TITLE: Coding ordinal independent variables in multiple regression analyses.

The authors present a coding scheme for ordinal independent variables which may be used in various forms of regression analysis. The scheme is useful in dose-response analyses, when the objective is to identify contrasts in the dependent (or response) variable between successive levels of the independent variable, or to identify critical threshold values of the independent variables at which significant changes occur in the response. An example is given of evaluating the survival of lung cancer patients according to their stage of symptomatology. The authors discuss the interpretation of the regression coefficients when this coding scheme is used with linear regression, logistic regression, or in the proportional hazards regression model.

The issue presented in this appeal was whether a California juvenile court could order a five-year-old boy to undergo periodic medical monitoring to detect the possible recurrence of cancer, notwithstanding objections by the child's parents that such an order offended their Christian Science beliefs. Although the parents had consented to removal of the boy's eye following a diagnosis of cancer, and had permitted court-ordered chemotherapy and radiation treatment, they opposed recommendations that the boy be monitored over a two-year period for possible recurrence of the cancer. The juvenile court intervened and ordered that the parents comply with the recommendations. The parents filed a motion to reconsider the monitoring order on both statutory and constitutional grounds. The Superior Court denied the motion and the Court of Appeal affirmed.

Six patients with cancers arising in the parotid region were treated with surgery and postoperative irradiation. All cases had extensive resections with sacrifice of the seventh nerve and primary autogenous cable graft repair. Four of the six patients had good to excellent dynamic function at 18 months; the fifth case had fair mimetic function with ability to close the eye and protect the cornea, and the final case failed to demonstrate any function at one year, but was found to have recurrent tumor at the skull base.

Nine patients, including 4 with primary advanced breast cancer (stage IV) and 5 with local recurrent cancer, underwent chest wall reconstruction using an omental flap and mesh skin grafting. In 2 of these patients the defect of bony chest wall was reconstructed with an acryl-resin plate and omental flap. The postoperative course in all patients was uneventful, except for a slight necrosis on the transposed mesh skin. Flail chest or dyspnea did not occur in those with a bony chest wall reconstruction. The immediate postoperative performance status in 6 of 9 patients and also quality of life improved.

Individual reactions to a report which identifies an excess of risk near a putative source are determined mainly by some quoted significance level. One reaction, involving a commonly used 'coincidence' argument is given a simple Bayesian explanation. It is argued that interpretations of such reports should if possible allow both for data selection and for uncertainty in the null expectations underlying the significance levels. Tests are proposed, based on the first isotonic regression estimator under an order restriction, which allow for the effects of selecting a study region in the light of the data and have a simple form. Data on cancer incidence around two nuclear plants are used to illustrate.

This review analyzes recent data from international literature concerning the antiestrogen action of progesterone, progestins and the antiprogesterone RU 486 at the level of mammary cells in culture from either breast cancer lines or normal breast obtained from reduction mammoplasties. Most data indicate that progesterone, progestins and even RU 486 have a strong antiestrogen effect on breast cell appreciated by the decrease of estradiol receptor content, the decrease of cell multiplication and the stimulation of 17 beta-hydroxysteroid activity which may be considered as a marker of breast cell differentiation dependent of progesterone receptor.

Six patients with advanced disseminated cancer were treated by operatively implanting a subcutaneous injection system for epidural opiate injection. The system consisted of an access port connected to an epidural catheter. The median period of treatment during hospitalization was 16 days (9-64). Two discharged patients were treated for 9 and 150 days, respectively. The median number of injections was 92 (48-542). Patient compliance was good in all cases. There was no case of displacement, kinking or occlusion of the catheters. All patients experienced sufficient pain relief, although four, before epidural opiate treatment, had their pain triggered by activity.

However, cooperation with chemotherapy is to be considered as a reasonable way to further improve the radical cure rate, because the indication of proton therapy is limited to a single primary lesion. Thereupon, combined chemotherapy before or after completion of the proton therapy of the primary lesion to control latent metastases of lung cancer, esophageal cancer, to control multicentric lesions of liver cancer, urinary bladder cancer, or to diminish the size of the lesions with too much volume, could well contribute to improved clinical results.

The records of 695 patients with confirmed gastroenterological malignancies over a 10 year period (January 1976-December 1985) were analysed. Of these, 264 patients (37.98%) were below the age of 40 years. Primary liver cell carcinoma was the commonest malignancy while gallbladder and oropharyngeal cancers were the least common. Most of the patients (66.3%) were in their fourth decade. The prognosis was poor as most of the patients presented late.

24,000 women a year are submitted in the Federal Republic of Germany to surgery because of breast cancer. This disease causes about 10,000 deaths a year. Early diagnosis by preventative check-up and wide-spread awareness of the disease in the population gives the best chance for reducing mortality. However, in our study we found that only 38% of the patients were presented at a favourable stage. Every tenth patient already had involvement of the thoracic wall or of the skin. Every sixteenth patient had an ulcerated carcinoma of the breast.

Hypertrichosis lanuginosa acquisita was observed in a female patient with small-cell follicular lymphoma stage IVB. The patient had additional complaints of a burning sensation on the tip of the tongue and the sudden appearance of several seborrheic keratoses on hands and forearms. The findings are well in agreement with those reported in the literature. It is concluded that the sudden appearance of down in an area previously perceived as hairless by the patient is highly indicative of widely disseminated internal malignancy and that this phenomenon can be seen in a wide range of cancers. Possible considerations for future investigations are given.

The authors report 15 patients with tuberculosis of the breast initially misdiagnosed as breast cancers. Criteria of differentiation of breast tuberculosis were: incidence (Tbc: cancer = 1: 245), lacking in constant site of involvement, possible concomitant involvement of other organs, eg. lung tuberculosis, youth of the patient, multi-para and history of breast feeding, in isolated cases, co-existence of tuberculosis and cancer is not impossible. The authors stress meticulous care in evaluating the history, symptoms, signs and supplementary examinations. Needle biopsy and pathology are indicated.
