reports, these data suggest that, to a large extent, family history and mammographic features have independent effects on breast cancer development.

The eph gene encodes a putative receptor tyrosine kinase for an as yet unknown ligand. Some human cancer cells have been found to overexpress eph mRNAs without gene amplification. We show here that NIH3T3 cells acquire tumorigenic ability in nude mice and make colonies in soft agar with a viral LTR (Long Terminal Repeat)-driven artificial expression of the eph gene to a high level. This result supports the alleged contribution of overexpressed receptor tyrosine kinases to cell transformation.

The Lynch syndromes, also referred to as hereditary nonpolyposis colorectal cancer, are reviewed, with particular attention given to their natural history, diagnosis, surveillance, and management. Lynch syndrome I is characterized by an autosomal dominantly inherited predisposition to early onset colorectal cancer, with proximal predominance and an excess of synchronous and metachronous colorectal cancers. Lynch syndrome II manifests all of the features of Lynch syndrome I but, in addition, shows an integral association with extracolonic cancers, particularly carcinomas of the endometrium and the ovary. There are no premonitory physical signs or biomarkers of genotypic risk. Therefore, the natural history features of the Lynch syndromes are crucial to their surveillance and management.

The activity of Etoposide (VP16) in combination chemotherapy against four human transitional cell carcinoma cell lines of bladder (TCCaB) was determined by in vitro colony formation assay. Four anti-tumor agents (methotrexate: MTX, vinblastine: VBL, adriamycin: ADM, cisplatin: DDP) were used for combination chemotherapy with VP16. The ADM + VP16 combination exhibited a strong synergistic antitumor effect against the human TCCaBs compared with other combinations in this study. The combination chemotherapy of ADM + VP16 may be useful as a new chemotherapeutic regimen for advanced bladder cancer.

The spontaneous life expectancy of patients with liver metastases from colorectal cancers is dismaying. Surgical resection is the only treatment which may help these patients. After resection the 5-year survival rate averages 25%. The best results are obtained in patients with one or few metastases localized to one liver lobe. There is no effective adjuvant therapy. Systemic or regional chemotherapy and immunomodulation have little effect in patients with inoperable metastases.
